研究目的：

以ACR指南强推荐的太极拳训练为对照，采用开放标签、随机对照临床的研究设计，以评价四步一牵点穴理筋术治疗KOA的近期与远期治疗效应

Objectives of Study：

An open-label randomized controlled clinical study will be conducted using Tai Chi trainingstrongly recommended by the American College of Rheumatology guidelinesas a control to evaluate the short-term and long-term therapeutic effects of the Four-Step One-Traction Acupoint Tendon Manipulation in the treatment of Knee Osteoarthritis.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

同时满足下述6项的患者方可纳入本研究内： ①符合上述KOA的诊断标准; ②年龄范围40-75周岁; ③X线Kellgren-Lawrence（KL）评级为II级或III级;（KL分级X线特征详见图2） ④疼痛NRS总分≥4分; ⑤若患者双腿均符合KOA选择较重一侧膝关节进行研究; ⑥自愿参加本研究并签署知情同意书者。

Inclusion criteria

Patients who meet all of the following six criteria will be eligible for inclusion in this study: Diagnosis: Meet the aforementioned diagnostic criteria for Knee Osteoarthritis (KOA). Age Range: Between 40 and 75 years old. Radiographic Grading: X-ray Kellgren-Lawrence (KL) grade II or III (see Figure 2 for radiographic characteristics of KL grading). Pain Assessment: Total pain score on the Numeric Rating Scale (NRS) ≥ 4. Affected Knee Selection: If both knees meet the KOA criteria the more severely affected knee will be selected for the study. Informed Consent: Voluntary participation with a signed informed consent form.

排除标准：

凡符合下述任意一项的患者在本研究内均予以排除： ①KOA致残后器械支持治疗者或等待手术治疗者; ②非原发性关节炎，如风湿性、类风湿性关节炎，痛风特发性关节炎、强直性脊柱炎、代谢性骨病、感染后关节炎、创伤后关节炎或重度骨质增生者等; ③病变关节红肿、灼热、全身发热，或皮肤红斑、硬结者； ④施术部位存在皮肤破溃或患有皮肤病者; ⑤纤维肌痛、足部畸形及其他疾病影响膝关节者; ⑥近12个月内进行过关节镜检查； ⑦近6个月内接受过关节腔内注射； ⑧近3个月内接受过推拿治疗； ⑨近3个月采用其他治疗方案或参与其他临床试验者； ⑩合并有严重心脑血管疾病、恶性肿瘤、传染病及精神疾病患者; 处在怀孕或哺乳期及有妊娠计划的妇女。

Exclusion criteria：

Patients will be excluded from this study if they meet any of the following conditions: Mechanical Support or Awaiting Surgery Due to KOA Disability: Patients receiving mechanical support treatments after disability caused by Knee Osteoarthritis (KOA) or those awaiting surgical intervention. Non-Primary Arthritis: Patients with secondary forms of arthritis such as rheumatic arthritis rheumatoid arthritis gouty arthritis ankylosing spondylitis metabolic bone disease post-infectious arthritis post-traumatic arthritis or severe osteophyte formation. Inflammatory Symptoms in Affected Joints: Patients exhibiting redness swelling heat in the affected joint systemic fever or skin erythema and induration. Skin Ulceration or Skin Diseases at Treatment Site: Patients with skin ulcers or dermatological conditions at the area designated for treatment. Conditions Affecting the Knee Joint: Patients with fibromyalgia foot deformities or other diseases that impact knee joint function. Recent Arthroscopy: Patients who have undergone arthroscopic examinations within the past 12 months. Recent Intra-Articular Injections: Patients who have received intra-articular injections in the knee within the past 6 months. Recent Massage Therapy: Patients who have received massage therapy within the past 3 months. Participation in Other Treatments or Trials: Patients who have adopted other treatment plans or participated in other clinical trials within the past 3 months. Serious Comorbid Conditions: Patients with severe cardiovascular or cerebrovascular diseases malignant tumors infectious diseases or mental illnesses. Pregnant or Lactating Women: Women who are pregnant breastfeeding or planning to become pregnant.

本试验将采用中央区组随机化，区组长度动态变化，研究中心将112例符合纳入标准的KOA受试者根据初次就诊顺序按1:1的比例随机分配至试验组或对照组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

This trial will employ centralized block randomization with dynamically varying block lengths. The research center will randomly assign 112 Knee Osteoarthritis (KOA) patients who meet the inclusion criteria to either the experimental group or the control group in a 1:1 ratio based on the order of their initial consultation.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

建立专门的资料库，数据库内容包括“数据编号”、“参加者姓名拼音缩写”。数据编号会同时被标记在随访记录册上，以便实现参加者信息、数据编号的“双重保险”，确保在数据分析时的信息准确性。数据在收集过程中将采用个人身份资料去识别处理，以保护参加者的隐私；患者入组后使用编号，个人资料和病例信息由数据管理的专人录入、保存，使用者看到的仅为个体编号。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

A special database which contains data number and participant initials will be established. The data number will be marked on the follow-up record book at the same time so as to realize the "double insurance" of participant's information and data number ensuring the accuracy of information during data analysis. In the process of data collection personal identity information will be used for identification and processing to protect the privacy of participants. After patients are enrolled they will be numbered. Personal data and case information will be input and saved by specially-assigned staffs in data management. Only individual number will be known by users.
