研究目的：

主要目的：评估大黄联合早期肠内营养对老年 PD 患者首次排便时间、术后 72 小时应激指标（皮质醇 [COR]、促肾上腺皮质激素 [ACTH]、肾上腺素 [EPI]）和炎症指标（C 反应蛋白 [CRP]、白细胞介素 - 6 [IL-6]）的影响。 次要目的：分析该联合干预对患者生活质量、舒适度、满意度以及不良事件发生情况的影响。

Objectives of Study：

Primary objectives: To evaluate the effects of rhubarb combined with early enteral nutrition on the first defecation time postoperative 72-hour stress indices (cortisol [COR] adrenocorticotropic hormone [ACTH] epinephrine [EPI]) and inflammatory indices (C-reactive protein [CRP] interleukin-6 [IL-6]) in elderly PD patients. Secondary objectives: To analyze the impacts of this combined intervention on patients' quality of life comfort satisfaction and the occurrence of adverse events.

纳入标准：

年龄60~80岁； 术前诊断明确； 存在营养不良且需要肠内营养支持； 患者自愿接受本研究的治疗方案并签署知情同意书。

Inclusion criteria

Aged from 60~80 years old; With a clear preoperative diagnosis; With malnutrition and in need of enteral nutrition support; The patient voluntarily accepts the treatment plan of this study and signs the informed consent form.

排除标准：

心肺肾功能、凝血指标异常，胃肠道功能不耐受； 对大黄或肠内 / 肠外营养溶液成分过敏； 自愿不接受本研究治疗方案； 符合纳入标准但纳入后未按计划实施肠内营养。

Exclusion criteria：

Abnormal cardio-pulmonary-renal function and coagulation indexes and intolerance to gastrointestinal function; Allergic to Rheum officinale or components of enteral/parenteral nutrition solutions; Voluntarily refuse to accept the treatment plan of this study; Meeting the inclusion criteria but failing to implement enteral nutrition as planned after inclusion.

术后常规治疗基础上，术后 12 小时经鼻肠管输注 5% 葡萄糖氯化钠 500ml，术后 24 小时开始输注营养科配置的营养液。每日干预 2 次，持续 72小时。短期随访为术后72小时。

On the basis of routine postoperative treatment, 500 ml of 5% glucose sodium chloride is infused through a nasointestinal tube at 12 hours postoperatively, and the nutrient solution prepared by the nutrition department is started at 24 hours postoperatively. The intervention is performed twice daily for 72 hours. Short-term follow-up is conducted at 72 hours postoperatively.

On the basis of the control group, rhubarb nasal feeding is administered through a nasointestinal tube at 6 hours postoperatively (30g of raw rhubarb powder is brewed with 100mL of warm boiled water, filtered to remove residues, and maintained at a temperature of 37°C), combined with early enteral nutrition. The intervention is performed twice daily for 72 hours. Short-term follow-up is conducted at 72 hours postoperatively.

本研究采用计算机生成随机数字表的方法进行随机分组。由不参与患者治疗与评估的专业统计人员，使用统计软件（如 SPSS）生成随机序列。在患者签署知情同意书并确定符合纳入标准后，依据生成的随机序列将患者随机分为观察组和对照组，确保分组过程的随机性和公正性。

In this study the method of generating a random number table by computer is used for random grouping. Professional statisticians who are not involved in patient treatment and assessment generate the random sequence using statistical software (such as SPSS). After patients sign the informed consent form and are determined to meet the inclusion criteria they are randomly assigned to the observation group and the control group according to the generated random sequence ensuring the randomness and fairness of the grouping process.
