研究目的：

对 86 例慢性阻塞性肺疾病急性加重期（acute exacerbation of chronic obstructive pulmonary disease,AECOPD）机械通气患者进行双盲、随机、平行对照的临床研究，评价固本化浊方在 AECOPD 撤机中的作用及其安全性。

Objectives of Study：

A double-blind randomized parallel controlled clinical study was conducted on 86 patients with acute exacerbation of chronic obstructive pulmonary disease undergoing mechanical ventilation to evaluate the role and safety of Guben Huazhuo Formula in AECOPD weaning.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

西医诊断标准 参考中华医学会呼吸病学分会慢性阻塞性肺疾病学组于 2021 年发表的《COPD 诊治 指南（2021 修订版）》的标准制定： （1）有慢性咳嗽或咳痰、呼吸困难、反复下呼吸道感染史； （2）和（或）有慢阻肺危险因素（如吸烟、职业性粉尘、年龄≥40 岁、支气管哮喘、 气道高反应性、低体重指数等）暴露史的患者； （3）肺功能检查表现为持续性气流受限，吸入支气管舒张剂后 FEV1/FVC＜70%。 中医诊断标准 参考慢性阻塞性肺疾病中医证候诊断标准（2011 版）中肺脾两虚、肺肾两虚，痰浊 阻肺以及兼血瘀证制定: （1）咳嗽或喘息、气短，动则加重；（2）神疲、乏力或自汗，动则加重；（3）痰 多、白黏或呈泡沫状；（4）纳呆、腹胀、便溏或便秘;（5）腰膝酸软；（6）舌体胖大或有齿痕，舌质淡黯、苔薄白或薄黄，脉以沉细弱为主；（7）舌下络脉迂曲、粗乱。 具备（1）、（2）、（3）中的 2 项，加（4）、（5）、（6）、（7）中的 2 项 纳入标准 ①符合以上诊断标准； ②年龄在 35~80 岁之间（含 35 和 80 岁），性别不限； ③入住 ICU 的 COPD 机械通气的患者；

Diagnostic criteria for Western medicineReferring to the diagnosis and treatment of COPD published by the Chronic Obstructive Pulmonary Disease Group of the Respiratory Branch of the Chinese Medical Association in 2021Standard formulation for the Guidelines (Revised 2021):(1) Have a history of chronic cough or sputum production, difficulty breathing, and recurrent lower respiratory tract infections;(2) And/or have risk factors for chronic obstructive pulmonary disease (such as smoking, occupational dust, age ≥ 40 years old, bronchial asthma, etc.)Patients with a history of exposure to airway hyperresponsiveness, low body mass index, etc;(3) Pulmonary function examination showed persistent airflow restriction, with FEV1/FVC<70% after inhaling bronchodilators.Diagnostic criteria for traditional Chinese medicineRefer to the diagnostic criteria for traditional Chinese medicine syndromes of chronic obstructive pulmonary disease (2011 edition), which include lung and spleen deficiency, lung and kidney deficiency, and phlegm turbidityDevelopment of obstructive pulmonary disease and concurrent blood stasis syndrome:(1) Coughing, wheezing, shortness of breath, and increased movement; (2) Mental fatigue, fatigue, or self sweating, and increased movement; (3) PhlegmMany, white sticky or foam like; (4) Nausea, bloating, loose stools or constipation; (5) Lumbar and knee soreness and weakness; (6) The tongue is plump or has tooth marks, the tongue is dull, the coating is thin white or yellow, and the veins are mainly thin and weak; (7) The sublingual meridians are tortuous and rough.Having 2 items from (1), (2), and (3), plus 2 items from (4), (5), (6), and (7)Inclusion criteria① Meets the above diagnostic criteria;② Age range from 35 to 80 years old (including 35 and 80 years old), regardless of gender;③ COPD mechanically ventilated patients admitted to the ICU;

排除标准 ①合并有心脑血管、肝、肾和造血系统等严重危及生命的原发性疾病以及精神病患者； ②对固本化浊方成分过敏者； ②妊娠和哺乳期患者；
