研究目的：

产后腰痛的发生与腰椎稳定性下降有关，手法可改善其症状和腰椎功能，但在增强腰椎稳定性方面的作用机制尚不清楚。研究表明，手法可以调整腰椎骨盆对称性，促进其“形闭合”；还可调节肌肉张力，提升腰腹部核心肌群力量，平衡脊柱与附着肌肉之间力学关系，从而促进“力闭合”。本课题组前期研究发现手法可提高腹横肌肌肉厚度，八段锦练习可提高腰部本体感觉，但二者结合改善产后腰痛的疗效机制尚不明确。为此我们提出假说：手法作用于腰椎骨盆及腰腹部肌肉筋膜组织后，再结合八段锦练习，可更有效的提升腰腹肌功能，达到“骨正筋柔”，增强产后腰痛患者的腰椎稳定性，从而改善腰痛症状。本课题拟采用表面肌电分析腰椎骨盆微调手法结合八段锦练习前后腰腹部肌肉激活、肌力、肌耐力等方面的变化，阐明腰椎骨盆微调手法配合八段锦练习对产后腰痛患者腰椎稳定性的肌骨机制。此治疗方法能更有效的缩短产后腰痛患者的康复进程，为手法防治产后腰痛提供新的理论依据。

Objectives of Study：

Postpartum lower back pain is associated with decreased lumbar spine stability. Manual therapy can alleviate symptoms and improve lumbar function but its mechanism for enhancing lumbar stability remains unclear. Studies have shown that manual therapy can adjust lumbar pelvic symmetry promoting 'form closure'; it also regulates muscle tension enhances core strength of the waist and abdomen and balances the mechanical relationship between the spine and attached muscles thereby promoting 'force closure'. Previous studies from our research group found that manual therapy increases the thickness of transverse abdominal muscles while Baduanjin exercises improve proprioception of the lower back. However the combined effect mechanism on alleviating postpartum lower back pain is still unknown. Therefore we propose a hypothesis: after manual therapy is applied to the lumbar-pelvic region and the musculature of the waist and abdomen followed by Baduanjin exercises it may more effectively enhance the function of the waist and abdominal muscles achieving 'aligned bones and flexible tendons' thus increasing lumbar stability in postpartum lower back pain patients and improving their symptoms. 纳入标准：

① 年龄20～40岁； ② 初产及二胎，且分娩后1年内出现腰痛症状的女性； ③ 自愿参加本研究并签署知情同意书； ④ 治疗期间未使用与本研究无关的其他理疗，未服用相关药物等。

Inclusion criteria

Ages 20 to 40; first-time mothers or second pregnancies with back pain symptoms within one year postpartum; willing to participate in this study and sign an informed consent form; no use of unrelated physical therapies or medications during treatment.

排除标准：

① 腰椎结核、腰椎狭窄、腰椎术后； ② 合并内科系统严重疾病； ③ 特殊人群如精神障碍、聋哑等； ④目前正在参加其他临床试验者。

Exclusion criteria：

Lumbar tuberculosis lumbar stenosis post-lumbar surgery; presence of severe internal medical conditions; special populations such as those with mental disorders or deaf-mutism; currently participating in other clinical trials.

按照1:1:1的比例原则，3组分别纳入30例受试者，采用SPSS26.0软件统计分析，产生随机数且封装入遮光信封，由研究者保存。受试者符合标准纳入课题，签署课题知情同意书后随机抽取信封，遵循随机数字原则进入手法组、八段锦练习组、手法结合八段锦练习组，总计90例。

Based on the 1:1:1 ratio principle, three groups each included 30 participants. The statistical analysis was conducted using SPSS version 26.0 software to generate random numbers, which were then sealed in opaque envelopes and kept by the researchers. Participants who met the criteria were enrolled in the study after signing the informed consent form. They then randomly drew an envelope, following the random number principle, to be assigned to either the manual therapy group, the Baduanjin exercise group, or the combined manual therapy and Baduanjin exercise group, totaling 90 participants.
