确定侧旋提推正骨手法治疗神经根型颈椎病的临床疗效及安全性

Objectives of Study：

To determine the clinical efficacy and safety of lateral rotation lifting and pushing bone setting manipulation in the treatment of cervical spondylotic radiculopathy

药物成份或治疗方案详述：

美洛昔康组采用口服药物治疗， 每次1粒，每天1次。侧旋提推组采用侧旋提推手法进行治疗，每次总时间约20 min。具体操作如下：理筋加正骨手法。理筋手法用双手拇指指腹进行揉捻、弹拨、点按颈项部双侧的肌肉，以行气止痛，疏通经络。然后医者以一手托患肢，另一手用揉、点、按、抖等手法疏通患肢经脉、行气止痛。待颈、肩部肌肉放松后对颈椎施以侧旋提推法。患者端坐位，医者右拇指置于偏移棘突右侧， 左手掌托住下颌部，颈部前屈 0°～ 15°，医者背胸部稍屈曲，使患者后头部紧靠医者胸骨柄处，令患者全身放松后，左侧侧旋 45°，双手均衡用力，左手稍用力向上提，瞬间右拇指同时用力向左侧轻推，常听到“ 咯”小声，手法完毕，头部恢复原位。再使患者头部偏向另一侧，再做 1 次。最后将患者头部放置于正中位，不要强求闻及“ 咔哒”声。每 3天 1 次，3次为 1 个疗程，疗程间休息2天，。牵引对照组采用枕颌带坐位牵引，头前倾20°，牵引重 3-5 kg。每天 1 次，每次30分钟，5次为1个疗程，疗程间休息 2天 。

Description for medicine or protocol of treatment in detail：

Meloxicam group was treated with oral drugs, 1 capsule each time, once a day. The lateral rotation lifting and pushing group was treated with lateral rotation lifting and pushing manipulation, with a total time of about 20 minutes each time. The specific operations are as follows: tendon regulation and bone setting manipulation. The tendon manipulation uses both hands' thumb pulp to knead, pluck and press the muscles on both sides of the neck, so as to promote qi, relieve pain and dredge the meridians. Then, the doctor holds the affected limb with one hand, and uses kneading, clicking, pressing, shaking and other techniques to dredge the meridians of the affected limb and relieve Qi and pain. After the neck and shoulder muscles were relaxed, the lateral rotation lifting and pushing method was applied to the cervical spine. The patient is seated at the end of the body, the doctor's right thumb is placed on the right side of the offset spinous process, the left palm holds the lower jaw, the neck is bent forward by 0 ° ~ 15 °, the doctor's back and chest are slightly flexed, so that the patient's back head is close to the doctor's sternal handle. After the patient's whole body is relaxed, the left side is rotated by 45 °, the hands are balanced, the left hand is lifted slightly, and the right thumb is pushed to the left at the same time, I often hear a whisper. After the manipulation is completed, the head returns to its original position. Turn the patient's head to the other side and do it again. Finally, place the patient's head in the middle, and do not force to hear the "click" sound. Once every 3 days, 3 times is a course of treatment, and rest for 2 days,. The traction control group was traction with occipital jaw belt sitting position, head tilt forward 20 °, traction weight 3-5 kg. Once a day, 30 minutes each time, 5 times as a course of treatment, and rest for 2 days between courses of treatment.

纳入标准：

诊断标准参照第二届全国颈椎病专题座谈会纪要和国家中医药管理局制定的中医病证诊断疗效标准。选择符合神经根型颈椎病的诊断标准患者，年龄18-58岁，性别不限，纳入本试验。

Inclusion criteria

The diagnostic criteria refer to the minutes of the second National Symposium on cervical spondylosis and the diagnostic efficacy criteria of TCM syndrome formulated by the State Administration of traditional Chinese medicine. Patients who meet the diagnostic criteria of cervical spondylotic radiculopathy, aged 18-58 years, regardless of gender, were included in this trial.

排除标准：

排除同时患有高血压病、冠心病、严重骨质疏松症、骨结核、骨肿瘤、颈椎外伤处于急性期、同时应用非甾类消炎镇痛药、糖皮质激素类药物者，以及孕妇或哺乳期妇女。

Exclusion criteria：

Patients with hypertension, coronary heart disease, severe osteoporosis, bone tuberculosis, bone tumor, acute cervical trauma, non steroidal anti-inflammatory analgesics and glucocorticoids, as well as pregnant or lactating women were excluded.
