研究目的：

本研究的目的是：（1）使用临床评估以及结构、弥散张量成像（DTI）和功能磁共振成像（fMRI）技术，研究KOA引起的慢性疼痛患者的大脑变化；并探讨与针刺治疗相比，接受推拿镇痛治疗的KOA患者脑可塑性变化的纵向机制。

Objectives of Study：

The aims of this study are: (1) to investigate brain changes in patients with chronic pain due to KOA using clinical assessments alongside structural diffusion tensor imaging (DTI) and functional magnetic resonance imaging (fMRI) techniques; and to explore the longitudinal mechanisms of brain plasticity changes in KOA patients undergoing tuina analgesia treatment compared to acupuncture treatment.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

①符合KOA的中、西医诊断标准和瘀血阻滞证；②年龄40～65周岁，左膝疼痛，右利手；③Kellgren-Lawrence放射学分级为I-II级；④VAS>3分，病程不小于3个月；⑤近4周未接受过其他治疗；⑥自愿参加本研究并签署知情同意书。

Inclusion criteria

1. Meet the diagnostic criteria for knee osteoarthritis (KOA) according to both traditional Chinese medicine and Western medicine; 2. Age between 40 and 70 years old with pain in the left knee and right-hand dominance; 3. Kellgren-Lawrence radiological grade of I-II; 4. Visual Analog Scale (VAS) score greater than 3 with a disease duration of no less than 3 months; 5. Have not received other treatments in the past 4 weeks; 6. Voluntarily participate in this study and sign the informed consent form.

排除标准：

①膝部有外伤和手术史者；②膝部有肿瘤、结核、骨髓炎等疾患者；③严重肝肾功能不全、严重心血管疾病、糖尿病及精神疾病影响推拿治疗者；④有其他病引起的身体疼痛者；⑤有药物过敏史者；⑥有闭锁恐惧症、心脏起搏器、除颤器、心脏支架、宫内节育器等MRI禁忌症者；⑦膝关节周围皮肤破损者。

Exclusion criteria：

1. Individuals with a history of trauma or surgery on the knee; 2. Individuals with tumors tuberculosis osteomyelitis or other diseases affecting the knee; 3. Individuals with severe liver or kidney dysfunction severe cardiovascular diseases diabetes or mental illnesses that may affect massage therapy; 4. Individuals with body pain caused by other diseases; 5. Individuals with a history of drug allergies; 6. Individuals with MRI contraindications such as claustrophobia pacemakers defibrillators heart stents intrauterine devices etc.; 7. Individuals with skin lesions around the knee joint.

随机方法（请说明由何人用什么方法产生随机序列）：

用随机信封法。采用SPSS21.0软件产生60个大小不等的随机数字并依次编号，按照大小对随机数字重新排序，规定数字较小的30个随机数是推拿组，数字较大的是针刺组，制作密闭不透明的随机信封，然后按照患者挂号顺序依次发放相应编号的信封，按照随机信封的分组给予相应的干预。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Using the random envelope method we utilized SPSS 21.0 software to generate 60 random numbers of varying sizes and sequentially numbered them. These random numbers were then reordered based on their magnitude with the smaller 30 numbers assigned to the tuina (massage) group and the larger numbers assigned to the acupuncture group. Opaque and sealed random envelopes were prepared and these envelopes were distributed to patients in the order of their registration. The corresponding interventions were administered according to the group assignments indicated in the random envelopes.
