研究目的：

本研究旨在通过对符合纳入标准的痰湿内盛型单纯性肥胖患者，随机分3组，分别进行地中海饮食+无痛穴位埋线、单用地中海饮食治疗、单用无痛穴位埋线治疗，并在治疗第0周、第4周、第8周、第12周对患者的体重、体重指数（BMI）、腰围、血脂四项、体脂率等进行对比评估疗效，从而研究地中海饮食联合无痛穴位埋线治疗痰湿内盛型单纯性肥胖的效果，以期为痰湿内盛型单纯性肥胖治疗提供佐证与参考。

Objectives of Study：

The purpose of this study was to randomly divide the patients with simple obesity of phlegm-dampness and internal excess who met the inclusion criteria into 3 groups, and respectively receive Mediterranean diet + painless acupoint embedding treatment, Mediterranean diet treatment alone, and painless acupoint embedding treatment alone. In addition, at week 0, week 4, week 8 and week 12, the patients' weight, body mass index (BMI), waist circumference, four items of blood lipids and body fat percentage were compared to evaluate the efficacy, so as to study the effect of Mediterranean diet combined with painless acupoint catgut embedding in the treatment of simple obesity with phlegm-dampness and internal excess, in order to provide evidence and reference for the treatment of simple obesity with phlegm-dampness and internal excess.

药物成份或治疗方案详述：

治疗方法 ①对照组1（地中海饮食）：不予穴位埋线处理，只干预饮食。由营养师根据个人习惯及食物成分，制定地中海饮食{低碳低脂}食谱[6]，总热量控制为1500 kcal/d 以内，其中(碳水化合物∶脂肪∶蛋白质为30∶20∶50)，患者根据食谱进餐；受试者食谱可根据实际情况替换同成分的不同食物，且两者总能量相同。 ②对照组2（无痛穴位埋线）：只予穴位埋线处理，不干预饮食。无痛穴位埋线每2周进行一次，穴位选择如下：上脘、中脘、气海、不容（双侧）、滑肉门（双侧）、梁丘（双侧）、丰隆（双侧）、曲池（双侧）、阴陵泉（双侧）、天枢（双侧）、大横（双侧）、带脉（双侧）。先标记穴位涂抹麻药，60分钟后擦掉麻药，进行皮肤消毒，穴位埋线。埋线时先仰卧后俯卧，局部皮肤常规碘伏消毒，操作者用镊子将 1～2cm 可吸收外科缝线置入埋线针前端，左手撑紧固定局部皮肤，右手持针，迅速将针刺入皮肤，待患者获得针感后旋转退针管，按压针孔止血，予瓶口贴贴敷，嘱患者针孔24小时内勿碰水，3天内勿激烈碰撞身体。所进行的操作均由本科室经培训的指定医师进行完成。 ③观察组（地中海饮食+无痛穴位埋线）：地中海饮食同上述对照组1，无痛穴位埋线同对照组2 。 3. 疗程及随访 •12周为一个疗程，实验第0、4、8、12周时记录体重、腰围、血脂四项、体脂率、BMI，对比评估疗效。并在疗程结束后3个月随访以上指标，评估反弹情况。三组患者均采用回院复查电话形式进行随访。 实验开始前及结束时，检测患者血、尿、便常规，肝肾功能，心电图

Description for medicine or protocol of treatment in detail：

Treatment method ① Control group 1 (Mediterranean diet) : no acupuncture point embedding treatment only intervention diet. According to personal habits and food composition a nutritionist will develop a Mediterranean diet {low carbohydrate and low fat} recipe [6] the total calorie is controlled within 1500 kcal/d of which (carbohydrate fat and protein is 302050) the patient will eat according to the recipe; The subjects' diet can replace different foods with the same composition according to the actual situation and the total energy of both is the same. ② Control group 2 (painless acupuncture point embedding) : only acupuncture point embedding treatment no intervention in diet. Painless acupoint embedding is performed every 2 weeks and the acupoint selection is as follows: Shangwan Zhongwan Qihai Bi (bilateral) Gliumen (bilateral) Liangqiu (bilateral) Fenglong (bilateral) Quchi (bilateral) Yinling Quan (bilateral) Tianshu (bilateral) Daheng (bilateral) Dai Mai (bilateral). Mark the acupuncture points and apply anesthetic wipe off the anesthetic after 60 minutes disinfect the skin and bury the acupuncture points. The operator placed 1 ~ 2cm absorbable surgical suture into the front end of the buried needle with tweezers held the local skin tightly with the left hand held the needle with the right hand and inserted the needle into the skin quickly. After the patient obtained needle sensation the needle was rotated and removed the needle hole was pressed to stop bleeding and the bottle mouth was applied and the patient was instructed not to touch water within 24 hours. Do not hit the body violently for 3 days. All procedures are performed by trained and designated physicians in the Department. Observation group (Mediterranean diet + painless point embedding) : Mediterranean diet with the above control group 1 painless point embedding with the control group 2. 3. Treatment course and follow-up •12 weeks for a course of treatment. At week 0 4 8 and 12 of the experiment weight waist circumference blood lipids body fat percentage and BMI were recorded to compare and evaluate the efficacy. The above indexes were followed up 3 months after the treatment to evaluate the rebound. All patients in the three groups were followed up by returning to the hospital to review the telephone. Blood urine stool routine liver and kidney function electrocardiogram were measured before and after the experiment

纳入标准：

•均符合上述有关肥胖的诊断标准; •年龄18-45岁; •入院前未接受各种减肥药物治疗; •患者依从性好，能按照食谱要求进食; •患者自愿参加本研究并签署知情同意书； •血脂四项正常； •身体健康，血、尿、便常规，肝肾功能，心电图检查均正常。

Inclusion criteria

• All meet the above diagnostic criteria for obesity; • Age 18-45 years old; • Did not receive various weight loss medications before admission; • The patient has good compliance and can eat according to the dietary requirements; • Patients voluntarily participate in the study and sign informed consent; • four normal blood lipids; • Healthy body, blood, urine, stool routine, liver and kidney function, electrocardiogram are normal.

排除标准：

•使用羊肠线及蛋白线不良反应的病史 •瘢痕体质 •妊娠期、哺乳期 •皮肤及药物过敏史 •糖尿病、心脑血管病、精神病史、传染病史、血液病(凝血功能障碍)史 •服用活血化瘀、抗凝等类药物情况。

Exclusion criteria：

• History of adverse reactions to the use of catgut and protein lines • Scar constitution • Pregnancy and lactation • History of skin and drug allergies • History of diabetes cardio-cerebrovascular disease mental illness infectious disease blood disease (coagulation dysfunction) • Taking activating blood circulation and removing blood stasis anticoagulation and other drugs.
