研究目的：

在社区建立老年衰弱及衰弱前期人群长期随访队列，观察分析老年衰弱临床表型与中医证候的动态变化；探讨老年衰弱评估模型的构成要素、分级依据，总结形成老年衰弱的评估模型；构建融合中西医技术、包含躯体衰弱和认知衰弱、以分级为基准的上海地区老年衰弱评估标准/体系。

Objectives of Study：

To investigate the prevalence of Frailty in community older residents. To explore the causes and risk factors of frailty.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）年龄≥65岁； （2）上海市户籍，并在本项目实施的社区长期居住（超过五年）； （3）Fried衰弱表型至少存在一项异常。

Inclusion criteria

1. Aged >= 65 years; 2. Permanent residence in Shanghai and long-term residence in the community implemented by this project (more than five years); 3. Be willing to and be able to join in the study and signed Informed consent.

排除标准：

（1）躯体功能因中风、帕金森病、肿瘤、骨折等疾病或残疾受影响者； （2）精神分裂症、抑郁症、焦虑症等精神疾病并依赖药物控制者； （3）痴呆，或有明显认知功能障碍（四分法画钟试验计分为0~1分者）。

Exclusion criteria：

1. Excluding the patients whose physical function is affected by diseases or disabilities such as stroke, Parkinson's disease, tumor, fracture, etc.; 2. Excluding schizophrenia, depression, anxiety and other mental diseases and dependent on drug control; 3. If they were unable to complete the study survey.
