研究目的：

腰痛是全球最常见的造成功能障碍的原因，造成巨大的社会及经济负担，且心理特征在LDH腰痛的发生发展中很重要，心理共病是影响腰痛的重要预后因素。但目前对心理因素在LDH或腰痛病例中的作用了解有限。针对腰突症焦虑状态的临床现实问题，为了发展改进更完善的LDH治疗和预防策略，拟运用临床随机对照试验验证推拿干预LDH焦虑状态、疼痛与单胺类神经递质的临床效应，确定推拿改善LDH焦虑、疼痛及功能障碍的相关性。

Objectives of Study：

Low back pain is the most common cause of dysfunction in the world, causing huge social and economic burden. Psychological characteristics are important in the development of low back pain in LDH, and psychological comorbidity is an important prognostic factor for low back pain.However, there is limited understanding of the role of psychological factors in LDH or low back pain cases.In view of the clinical reality of anxiety state in lumbar process disease, in order to develop improved and more perfect treatment and prevention strategies for LDH, clinical randomized controlled trials are planned to verify the clinical effects of massage intervention on LDH anxiety state, pain and monoamine neurotransmitters, and determine the correlation between massage intervention and LDH anxiety, pain and dysfunction.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

纳入标准 （1）符合上述腰椎盘突出症诊断标准；影像学见腰椎椎体、关节突关节、腰椎间盘等退行性改变；反复腰痛≥3个月。 （2）伴有焦虑状态，且焦虑自评量表（SAS）评分≥50，不需抗焦虑西药治疗的焦虑状态患者。 （3）年龄18-65周岁，男女性别不限。 （4）符合中医郁病诊断标准。 （5）近2周内未服用其他抗焦虑药物者。 （6）自愿参加本研究并签署知情同意书者。

Inclusion criteria

Inclusion criteria (1) meeting the above diagnostic criteria for lumbar disc herniation;Imaging showed degenerative changes of lumbar vertebrae, facet joints, and lumbar intervertebral disc.Recurrent low back pain ≥3 months.(2) Patients with anxiety state, self-rating Anxiety scale (SAS) score ≥50, and do not need anti-anxiety western medicine treatment.(3) Aged 18-65, male or female.(4) In line with the diagnostic criteria of depression in traditional Chinese medicine.(5) Patients who have not taken other anti-anxiety drugs in the past 2 weeks.(6) Participants who voluntarily participated in the study and signed the informed consent form.

排除标准：

2.排除标准 （1）不符合诊断标准和纳入标准者。 （2）合并其它腰椎重大疾病，如肿瘤、结核等。 （3）外伤、手术导致的腰椎急性损伤疼痛。 （4）伴有心血管、肺、肾、造血系统等重大疾病者。 （5）精神病患者。 （6）孕妇或哺乳期妇女。 （7）严重焦虑且需口服抗焦虑西药治疗。 （8）无论何原因对于推拿不能接受者。

Exclusion criteria：

Exclusion criteria (1) Those who do not meet the diagnostic criteria and inclusion criteria.(2) Combined with other major lumbar diseases, such as tumor and tuberculosis.(3) Acute injury and pain of lumbar spine caused by trauma and operation.(4) accompanied by cardiovascular, lung, kidney, hematopoietic system and other major diseases.(5) mental patients.(6) Pregnant or lactating women.(7) Severe anxiety and need to be treated with oral anti-anxiety western drugs.(8) Massage can not be accepted for any reason.

由统计人员采用统计软件成随机数字表，然后将随机数字表发放给第三方研究人员进行分配隐藏。第三方人员将数字表装入不透明的信封，每个信封都标记有时间和签名，然后统一交给研究协调人员。研究协调人员对于潜在的腰突症受试者电话联系咨询是否继续愿意参加临床试验，并按照随机号码将患者分配到随机号码将受试者按照1:1分配入推拿治疗组和牵引对照组。

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

对研究结局的评估采用盲法。 采用中国临床试验注册中心网上病例报告表数据管理与分析平台进行电子化数据管理。病例报告表（CRF）的填写与数据的录入：数据录入与管理在该平台（ResMan Research Manager，http://www.medresman.org/login.aspx）进行。该系统集成了电子病例报告表（eCRF）构建、数据录入、数据锁定及导出，能够科学的对试验数据进行跟踪管理和实时监控。

Study outcomes were assessed in a blinded manner.The online case report form data management and analysis platform of China Clinical Trial Registry was used for electronic data management.Case report form (CRF) and fill in the data input, data entry and management on the platform (ResManResearchManager, http://www.medresman.org/login.aspx).The system integrates electronic case report form (eCRF) construction, data entry, data locking and export, which can track and manage the test data scientifically and monitor it in real time.
