评价电针治疗血管性痴呆的临床疗效

Objectives of Study：

To evaluate the clinical effect of electroacupuncture in the treatment of vascular dementia

药物成份或治疗方案详述：

给予对照组脑血管病常规药物治疗，包括控制血压、血脂、血糖，抗血小板聚集，利尿等。在此基础上，口服盐酸多奈哌齐片，每日睡前1次，每次5mg，连续治疗6周。治疗组在与对照组相同的药物治疗基础上，给予电针治疗。针刺取穴：百会、神庭、印堂，以及双侧膈俞、肾俞、悬钟、足三里等。局部常规消毒后针刺，采用提插捻转手法至得气后留针25min，留针期间百会-神庭、肾俞-膈俞接电针。治疗仪参数：连续波，2Hz。治疗频次：每周3次（周一、周三、周五），连续治疗6周。

Description for medicine or protocol of treatment in detail：

The control group was given conventional drug treatment for cerebrovascular disease including control of blood pressure blood lipid blood sugar anti-platelet aggregation diuresis and so on. On this basis Donepezil hydrochloride tablets were taken orally 5mg once a day before going to bed for 6 weeks. The treatment group was given electroacupuncture therapy based on the same drug therapy as the control group. Acupuncture points: Baihui Shenting Yintang and bilateral Ji Shu Shen Shu Hung Zhong Zusanli etc. After local routine disinfection the needle was retained for 25min by lifting inserting and twisting until qi was obtained. During the retention period Baihui-Shenting and Shenyu-Geshu were given electroacupuncture. Therapeutic instrument parameters: continuous wave 2Hz. Treatment frequency: 3 times a week (Monday Wednesday Friday) for 6 weeks.

纳入标准：

（1）符合血管性痴呆的诊断标准； （2）年龄45-85周岁，男女均可； （3）神志清楚，可以配合神经心理学评估； （4）Hachinski缺血量表（见附录2）≥7分； （5）近2周内未接受过血管性痴呆相关的中西医治疗，如果接受其他治疗需经过7天的洗脱期； （6）自愿参与本试验，签署知情同意书

Inclusion criteria

(1) Meet the diagnostic criteria for vascular dementia; (2) Age 45-85 years old male and female; (3) Mentally sound and can cooperate with neuropsychological evaluation; (4) Hachinski Ischemia Scale (see Appendix 2) ≥7 points; (5) had not received traditional Chinese and Western medicine treatment related to vascular dementia in the past 2 weeks and had to undergo a 7-day washout period if receiving other treatment; (6) Voluntarily participate in this experiment and sign the informed consent

排除标准：

（1）排除阿尔茨海默病、帕金森、脑外伤、先天智力迟钝、颅内肿瘤等其他疾病引起的痴呆； （2）意识障碍，严重语言障碍、严重听力障碍，不能或不愿配合检查及治疗者； （3）同时伴有精神分裂症、癫痫、重度抑郁、严重心肝肾功能异常、恶性肿瘤、严重出血倾向、严重胃肠道疾病、严重肺部疾病、代谢紊乱者，或正处于其他疾病急性期者； （4）脑血管病发病前就伴有痴呆者； （5）有酒精滥用史、滥用精神性药物或吸毒者； （6）既往出现晕针者、对针灸针过敏者、针刺瘢痕体质者； （7）对盐酸多奈哌齐、哌啶衍生物过敏者； （8）对半乳糖不耐受、Lapp乳糖酶缺乏症或葡萄糖-半乳糖吸收不良者； （9）妊娠期及哺乳期妇女。

Exclusion criteria：

(1) Excluding dementia caused by Alzheimer's disease Parkinson's disease brain trauma congenital mental retardation intracranial tumors and other diseases; (2) consciousness disorder severe speech disorder severe hearing impairment unable or unwilling to cooperate with examination and treatment; (3) Patients with schizophrenia epilepsy major depression severe cardiac liver and kidney dysfunction malignant tumors severe bleeding tendency severe gastrointestinal diseases severe lung diseases metabolic disorders or are in the acute phase of other diseases; (4) Patients with dementia before the onset of cerebrovascular disease; (5) Have a history of alcohol abuse abuse of psychotropic drugs or drug users; (6) Patients with previous acupuncture dizziness allergic to acupuncture needles and cicatricial constitution of acupuncture; (7) Those allergic to Donepezil hydrochloride and piperidine derivatives; (8) Galactose intolerance Lapp lactase deficiency or glucose-galactose malabsorption; (9) Pregnant and lactating women.
