研究目的：

本研究在观察电针干预原发性骨质疏松症(POP)疼痛临床疗效的基础上，应用rs-fMRI评价治疗前后自发性脑活动，探讨疼痛改善的中枢机制。在获得电针干预POP疼痛有效的影像学依据前提下，从而为临床提供电针治疗原发性骨质疏松症疼痛可供参考的治疗方案，丰富传统中医治疗骨质疏松症疼痛的治疗手段。

Objectives of Study：

In this study, on the basis of observing the clinical efficacy of electroacupuncture in the treatment of primary osteoporosis (POP) pain, rs-fMRI was used to evaluate spontaneous brain activity before and after treatment, and to explore the central mechanism of pain improvement. Under the premise of obtaining an effective imaging basis for electroacupuncture to intervene in POP pain, it is necessary to provide a reference treatment plan for the treatment of primary osteoporosis pain in clinical practice, and enrich the treatment methods of traditional Chinese medicine for osteoporosis pain.

药物成份或治疗方案详述：

治疗方案详述如下： 1.试验组：在基础防护（建议患者生活方式如补充营养、膳食均衡、增加日照时间、适当规律运动、避免吸烟及过量饮酒和补充足量钙等）的基础上，取穴双侧肝俞、肾俞、脾俞、阳陵泉、悬钟、足三里、委中，定位严格按照《中华人民共和国国家标准:穴位的命名和位置》（GB12346-90）。操作：皮肤常规消毒，将固定绝缘垫片粘贴在穴位上，患者取俯卧位，选用0.25mm×40mm一次性无菌针灸针。肝俞、脾俞与皮肤呈45°角向脊柱方向斜刺，针刺15mm，肾俞直刺25mm，足三里、悬钟、阳陵泉、委中均直刺25mm，进针后行捻转平补平泻法。双侧肝俞、肾俞4个穴位针柄连接XS-998B04型低频脉冲电针疗仪（南京医疗仪器研究所），予疏密波，频率为2 Hz/100Hz，30min后起针。隔日治疗1次，每周3次，共4周。 2.对照组：在基础防护（建议患者生活方式如补充营养、膳食均衡、增加日照时间、适当规律运动、避免吸烟及过量饮酒和补充足量钙等）的基础上，取穴同试验组，皮肤常规消毒，将固定绝缘垫片粘贴在穴位上，选用0.25mm×40mm安慰钝针，各穴位以同样手法将安慰钝针斜刺入泡沫垫片并到达绝缘胶层，患者有针头抵触感，但没有皮肤穿透。电针的波形、频率和电流强度同试验组，持续刺激30min后起针。隔日治疗1次，每周3次，共4周。对照组在研究结束给予4周电针补偿治疗。

Description for medicine or protocol of treatment in detail：

纳入标准：

①符合疾病诊断标准； ②有腰背疼痛表现，且VAS评分：3-7分； ③年龄在50岁-75岁； ④能够正确描述本人意愿，自愿填写知情同意书，同意参加临床试验者。

Inclusion criteria

(1) Meet the criteria for disease diagnosis; (2) Have low back pain and VAS score: 3-7 points; (3) Age 50-75 years old; (4) Those who can correctly describe their own wishes, voluntarily fill in the informed consent form, and agree to participate in clinical trials.

排除标准：

①有甲状腺功能亢进、类风湿关节炎、多发性骨髓瘤等继发性骨质疏松者； ②近3月内有新鲜椎体压缩性骨折； ③近6个月有抗骨质疏松药物服用史（双膦酸盐类、地舒单抗类、降钙素类等，钙尔奇不在抗骨质疏松症药物范围） ④伴有腰肌劳损、腰椎间盘突出、腰椎管狹窄、腰椎结核等其他腰部疾病的患者； ⑤有肾结石、带状疱疹等疾病或合并心血管､脑血管､肝､肾､造血系统等严重原发性疾病或全身多器官衰竭者 ⑥头动幅度大于2mm； ⑦存在fMRI检测禁忌者。

Exclusion criteria：

(1) Patients with secondary osteoporosis such as hyperthyroidism, rheumatoid arthritis, and multiple myeloma; (2) Fresh vertebral compression fracture in the past 3 months; (3) History of taking anti-osteoporotic drugs in the past 6 months (bisphosphonates, denosumabs, calcitonins, etc., calcineur is not in the scope of anti-osteoporosis drugs) (4) Patients with other lumbar diseases such as lumbar muscle strain, lumbar intervertebral disc herniation, lumbar spinal narrowness, lumbar tuberculosis, etc.; (5) Patients with kidney stones, herpes zoster and other diseases, or serious primary diseases such as cardiovascular, cerebrovascular, liver, kidney, hematopoietic system, or systemic multi-organ failure (6) The head movement amplitude is greater than 2mm; (7) Those who have contraindications to fMRI detection.
