研究目的：

1.以电针膈神经刺激治疗为暴露因素，观察脑卒中后肺炎患者中暴露组（电针膈神经刺激治疗+呼吸康复训练治疗）与非暴露组（呼吸康复训练治疗）肺炎改善情况、排痰能力、呼吸功能恢复及疗效评价。 2.观察脑卒中后肺炎患者中暴露组（电针膈神经刺激治疗+呼吸康复训练治疗）与非暴露组（呼吸康复训练治疗）在改善膈肌活动度及膈肌厚度的疗效差异。

Objectives of Study：

1. Using electroacupuncture diaphragmatic nerve stimulation therapy as the exposure factor, observe the improvement of pneumonia, sputum excretion ability, respiratory function recovery, and therapeutic effect evaluation in the exposed group (electroacupuncture diaphragmatic nerve stimulation therapy+respiratory rehabilitation training therapy) and the non exposed group (respiratory rehabilitation training therapy) of post-stroke pneumonia patients. 2. Observe the difference in the efficacy of improving diaphragm activity and thickness between the exposed group (electroacupuncture diaphragmatic nerve stimulation treatment+respiratory rehabilitation training treatment) and the non exposed group (respiratory rehabilitation training treatment) in patients with post-stroke pneumonia.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）于2023年1月1日至2024年9月15日就诊于广东药科大学附属第一医院中西医结合科的住院患者，且符合脑卒中后肺炎诊断标准且脑卒中病史病程在1个月以上，遗留长期卧床且言语交流功能障碍或意识功能障碍患者。 （2）年龄≥18岁，性别不限。 （3）NIHSS评分≥3分，GCS评分＞3分 （4）住院期间接受呼吸康复训练及电针膈肌刺激治疗10次以上患者。 同时符合以上条件者，方可纳入。

Inclusion criteria

(1) A hospitalized patient who visited the Department of Integrated Traditional Chinese and Western Medicine at the First Affiliated Hospital of Guangdong Pharmaceutical University from January 1, 2023 to September 15, 2024, meets the diagnostic criteria for post-stroke pneumonia, has a history of stroke for more than one month, and remains bedridden for a long time with speech or consciousness impairments. (2) Age ≥ 18 years old, gender not limited. (3) NIHSS score ≥ 3 points, GCS score>3 points (4) Patients who receive respiratory rehabilitation training and electroacupuncture diaphragmatic stimulation treatment more than 10 times during hospitalization. Only those who meet the above conditions can be included.

排除标准：

（1）新发或进展性脑卒中患者，或病程不足1个月； （2）特殊感染者（如肺结核、甲型H1N1、新冠病毒感染、禽流感、其他非 典型性肺炎等）、支气管扩张、肺部恶性病变者以及使用机械通气的患者； （3）患有精神或心理疾病者； （4）存在急性心力衰竭、休克等急危重症患者； （5）研究期间接受了除本研究中气道廓清技术、呼吸训练、吞咽功能训练 的其他呼吸康复训练治疗患者； （6）同时参与其他课题研究者。

Exclusion criteria：

(1) Newly diagnosed or progressive stroke patients, or with a course of less than one month; (2) Special infected persons (such as tuberculosis, A H1N1, COVID-19 infection, avian influenza, other non Patients with typical pneumonia, bronchiectasis, malignant lung lesions, and those using mechanical ventilation; (3) Individuals with mental or psychological disorders; (4) Patients with acute heart failure, shock and other critical illnesses; (5) During the research period, in addition to the airway clearance technique, respiratory training, and swallowing function training mentioned in this study Other respiratory rehabilitation training treatments for patients; (6) Simultaneously participating in other research projects.

回顾性队列研究方法，以脑卒中后恢复期（包括缺血性脑卒中及出血性脑卒中）并发肺炎患者为研究对象，纳入暴露组及非暴露组两组研究对象，每组拟纳入60例，其中非暴露组采用目前西医公认有效的呼吸康复治疗，暴露组同时接受呼吸康复治疗及电针膈神经刺激治疗.

A retrospective cohort study method was used to study patients with pneumonia in the recovery period after stroke (including ischemic stroke and hemorrhagic stroke). Two groups of study subjects, exposed group and non exposed group, were included, with 60 cases planned to be included in each group. The non exposed group received respiratory rehabilitation treatment, which is currently recognized as effective in Western medicine, while the exposed group received both respiratory rehabilitation treatment and electroacupuncture diaphragmatic nerve stimulation treatment

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

在广东药科大学第一附属医院电子病历系统中，根据上述纳入标准确定目标患者人群。由经过专门培训的医生（熟练操作电子病历住院系统）检索这部分人群的病历，提取以下变量信息： （1）基线特征：年龄、性别、发病时间、既往史、家族史（2）脑卒中严重程度：采用NIHSS及GCS评分；（3）肺炎严重程度：CPIS；（4）临床观察指标：血常规、感染三项（CRP、IL-6、PCT）、血气分析、胸片、膈肌彩超；（5）呼吸康复及电针干预次数；（6）干预前NIHSS、GCS、CPIS、WBC、CRP、IL-6、PCT、膈肌活动度、膈肌厚度，10次干预后NIHSS、GCS、CPIS、WBC、CRP、IL-6、PCT、膈肌活动度、膈肌厚度，电针膈神经刺激时膈肌活动度、膈肌厚度；（7）并发症。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

In the electronic medical record system of the First Affiliated Hospital of Guangdong Pharmaceutical University, the target patient population is determined based on the inclusion criteria mentioned above. Specially trained doctors (proficient in operating electronic medical record hospitalization systems) will retrieve the medical records of this population and extract the following variable information: (1) Baseline characteristics: age, gender, onset time, medical history, family history. (2) Severity of stroke: NIHSS and GCS scores were used; (3) Severity of pneumonia: CPIS; (4) Clinical observation indicators: blood routine, three items of infection (CRP, IL-6, PCT), blood gas analysis, chest X-ray, diaphragmatic ultrasound; (5) Number of respiratory rehabilitation and electroacupuncture interventions; (6) NIHSS before intervention GCS、CPIS、WBC、CRP、IL-6、PCT、 Diaphragmatic activity and thickness, NIHSS after 10 interventions GCS、CPIS、WBC、CRP、IL-6、PCT、 Diaphragm activity and thickness, diaphragm activity and thickness during electroacupuncture of the phrenic nerve stimulation; (7) Complications.
