研究目的：

1.基于影像组学技术，以磁共振结果作为参考标准，通过X线片建立诊断肩袖损伤的模型。 2.比较超声引导下小针刀和热敷治疗肩袖损伤的临床疗效，初步探索影像组学模型在不同治疗方式干预肩袖损伤疗效机制研宄中的应用方式与应用价值，为影像组学模型的临床应用奠定基础。 3.利用转录组学方法，分别筛选肩袖损伤小针刀治疗前后受试者产生的差异表达基因，利用GO与KEGG Pathway富集分析差异表达基因所调控的与肩袖损伤相关的生物学过程与信号通路，进一步探索小针刀治疗肩袖损伤的疗效机制。

Objectives of Study：

1. Based on the imaging omics technology and the magnetic resonance results as the reference standard, the diagnostic model of rotator cuff injury was established by X-ray film. 2. The clinical efficacy of ultrasound-guided small acupotomy and hot compressions in the treatment of rotator cuff injury was compared, and the application mode and application value of the imaging model in the study of the therapeutic mechanism of different treatment methods in the intervention of rotator cuff injury were preliminarily explored, laying a foundation for the clinical application of the imaging model. 3. Transcriptomic methods were used to screen the differentially expressed genes of subjects before and after rotator cuff injury treatment, and GO and KEGG Pathway were used to enrichment analysis the biological processes and signaling pathways related to rotator cuff injury regulated by differentially expressed genes, so as to further explore the therapeutic mechanism of rotator cuff injury treated by small needle knife.

药物成份或治疗方案详述：

对照组（热敷治疗）:15min/天，隔天1次，3周为1疗程。治疗结束后4周进行随访。 治疗组（超声引导下小针刀治疗）:小针刀每周1次，3次为1疗程。治疗结束后4周进行随访。

Description for medicine or protocol of treatment in detail：

Control group (hot compress treatment) :15min/ day, once every other day, 3 weeks for 1 course of treatment. Follow-up was performed 4 weeks after the end of treatment. Treatment group (ultrasound-guided small needle knife treatment) : small needle knife once a week, 3 times for 1 course. Follow-up was performed 4 weeks after the end of treatment.

纳入标准：

①符合肩袖损伤中医或西医诊断标准； ②年龄18~60岁，性别不限； ③处于II期的患者； ④VAS评分＜7分； ⑤没有同时进行其他治疗； ⑥神志清楚，生命体征平稳；配合相关检查及治疗，并签署知情同意书者。

Inclusion criteria

① Meet the diagnostic criteria of Chinese medicine or Western medicine for rotator cuff injury; Age 18-60 years old, gender is not limited; ③ Patients in stage II; ④VAS score < 7 points; ⑤ no other treatment was given at the same time; ⑥ Clear mind, stable vital signs; Cooperate with relevant examination and treatment, and sign informed consent.

排除标准：

①符合手术指征； ②鉴别诊断为其他肩周疾病者，如急性肩关节损伤，肩关节骨折、脱位、结核、肿瘤，中风后遗症，颈性肩痛、胸廓出口综合征、类风湿性关节炎、其他内科疾病引起的反射痛； ③合并严重心脑血管等内科疾病，生命体征不稳，神志异常者； ④凝血功能障碍、糖尿病患者血糖控制不稳定者； ⑤妊娠、哺乳期妇女； ⑥实施部位有破损，严重感染者； ⑦麻醉过敏者； ⑧有认知功能障碍者。

Exclusion criteria：

① meet the surgical indications; ② Differential diagnosis of other shoulder diseases, such as acute shoulder injury, shoulder fracture, dislocation, tuberculosis, tumor, apoplexy sequelae, neck shoulder pain, thoracic outlet syndrome, rheumatoid arthritis, reflex pain caused by other medical diseases; ③Patients with severe cardiovascular and cerebrovascular diseases, unstable vital signs and abnormal consciousness; ④ Patients with coagulation dysfunction and unstable blood sugar control in diabetes mellitus; ⑤ Pregnant and lactating women; ⑥ The implementation site is damaged, seriously infected; ⑦ anesthesia allergy.

本研究采用临床随机对照实验。根据就诊时间对符合入排标准的患者进行编号，采用随机数字表以及随机数余数分组法。 操作步骤：①将88例病例编号；②获取随机数字：从随机数字表中任意一个数开始，沿同一方向顺序获取每个试验对象一个随机数字；标记小于等于88的数，查找够44个数后停止；③将这44个数对应编号的患者编为一组，其余44个编为另外一组。

This study was conducted in a randomized controlled clinical trial. Patients meeting the criteria were numbered according to the time of visit, and random number table and random number remainder grouping method were used. Procedure: ① The 88 cases were numbered; ② Obtain random numbers: Start from any number in the random number table, and obtain a random number for each test object in the same direction in sequence; Mark the number less than or equal to 88, and stop after finding 44 numbers; ③ The patients corresponding to the 44 numbers were assigned to one group, and the remaining 44 were assigned to another group.
