研究目的：

艾灸治疗对下腰痛有明确疗效，但艾灸操作标准化程度较低且几乎完全依赖人工，治疗的规范化程度有待提高，为此研发一款“智能雷火灸”机器人，并将其应用于下腰痛的临床治疗中，对比人工艾灸组，以验证其临床疗效和安全性，以期为艾灸智能化提供新的思路。

Objectives of Study：

Moxibustion treatment has a clear effect on lower back pain, but the standardized degree of moxibustion operation is low and almost depends on manual work, and the standardized degree of treatment needs to be improved. Therefore, a "intelligent lightning fire moxibustion" robot is developed and applied to the clinical treatment of lower back pain. Compared with the artificial moxibustion group, its clinical efficacy and safety are verified, so as to provide a new idea for intelligent moxibustion.

药物成份或治疗方案详述：

“智能雷火灸”组 1.选穴： ? 取穴：参照高树中主编的全国高等中医药院校规划教材《针灸治疗学》，取肾俞、大肠俞、腰阳关、阿是穴。 定位：参照《针灸经穴定位国家标准》（国家GB1234-90标准制定）相关规定进行选取。肾俞：在腰部，当第2腰椎棘突下，旁开1.5寸。每次选双侧2穴。大肠俞：在腰部，当第4腰椎棘突下，旁开1.5寸。每次选双侧2穴。腰阳关：在腰部，当后正中线上，第4腰椎棘突下凹陷中。 2.疗程： 治疗10次，治疗时间为30min。 （我院每周，每位患者做5次，患者治疗通常2-3周） 3.艾灸机器人操作的模式 ? 进入智能雷火灸“自由艾灸”的设定页面，使用摇杆操控六自由度机械臂在患者腰背部进行穴位定位——肾俞、大肠俞、腰阳关、阿是穴。定位后，点燃并装置雷火灸艾条，在操作页面上设定艾灸操作——每穴10min（先回旋灸5min，后雀啄灸5min），4穴位依次艾灸，共40min；回旋灸的操作法为：平面回旋灸，机械臂夹持点燃的雷火灸条在上述穴位上方作平行往复回旋施灸，回旋半径为4cm；雀啄灸的操作法为：机械臂夹持点燃的雷火灸条对准所选穴位，采用类似麻雀啄食般的一起一落忽近忽远的手法施灸，升降幅度为5cm，给以较强烈的温热刺激；灸距根据患者的耐受温度手动调节。 ? 注意事项：随时巡视病房，观察皮肤情况和患者耐热程度，如有不适，立即停止治疗，对症处理。 人工艾灸组：采用人工操作雷火灸进行治疗，疗程、穴位、治疗时长等，均同上。

"Intelligent Thunder Fire Moxibustion" Group 1. Point selection: Selection of acupoints: referring to the planned textbook "Acupuncture and Moxibustion Therapeutics" compiled by Gao Shuzhong in charge of national colleges and universities of traditional Chinese medicine, Shenshu, Dalianshu, Yaoyangguan and Ashi acupoints were selected. Positioning: selected according to the relevant provisions of the National Standard for the Positioning of Acupuncture and Moxibustion Channels and Points (formulated according to the national GB1234-90 standard). Shenshu: At the waist, when the second lumbar spine is under the process, the side opening is 1.5 inches. Select two points on both sides each time. Large intestine shu: at the waist, when the fourth lumbar spine is under the spinous process, the lateral opening is 1.5 inches. Select two points on both sides each time. Waist Yang Guan: at the waist, on the back midline, in the depression under the fourth lumbar spinous process. 2. Course of treatment: The treatment time was 30 minutes. (Each patient is treated 5 times a week in our hospital, usually 2-3 weeks) 3. Operation mode of moxibustion robot Enter the setting page of "Free Moxibustion" of intelligent lightning fire moxibustion, and use the rocker to control the 6-DOF mechanical arm to locate the points on the patient's lumbar back - Shenshu, Dachangshu, Yaoyangguan, Ashi. After positioning, ignite and install the lightning fire moxibustion stick, and set the moxibustion operation on the operation page - 10 min for each point (first convolute moxibustion for 5 min, then peck moxibustion for 5 5min), and moxibustion at four points in turn for 40 5min in total; The operation method of whirling moxibustion is: plane whirling moxibustion, the mechanical arm holding the ignited thunder fire moxibustion strip makes parallel whirling moxibustion above the above points, with a whirling radius of 4 cm; The operation method of bird pecking moxibustion is as follows: the mechanical arm holds the ignited thunder fire moxibustion stick to the selected acupoint, and uses a method similar to the sparrow pecking to apply moxibustion, with a rise and fall range of 5cm, giving a strong warm stimulus; The moxibustion distance is manually adjusted according to the patient's tolerance temperature. Precautions: Patrol the ward at any time to observe the skin condition and the patient's heat resistance. If you feel unwell, stop treatment immediately and treat symptomatically. Artificial moxibustion group: artificial thunder fire moxibustion was used for treatment. The course of treatment, acupoints and treatment duration were the same as above.

纳入标准：

纳入标准： ①与下腰痛诊断标准和寒湿型腰肌劳损辨证标准相符合； ②性别不限，年龄控制在25岁至70岁之间； ③近2周前未进行任何腰痛相关的中西医治疗;? ④病程控制在８年内； ⑤知情同意，愿意配合全程治疗； ⑥同意记录量表评分，自愿参加本试验。 ⑦患者预留至少一种联系方式,便于接受随访; ⑧为降低脱落率，优先纳入住院患者。 同时满足以上7项者方可纳入本研究。

Inclusion criteria

Inclusion criteria: ① It is consistent with the diagnostic criteria of low back pain and the syndrome differentiation criteria of cold damp type lumbar muscle strain; ② The age should be between 25 and 70 years old, regardless of gender; ③ No traditional Chinese and western medicine treatment related to low back pain was carried out nearly 2 weeks ago; ④ The course of disease was controlled within 8 years; ⑤ Informed consent, willing to cooperate with the whole process of treatment; ⑥ Agree to record the score of the scale and voluntarily participate in this trial. ⑦ At least one contact way shall be reserved for patients to receive follow-up; ⑧ In order to reduce the dropout rate, priority was given to inpatients. Only those who meet the above 7 items can be included in this study.

排除标准：

①患者不符合纳入标准； ②患者处于妊娠期或哺乳期； ③患者对灸烟过敏，难以接受艾灸治疗； ④患者患有腰椎结核，骨癌等疾病，影响实验结果； ⑤患者患有血液和免疫系统疾病； ⑥患者心、肝、肾功能不全； ⑦患者患有精神疾病，无法配合艾灸。

Exclusion criteria：

① Patients do not meet the inclusion criteria; ② The patient is in pregnancy or lactation; ③ The patient is allergic to moxibustion smoke and difficult to accept moxibustion treatment; ④ The patient suffered from lumbar tuberculosis, bone cancer and other diseases, which affected the experimental results; ⑤ The patient has blood and immune system diseases; ⑥ Heart, liver and kidney dysfunction; ⑦ The patient suffers from mental illness and cannot cooperate with moxibustion.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始观察记录，将数据及时、完整、正确、清晰地载入病例报告表，录入采用相应的数据库系统双人双机录入，之后对数据库进行两遍比对。电子数据文件分类保存，并有多个备份保存于不同磁盘或者记录介质上，妥善保存，防止损坏。
