研究目的：

本研究在系统评价和质性访谈基础上，发现目前脑卒中尿潴留的护理多集中于外科导尿、针灸、电针等侵入性护理方案，尚缺乏简便效廉、舒适度高的脑卒中尿潴留患者管理方案。采用现象学质性研究方法，探究卒中后尿潴留患者的病耻感真实体验，深刻了解其应对病耻感情绪的策略及社会支持需求，以期增进临床医护人员及社会大众对该群体隐性情绪的认知。同时，根据脑卒中尿潴留患者的临床症状和实证研究，构建适合脑卒中患者的尿潴留护理方案，结合舒适护理理论，验证其理论框架的应用在脑卒中尿潴留患者当中的有效性。具体研究目标：①掌握脑卒中尿潴留患者的真实感受和需求；②充分考虑患者感受和困扰，结合相关文献和指南，构建基于舒适理论的脑卒中尿潴留患者的多维度护理方案；③验证该方案的可行性及有效性。

Objectives of Study：

Based on systematic review and qualitative interviews this study found that the current nursing of urinary retention in stroke mostly focuses on invasive nursing programs such as surgical catheterization acupuncture and electroacupuncture and there is a lack of simple inexpensive and comfortable management programs for patients with urinary retention after stroke. In order to enhance the understanding of clinical medical staff and the public this paper explores the real experience of stigma in patients with urinary retention after stroke and deeply understands their strategies and social support needs for stigma. At the same time according to the clinical symptoms and empirical studies of stroke urinary retention patients a urinary retention nursing program suitable for stroke patients was constructed and the effectiveness of the application of the theoretical framework in stroke urinary retention patients was verified by combining the comfort nursing theory. 观察组B在实施过程中，小组成员在干预前可通过向患者咨询护理需求或提供感兴趣知识使患者放松，增强对医务人员的信任感。实际干预时间以不影响患者护理与治疗及正常休息为前提，可根据患者实际情况进行调整。干预过程中鼓励家属积极参与配合，提供有关症状评估、艾灸干预等观察与实施要点，并在干预后耐心解答家属及患者相关疑问，给予患者应对尿潴留症状的信心，加快患者康复进程，重返社会。

Description for medicine or protocol of treatment in detail：

In Group B's observation, during the implementation process, team members can help patients relax and build trust in medical staff by consulting them about their nursing needs or providing interesting information before the intervention. The actual intervention time can be adjusted based on the patient's situation, as long as it doesn't interfere with their care, treatment, or normal rest. Family members are encouraged to actively participate and assist during the intervention, providing key points on symptom assessment and moxibustion interventions. After the intervention, they should patiently answer any questions from the family and patients, helping to boost the patients' confidence in coping with urinary retention symptoms and speeding up their recovery process so they can reintegrate into society.

纳入标准：

（1）符合脑卒中、尿潴留中西医诊断标准的患者； （2）病人发病前排尿正常，卒中病情稳定后仍存在尿潴留的症状； （3）留置尿管已经拔除者； （4）年龄18-75岁； （5）膀胱残余尿量＞100ml； （6）自愿签署知情同意书；

Inclusion criteria

(1) Patients who meet the diagnostic criteria of stroke and urinary retention in traditional Chinese and Western medicine; (2) The patient had normal urination before the onset of the disease and still had symptoms of urinary retention after the stroke was stable; (3) Those who have had their indwelling urinary catheter removed; (4) Age 18-75 years old; (5) Bladder residual urine volume> 100ml; (6) voluntarily sign the informed consent form;

排除标准：

（1）非脑卒中疾患引发的尿潴留或基础排尿障碍者； （2）合并泌尿系统器质性病变或者有机械性梗阻病变一类的患者； （3）伴有其他重病及原发病的患者，如体位性低血压、肝肾功能不全、严重脊髓损伤、前列腺癌等脏器疾病患者； （4）实验期间皮肤有红肿、破损感染者； （5）伴意识障碍、严重的认知障碍、语言障碍等； （6）治疗过程中出现病情加重或死亡，无法继续进行实验者； （7）接受其他疗法，或参与其他相关研究者；

Exclusion criteria：

(1) Patients with urinary retention or underlying urination disorders not caused by stroke; (2) Patients with organic lesions of the urinary system or mechanical obstructive lesions; (3) Patients with other serious diseases and primary diseases such as orthostatic hypotension liver and kidney insufficiency severe spinal cord injury prostate cancer and other organ diseases; (4) Those whose skin is red swollen broken and infected during the experiment; (5) Accompanied by consciousness disorder severe cognitive impairment language impairment etc.; (6) Those who have aggravated or died during the treatment and are unable to continue the experiment; (7) Those who receive other therapies or participate in other related research;
