研究目的：

应用随机对照的研究方法，观察手机APP进行慢性肾脏病3-5期患者中西医营养管理的有效性和可行性。

Objectives of Study：

A randomized controlled method was used to observe the effectiveness and feasibility of traditional Chinese and western medicine nutrition management for patients with chronic kidney disease stage 3-5 using mobile APP.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）年龄18-80岁，性别不限。 （2）临床诊断为慢性肾脏病3-5期非透析的患者。 （3）掌握智能手机的使用技能，能自行安装使用手机APP。 （4）签署知情同意书者。

Inclusion criteria

1. Aged 18-80 years old male and female. 2. Clinical diagnosis of chronic kidney disease 3-5（non-dialysis）. 3. Master the skills of using smart phones, can install and use mobile apps by selves. 4. Sign the written informed consent form.

排除标准：

（1）孕妇或哺乳期患者。 （2）合并有活动期恶性肿瘤，肝硬化失代偿期、消化道出血、造血系统严重原发性疾病者；合并严重心肺疾病者；严重高血压（收缩压≥200，舒张压≥120）并且血压难以控制者。 （3）精神病患者。 （4）正在参加其他研究者。

Exclusion criteria：

1. Pregnant or lactating patients. 2. Patients with active malignant tumors, decompensated liver cirrhosis, gastrointestinal bleeding, severe primary diseases of the hematopoietic system; patients with severe cardiopulmonary disease; severe hypertension (systolic blood pressure >= 200, diastolic blood pressure >= 120) Patients whose blood pressure is difficult to control. 3. Mental disorder. 4. Those who have participated in or are undergoing other clinical trials.

Data management using the Chronic Diseases Management System of Guangdong Provincial Hospital of Chinese Medicine

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

（1）完成的病例观察表由监查员审查后收回，随后相关人员进行数据处理。病例报告表的数据采用双人双份录入，经核查确认无误后锁定数据库。 （2）研究者应当使资料保存完整，有固定地方存放并落锁保管，以备今后查看。资料保存至少5年。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. The completed case observation form was reviewed by the inspector, and the project team members then processed the data. The data in the case report form is entered in double-double, and the database is locked after verification and confirmation. 2. The researcher should keep the data intact, store it in a fixed place, and lock it for future reference. The data is kept for at least 5 years.
