研究目的：

基于“筋骨”理论制定 LDH 中医方案，创新性的采用急性期调筋以治骨，缓解期正骨与调筋并重，康复期养骨以柔筋。以中医特色斜扳手法结合传统中药内服或外用，为主要干预措施，并形成 LDH 中医方案的临床评价方案，以验证中医综合方案疗效。采用多中心开放性随机对照设计，下肢疼痛 VAS 为主要疗效指标，腰椎 JOA、改良 ODI、直腿抬高试验角度、影像学改变为次要疗效指标，评价 LDH 中医方案有效性与安全性，优化临床方案。

Objectives of Study：

Based on the theory of "muscles and bones", the LDH TCM program is innovatively adopted to adjust tendons in the acute phase to treat bones, the orthopedic bone and tendon adjustment in the remission period, and the bone nourishment during the rehabilitation period to flex the tendons. The characteristic diagonal wrench method of traditional Chinese medicine combined with traditional Chinese medicine internal or external use was the main intervention, and the clinical evaluation protocol of LDH TCM protocol was formed to verify the efficacy of TCM comprehensive scheme. A multi-center open-label randomized controlled design was adopted, with lower limb pain VAS as the main efficacy index, lumbar spine JOA, modified ODI, straight leg elevation test angle, and imaging changes as secondary efficacy indexes to evaluate the efficacy and safety of LDH TCM regimen and optimize the clinical protocol.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

1.符合腰椎间盘突出症诊断，CT和MRI证实有腰椎间盘突出，临床症状为腿部疼痛延伸至膝盖以下，并符合影像学对应神经根分布区域； 2.年龄18岁至65岁(含18岁和65岁)； 3.签署知情同意书； 4.腰腿疼痛VAS≥7.0分，纳入到重度；腰腿疼痛VAS＜7.0分且≥4.0分，纳入中度；腰腿疼痛VAS＜4.0且＞0分，纳入轻度。

Inclusion criteria

1. In line with the diagnosis of lumbar disc herniation, CT and MRI confirmed that there is lumbar disc herniation, the clinical symptoms are leg pain extending below the knee, and in line with the imaging corresponding nerve root distribution area; 2. Age 18 to 65 years old (including 18 and 65 years old); 3. Sign the informed consent form; 4. Low back and leg pain VAS ≥ 7.0 points, included in severe; low back and leg pain VAS < 7.0 points and ≥ 4.0 points, included in moderate; low back and leg pain VAS < 4.0 and >0 points, included mild.

排除标准：

1.有腰椎手术史； 2.脊柱压缩骨折、腰椎Ⅱ度及以上滑脱、腰椎峡部裂、腰椎管狭窄； 3.脊柱肿瘤；脊柱结核；严重骨质疏松(T值≤-3.0或伴发骨质疏松性骨折)；糖尿病伴周围神经病变； 4.对非甾体药物、中药等相关药物过敏； 5.消化道溃疡/出血病史； 6.服用双抗药物或凝血系统障碍等潜在高出血风险人群 ,如冠状动脉旁路搭桥围手术期患者； 7.腰部有严重皮肤病或腰部皮肤破损等； 8.孕妇及哺乳期妇女； 9.重度心力衰竭、脑卒中等严重心脑血管疾病；严重肝、肾功能不全； 10.出现马尾神经损害，或下肢肌力≤3级，或下肢运动、感觉功能持续性降低等手术指征； 11.不适合参加临床研究的特殊人群(盲、聋、哑、智力或精神障碍等)。

Exclusion criteria：

1. Have a history of lumbar spine surgery; 2. Spinal compression fracture, lumbar spondylolisthesis of degree II and above, lumbar isthmus fissure, lumbar spinal stenosis; 3. Spinal tumors; Tuberculosis of the spine; severe osteoporosis (T-≤-3.0 or accompanied by osteoporotic fractures); Diabetes mellitus with peripheral neuropathy; 4. Allergy to non-steroidal drugs, traditional Chinese medicine and other related drugs; 5. History of peptic ulcer/bleeding; 6. People at potentially high risk of bleeding such as taking bispecific antibody drugs or coagulation system disorders, such as perioperative patients with coronary artery bypass grafting; 7. Severe skin diseases or skin damage at the waist; 8. Pregnant and lactating women; 9. Severe heart failure, stroke and other serious cardiovascular and cerebrovascular diseases; severe hepatic and renal insufficiency; 10. Damage to the cauda equina nerve, or lower limb muscle strength ≤ grade 3, or continuous reduction of lower limb movement and sensory function; 11. Special populations (blind, deaf, mute, intellectual or psychiatric disabilities, etc.) who are not suitable for participating in clinical research.
