研究目的：

在重症IAD患者中，探讨相对于结构化皮肤护理方案，中药熨烫联合结构化皮肤护理方案对于IAD严重程度、腹泻程度、IAD相关性疼痛及相关不良事件的效果。

Objectives of Study：

In patients with severe IAD, this study explores the effects of a combination of Chinese herbal fomentation and a structured skin care regimen compared to a structured skin care regimen alone on the severity of IAD, the degree of diarrhea, IAD-related pain, and associated adverse events.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）年龄≥18周岁； （2）预估ICU入住时间≥7天； （3）大便或尿失禁，或双便失禁； （4）患者已产生失禁相关性皮炎； （5）患者或家属知情同意自愿参加本研究。

Inclusion criteria

(1) Age ≥ 18 years old; (2) Planed ICU stay ≥ 7 days; (3) Being incontinent fo urine and stool; (4) With clinical signs of IAD (incontinence-associated dermatitis and its severity IADS); (5) Written informed consent (signed by the participant or a legal representative).

排除标准：

（1）妊娠期、哺乳期或经期者； （2）血流动力学不稳定等病情危重者； （3）熨烫部位及发生IAD部位的皮肤存在水肿、烧伤、造口、破损、瘢痕、感染或其他皮肤疾病者； （4）正在使用大便排泄物收集者（如大便引流装置）； （5）过敏试验显示对中药熨烫包、皮肤清洗产品、皮肤保护产品过敏者； （6）若为清醒患者，则排除严重糖尿病、截瘫、偏瘫等感觉神经功能障碍者； （7）正参与其他课题者。

Exclusion criteria：

(1) Patients who are pregnant breastfeeding or menstruating; (2) Patients with severe conditions such as hemodynamic instability; (3) Patients with edema burns stomas skin breaks scars infections or other skin diseases at the fomentation site or the area affected by IAD; (4) Patients currently using fecal collection devices (such as fecal drainage devices); (5) Patients who show allergic reactions to the Chinese herbal fomentation pack skin cleansing products or skin protection products; (6) For conscious patients those with severe diabetes paraplegia hemiplegia or other sensory neuropathies are excluded; (7) Patients who are currently participating in other research studies.

采用IBM SPSS Statistics software (version 26.0; IBM, Armonk, NY, USA)产生随机序列编码。按照1:1的比例将患者随机分到干预组和对照组。将产生的随机序列分组结果打印出来并进行编号，将之放入不透光的空白信封密封备用。研究实施开始时，将这些信封转给本研究招募人员。研究招募人员负责招募研究对象并让其签署知情同意书，按照随机序列顺序打开密封的信封以确定研究对象的分组情况，然后将组别信息转给本研究干预人员。

Randomization Procedure (please state who generates the random number sequence and by what method)：

A random sequence code was generated using IBM SPSS Statistics software (version 26.0; IBM Armonk NY USA). Patients were randomly assigned to the intervention group and the control group in a 1:1 ratio. The generated random sequence for group allocation was printed out and numbered then placed into opaque sealed envelopes. At the start of the study implementation these envelopes were transferred to the study recruiters. The recruiters were responsible for enrolling participants and obtaining their informed consent. They would then open the sealed envelopes in the order of the random sequence to determine the group assignment of each participant and subsequently pass this information to the intervention personnel of the study.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection and documentation will be jointly conducted by the frontline research implementers and the data manager. Relevant information will be recorded in the Case Report Forms (CRFs) and subsequently organized into an Excel spreadsheet. The electronic data will then be transferred to ResMan online platform to ensure secure and systematic handling.
