研究目的：

本研究旨在运用电针进行围术期预处理、后处理或全程处理，观察其对两种麻醉方式下老年髋部手术患者POCD早期（1/3/7d及1m）与远期（3/6/12m）的影响，同时进行生化指标、血气分析、炎性因子及血清标志物的检测，为电针干预POCD提供理论和临床依据。

Objectives of Study：

Of this study is to use electric acupuncture for perioperative pretreatment, aftertreatment or full treatment, observe its under the two kinds of anesthesia in elderly patients with hip surgery POCD early (1/3/7 d and 1 m) and long-term (3/6/12 m), the effect of simultaneous biochemical indexes, blood gas analysis, inflammatory factors and serum marker detection, provide theoretical and clinical basis for POCD electric acupuncture intervention.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

A. 髋部骨折患者：股骨颈骨折、股骨粗隆间骨折、髋臼骨折； B. 年龄≥65 岁； C. ASA 分级I~III； D. 术前MMSE 评分中学>24分，小学>20 分，文盲>17 分； E. 病人签署知情同意书。

Inclusion criteria

1. Patients with hip fracture: femoral neck fracture, femoral intertrochanteric fracture, acetabular fracture; 2. Patients over 65 years old; 3. Patients with ASA grade I ~ III; 4. The preoperative MMSE score of patients with secondary school score > 24, primary school score > 20 and illiteracy score > 17; 5. The patient signs the informed consent.

排除标准：

A. 有严重的视力、听力障碍或因其它原因与访视者无法交流； B. 老年性痴呆、脑出血、中风后遗症、安装心脏起搏器及其他重要脏器功能衰竭者； C. 癫痫、酗酒、神经系统疾病及精神病史； D. 术前检查提示肾功能障碍（血肌酐>177μmol/L）或活动性肝病。

Exclusion criteria：

1. Patients with severe visual and hearing impairment or unable to communicate with the visitors due to other reasons; 2. Patients with senile dementia, cerebral hemorrhage, sequelae of stroke, pacemaker installation and other important organ failure; 3. Patients with epilepsy, alcoholism, nervous system diseases and mental history; 4. Patients whose preoperative examination indicated renal dysfunction (serum creatinine > 177 μ mol / L) or active liver disease.

本研究采用单中心、随机、平行对照试验设计，拟纳入病人 120 例。 随机分组四组：1 组（空白对照组，CON 组）30 例，2 组（电针预处理组，pro-EA 组）30例，3组（电针后处理组，post-EA 组）30例，4组（电针全程处理组，all-EA 组）30 例。四组再均分为硬膜外麻醉组（E 组）、全身麻醉组（G 组）各15 例。

采用SPSS22.0 统计软件进行数据分析，计数资料采用构成比表示，组间差异性分析采用χ2 检验，单向有序资料比较采用非参数检验；计量资料服从正态分布的采用均数±标准差表示，不服从正态分布的采用中位数（ 四分位数） 表示， 组间差异性分析多组比较采用方差分析或Kruskal-Wallis H 检验。以 P<0.05 为差异有统计学意义。

The statistical data are represented by composition ratio, the differences between groups are analyzed by chi-square test, and one-way ordered data are compared by non-parametric test.
