研究目的：

1）发挥中医药防治特色，探索基于中医药理论的医养结合模式。 2）评价在社区卫生服务机构中运用该模式的效果。

Objectives of Study：

1) Give full play to the characteristics of traditional Chinese medicine prevention and treatment, and explore a medical care integrated model based on traditional Chinese medicine theory. 2) Evaluate the effect of applying this model in community health service institutions.

药物成份或治疗方案详述：

本研究为前瞻性研究。本项目研究拟以传统医学养生观、疾病观、用药观等核心理念为视角，对中国传统老年医学文献进行系统梳理，从生活起居、运动健身、饮食营养、药物调补、防治未病等方面归纳整理，并通过咨询专家意见，建立一套简单易行，易于在基层医疗机构推广的中医特色日间照料医养结合服务模式；在高碑店、孙河2个社区卫生服务中心实施老年人中医医养结合模式，选取本辖区内240名符合入组条件的老年人，以中医特色日间照料医养结合方案为暴露因素，分为中医医养结合组和常规干预组，两个组的老年人均提供常规日常诊疗、体检等服务，中医医养结合组老年人进行中医特色日间照料医养结合服务2周，其内容包括：①教授并每日练习八段锦养生操（根据患者具体身体情况选择立式或坐式八段锦）、老年健康操；②每日根据老年人需要进行中药足浴、刮痧、艾灸、拔罐、蜡疗、理疗等中医养生保健项目；③根据老年人中医体质辨识给予中药代茶饮膳食指导；④定期进行中医养生保健讲座、防跌倒知识讲座、老年保健知识讲座等相关活动。2周后每月进行强化治疗指导、健康讲座和随访，对比两组各项指标，评价该模式效果。

This study is a prospective study. The research of this project intends to systematically sort out Chinese traditional geriatrics literature from the perspective of the core concepts of traditional medical health, disease, and medication, from the aspects of daily life, exercise and fitness, diet and nutrition, drug supplementation, prevention and treatment of disease, etc. Summarize and organize, and through consultation with experts, establish a simple, easy-to-use and easy-to-promote service model of integrated day care and medical care with traditional Chinese medicine in primary medical institutions; implement the elderly in Gaobeidian and Sunhe community health service centers The model of integrated traditional Chinese medicine, medical and elderly care, selected 240 elderly people in this jurisdiction who meet the requirements for inclusion in the group, and used the characteristic day care integrated medical care program of traditional Chinese medicine as the exposure factor, and divided into two groups: The elderly are provided with routine daily diagnosis and treatment, physical examination and other services. The elderly in the integrated traditional Chinese medicine and medical care group will be provided with the characteristic day care and integrated medical care services of traditional Chinese medicine for 2 weeks. The content includes: ①Teacher and practice Ba Duan Jin health exercises every day (according to the patient's specific Choose vertical or seated Baduanjin for physical condition), health exercises for the elderly; ②Do traditional Chinese medicine health care items such as foot bath, scraping, moxibustion, cupping, wax therapy, physical therapy, etc. according to the needs of the elderly; ③According to traditional Chinese medicine for the elderly Physique identification is provided for the dietary guidance of Chinese medicine instead of tea; ④Regularly carry out traditional Chinese medicine health care lectures, fall prevention knowledge lectures, elderly health care knowledge lectures and other related activities. After 2 weeks, intensive treatment guidance, health lectures and follow-up were conducted every month to compare the indicators of the two groups to evaluate the effect of the model.

纳入标准：

入选标准 同时具备以下3项条件者方可进入研究： ①年龄60-85岁； ②本试验开始前6月未参加过医养结合相关研究者； ③受试者神智精神正常，自愿参加研究并签署知情同意书。

Inclusion criteria

The selection criteria are those who meet the following 3 conditions at the same time to enter the study: 1. 60-85 years old; 2. Those who have not participated in the research related to the integration of medical and elderly care in the 6 months before the start of the trial; 3. 排除标准：

有以下之一项者不入选： ①对本项研究不理解、不按时配合随访者； ②有精神疾患、不能很好合作者； ③恶性肿瘤及血液病患者； ④肢体功能障碍影响行为干预活动实施者。
