评估本体感觉神经肌肉促进疗法技术结合推拿与推拿或本体感觉神经肌肉促进疗法技术治疗非特异性颈痛的疗效

Objectives of Study：

Evaluating the efficacy of proprioceptive neuromuscular facilitation（PNF） techniques combined with tui na versus tuina or PNF techniques for the treatment of nonspecific chronic neck pain（NCNP）

药物成份或治疗方案详述：

试验方案分为三组，分别是PNF组、推拿组和联合组。 PNF组(本体感觉神经肌肉促进疗法技术) PNF技术包括PNF牵伸和PNF肌力训练。 PNF牵伸，持续约20min，采用保持-放松-拮抗肌收缩（HRAC）牵伸技术：斜方肌上部的拉伸、肩胛提肌的拉伸、胸锁乳突肌拉伸、枕下肌群拉伸、胸大肌拉伸、胸小肌拉伸、斜角肌的拉伸： PNF肌力训练，持续约10min，10-15次/组，3组/d。 治疗师对受试者灵活采用以下技术：拮抗肌反转、等张组合、颈部对角线运动。 推拿组 对推拿组进行中医推拿干预，干预4周，每周3次，每次30min。 本病以松筋、祛瘀、通经、整复、展筋、助动、宣散为治疗原则。包括以下手法：按揉松筋、点揉痛点、对症治疗、扳法复位、端提治乱、拔伸减压、摇法助动、侧扳助动、推擦宣散。 联合组（PNF技术联合推拿） 联合组先进行PNF技术大约30min，随后进行推拿治疗约30min，总共60min。PNF技术操作和推拿操作同前。

Description for medicine or protocol of treatment in detail：

Group 1: PNF technique group PNF techniques include PNF stretching and PNF plyometrics. Step one: PNF stretching for approx. 20 min: Hold-Relax-Antagonist Contraction (HRAC) stretching technique is used: stretching the upper trapezius, scapularis, sternocleidomastoid, suboccipital group, pectoralis major, pectoralis minor, trapezius. Step two: PNF muscle training for approx.10min, 10-15 repetitions/set, 3 sets/day: subjects received antagonist muscle inversion, isotonic combination and diagonal neck exercise. Group 2: Tuina group: The tuina group will receive TCM tuina therapy for 30 min each time. TCM Tuina intervention was used for 4 weeks, 3 times a week, 30 min each time. The following techniques were included: pressing and kneading to loosen tendons, pointing and kneading pain points, symptomatic treatment, wrenching to reset, end lifting to treat disorders, pulling and stretching to reduce pressure, shaking to help move, side wrenching to help move, and pushing and rubbing to disseminate. Group3: the PNF and tuina combined group The combined group will first receive 30 minutes of PNF technique followed by 30 minutes of tuina therapy, thus the total time will be approximately 60 minutes. PNF technique is the same as the PNF group. Tuina therapy is the same as the tuina group.

纳入标准：

1.男女不限，18-30 岁； 2.以颈痛为主诉,包括枕部、颈肩部或肩胛区域疼痛、僵硬、活动受限； 3.颈椎体格检查未见神经根受压体征； 4.病程≥3 个月； 5.自愿参与研究，愿意配合治疗并随访，签署知情同意书。

Inclusion criteria

1.Male or female with age range: 18-30 years; 2.Complaint of neck pain (VAS ≥ 30mm) including presence of pain, stiffness, and restricted movement in the occipital, neck, shoulder, or scapular regions. 3.Physical examination of the cervical spine does not exhibit indications of nerve root compression. 4.Duration of symptoms lasting for a minimum of three months. 5.Voluntary participation in the study and provision of informed consent through signature.

排除标准：

1.正在接受或过去1个月接受其他针对慢性非特异性颈痛有效的方法治疗； 2.伴有神经根、脊髓、椎动脉、交感神经等压迫的颈痛患者； 3.局部皮肤破溃，皮肤病或皮肤感觉障碍； 4.妊娠期和哺乳期女性； 5.认知障碍、感觉性失语、严重的视力或听力障碍； 6.合并有颈椎骨折、脱位、肿瘤、感染、类风湿等具体原因导致的颈部疼痛患者； 7.除颈肩部位外，伴有超过3个部位疼痛者； 8.合并严重的内科疾病，如心血管系统疾病，造血系统疾病，内分泌系统疾病。

Exclusion criteria：

1.Being treated with, or have been treated in the past 1 month with, another effective method for NCNP; 2.Patients with neck pain accompanied by compression of nerve roots, spinal cord, vertebral arteries, sympathetic nerves, etc; 3.Patients with localised skin breakdown, skin disease or skin sensory impairment; 4.Pregnant and breastfeeding women; 5.Patients with cognitive impairment, sensory aphasia, severe visual or hearing impairment; 6.Patients with a combination of cervical fractures, dislocations, tumours, infections, rheumatoid and other specific causes of neck pain; 7.Patients with pain in more than 3 areas other than the neck and shoulder area; 8.Patients with a combination of serious medical conditions such as cardiovascular, haematopoietic and endocrine diseases.

随机化序列将由随机数发生器（IBM SPSS 统计 27.0 版软件；IBM 公司，美国纽约州阿蒙克市）生成，并用不透明信封按顺序编号发送给治疗师。治疗师将按顺序打开随机分配信封，并据此分配参与者。符合条件的患者将被随机分配到推拿组、PNF 组或 PNF 和推拿联合组，每组 20 人。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The randomization sequence will be generated by a random number generator (IBM SPSS Statistics version 27.0 software; IBM Corp., Armonk, NY, USA), which will be sent to a therapist with an opaque envelope numbered sequentially. The therapist will sequentially open random-allocation envelopes and allocate the participants accordingly. Eligible patients will be randomized to the tuina group, the PNF group, or the PNF and tuina combined group with 20 patients in each group.
