研究目的：

本研究拟评价丁氏推拿手法联合易筋经功法干预腰椎间盘突出症（LDH）临床疗效，为中医非药物疗法--手法联合功法干预LDH提供高质量循证医学证据及临床诊疗模式并推广应用。本研究的目的是评估推拿联合易筋经组疗法的临床效果，并且将重点放在推拿手法联合易筋经功法干预腰椎间盘突出症临床疗效是否优于单一易筋经功法习练的疗效评估上。

Objectives of Study：

This study aims to evaluate the clinical efficacy of Ding's Tuina therapy combined with the YiJinJing exercise in the treatment of lumbar disc herniation (LDH). It seeks to provide high-quality evidence-based medical support and a clinical treatment model for the integration of manual therapy and exercise therapy in traditional Chinese medicine (TCM) facilitating its wider application. The primary objective of this study is to assess the clinical effectiveness of the combined Tuina and YiJinJing therapy with a particular focus on determining whether the combined intervention is superior to YiJinJing exercise alone in the treatment of LDH.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）年龄≥20周岁且≤50周岁，性别不限； （2）符合西医腰椎间盘突出症诊断标准； （3）病程在3个月以上； （4）疼痛视觉模拟评分≥3分且≤7分； （5）治疗期间同意不接受其它治疗（包括肌松剂治疗药物）者； （6）自愿参加本研究并签署知情同意书。

Inclusion criteria

(1) Age ≥ 20 years and ≤ 50 years, with no gender restrictions; (2) Meeting the diagnostic criteria for lumbar disc herniation (LDH) in Western medicine; (3) Disease duration of more than 3 months; (4) Visual Analog Scale (VAS) pain score ≥ 3 and ≤ 7; (5) Agree not to receive other treatments (including muscle relaxant medications) during the study period; (6) Willing to participate in the study and sign the informed consent form.

排除标准：

（1）既往有或疑似有腰椎重大疾病，如骨折、肿瘤、骨质疏松、马尾综合征等； （2）一年内有脊柱手术病史者； （3）有严重器质性疾病或精神疾病者； （4）在入组前三个月内参与过其他临床试验者； （5）妊娠期、哺乳期女性； （6）有明显传染病史、遗传病史者； （7）有智力或精神障碍无法进行问卷填写及检查者； （8）酗酒和（或）精神情志异常、药物滥用和药物依赖者（含镇痛药）； （9）既往长期习练或在过去3个月内习练过易筋经、气功或瑜伽者； （10）接受本疗法前1个月内接受包括西药、中药、针灸、推拿等治疗。

Exclusion criteria：

(1) History or suspected presence of major lumbar spine diseases, such as fractures, tumors, osteoporosis, or cauda equina syndrome; (2) History of spinal surgery within the past year; (3) Presence of severe organic diseases or mental disorders; (4) Participation in other clinical trials within the three months prior to enrollment; (5) Pregnant or lactating women; (6) History of significant infectious diseases or genetic disorders; (7) Intellectual or mental disabilities that prevent completion of questionnaires or medical examinations; (8) Alcohol abuse and/or significant emotional or psychological abnormalities, drug abuse, or drug dependence (including analgesics); (9) Previous long-term practice or practice of Yi Jin Jing, Qigong, or Yoga within the past three months; (10) Receipt of treatments such as Western medicine, traditional Chinese medicine, acupuncture, or Tui Na within one month prior to the intervention.

本项目采用随机分组形式，随机数字表通过随机数生成器(Strategic Applications Software，version9.1.3;SAS Institute Inc,，Cary，NC，USA)生成。课题组中一名研究人员负责记录受试者的详细信息(姓名、出生日期、纳入时间等)；通过随机数生成器生成的所有随机数字将被制作成卡片并按顺序放入预先准备好的信封中，由一名独立于课题组外，事先未知悉的研究人员来进行该操作。在基线评估后符合条件的受试者, 治疗师会按顺序打开信封并根据显示数字进行分组。对照组采用易筋经治疗，试验组采用推拿手法联合易筋经治疗。

Randomization Procedure (please state who generates the random number sequence and by what method)：

This study employs a randomized grouping design with random numbers generated using a random number generator (Strategic Applications Software version 9.1.3; SAS Institute Inc. Cary NC USA). One researcher from the study team is responsible for recording detailed information about the participants (e.g. name date of birth enrollment time). All randomly generated numbers will be printed on cards and sequentially placed into pre-prepared envelopes. This procedure will be conducted by an independent researcher who is not part of the study team and has no prior knowledge of the allocation. After the baseline assessment eligible participants will be assigned to groups by the therapist who will sequentially open the envelopes and allocate participants according to the displayed numbers. The control group will receive YiJinJing therapy while the experimental group will receive Tuina therapy combined with YiJinJing therapy.
