研究目的：

1.借助数据挖掘技术分析针灸治疗 KOA 的取穴规律; 2.探讨根据数据挖掘结果结合西医康复训练治疗 KOA 患者疼痛的临床疗 效及安全性。

Objectives of Study：

1. To analyze the pattern of acupuncture points in the treatment of KOA with the help of data mining technology; 2. To explore the clinical efficacy and safety of treating the pain of KOA patients with the combination of western rehabilitation training based on the results of data mining.

药物成份或治疗方案详述：

对照组：通过康复训练进行治疗。遵循科学、循序渐进、量力而行的原则进 行。主要包括：从入院第二天开始，每日 2 次，训练 2 周。①关节活动度训练： 以膝关节为轴，前后甩动小腿，每侧 10 次，两侧交替进行；②髋关节外展内收训 练：一侧膝关节伸直，脚尖绷直，做外伸与内收，每侧 10 次，两侧交替进行；③ 股四头肌训练：伸直两侧膝关节，勾足尖到最大范围坚持与放松各 5s，勾起、放 松为 1 次，10 次为 1 组，进行 2 组；④腘绳肌训练：在病人腘窝处放一卷卫生纸， 嘱其用力夹紧卫生纸，坚持与放松各 5s，10 次为 1 组，两侧交替进行 2 组。 试验组 ：在对照组的基础上给予数据挖掘的针灸选穴。针灸穴位:犊鼻、足 三里、阳陵泉、血海、内膝眼、梁丘。操作步骤：患者仰卧位，踩关节稍垫高， 下肢放松，充分暴露患肢，针刺部位碘伏消毒，刺者带无菌手套，由助手打开无 菌针灸针，针刺得气以后，施提插捻转行针，平补平泄进针法，每日 1 次，每次 15-20min，7 天为 1 个疗程，连续治疗 2 个疗程。

Description for medicine or protocol of treatment in detail：

Control group: treatment through rehabilitation training.It follows the principles of science gradual progress and ability to pay.Mainly include: Starting from the second day of hospitalization training twice a day for 2 weeks.Hamstring muscle training: put a roll of toilet paper in the popliteal fossa of the patient ask him to forcefully clamp the toilet paper hold and relax for 5s 10 times for 1 group alternating between the two sides of 2 groups. Experimental group: Based on the control group the patients were given acupuncture points for data mining.Acupuncture points: calf's nose foot Sanli Yanglingquan Sea of Blood inner knee eye Liangqiu.Procedures: patients were placed in supine position the pedal joints were slightly elevated the lower limbs were relaxed the affected limbs were fully exposed the needling site was sterilized by iodine-vapor the needler was wearing sterile gloves and the assistant opened the sterile acupuncture needles and the needles were pricked to obtain qi then the needles were lifted inserted and twisted the needles were inserted with a flat tonic and flat drainage the needles were injected once a day each time was 15-20 min 7 days was a course of treatment and the treatment was consecutively carried out for 2 courses of treatment.

纳入标准：

(1)性别不限，年龄 40~80 岁; (2)符合膝关节骨性关节炎的诊断标准、疼痛在轻度(NRS 1-3 分)以上且保守治疗的患者; (3)生命体征稳定，无急性、严重的内科疾病和其它并发症，可配合检 查者; (4)神志清晰，无沟通障碍者; (5)自愿加入本实验，并签署“知情同意书”者。

Inclusion criteria

(1) Sex is not limited and the age is 40-80 years old; (2) Patients who meet the diagnostic criteria of knee osteoarthritis and have mild pain (NRS 1-3) and are treated conservatively; (3) The vital signs are stable and there are no acute and serious medical diseases and other complications and the patient can cooperate with the inspection; (4) Those who are conscious and have no communication barriers; (5) Those who voluntarily participate in this experiment and sign the "informed consent form".

排除标准：

(1)无法进行主观感觉表达，语言障碍者，意识功能障碍者; (2)有严重心、脑、肝、肾功能异常、恶性肿瘤等严重疾病者; (3)合并多处骨折的患者。

Exclusion criteria：

(1) unable to express subjective feelings people with language barriers and people with consciousness dysfunction; (2) those with serious diseases such as abnormal heart brain liver and kidney function and malignant tumor; (3) Patients with multiple fractures.

研究者选择符合纳入和排除标准的84例膝骨关节炎患者，签署知情同意书后，以入院顺序编号为 1~ 84，依次输入 SPSS 26.0 的编号栏中，进行随机化分组（产生随机数的固定值为：2000000），生成 0~1 之间的 84个随机数。利用生成的随机数进行可视化分箱，分割点数设置为 1，分组结果为“1”则分配到对照组，分组结果为“2”则分配到试验组。为避免样本沾染，将对照组分配在骨科 A病区，试验组分配在 B 病区。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The researchers selected 84 patients with knee osteoarthritis who met the inclusion and exclusion criteria signed the informed consent form and then entered them into the numbering column of SPSS 26.0 in turn and randomly grouped them (the fixed value of generating random numbers was 2000000) to generate 84 random numbers ranging from 0 to 1. The generated random numbers are used for visual grouping. When the number of points is set to 1 the grouping result of "1" is assigned to the control group and the grouping result of "2" is assigned to the experimental group. In order to avoid sample contamination the control group was assigned to orthopedic ward A and the experimental group was assigned to orthopedic ward B.
