研究目的：

确定针刺治疗的特异性疗效，明确针刺治疗特异性作用脑区，探究针刺治疗慢性疼痛的中枢神经调节机制

Objectives of Study：

To determine the therapeutic effect and explore the neural mechanism of acupuncture in patient with chronic plantar fasciitis.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）符合足底筋膜炎诊断的患者，病程≥6月；经以下任何一种保守治疗（如物理治疗、口服非甾体抗炎药和1次局部激素注射治疗或神经阻滞等）≥1月，改善不明显或反复发作者； （2）入组时过去3天晨起足跟痛最痛程度VAS评分平均值≥50mm； （3）年龄在18至75岁之间； （4）无其它急慢性疼痛性疾病； （5）右利手； （6）签署知情同意书，自愿参与本试验。

Inclusion criteria

(1) Participants with clinical diagnosis of chronic plantar fasciitis. History of plantar medial heel pain for at least 6 months before enrolment. Failure to respond to conservative treatment for ≥1month, including any of the following modalities: stretching exercises, NSAIDs, shockwave therapy, dry needling and orthotics; (2) Reported an average worst pain intensity at first steps in the morning over the last 3 days of at least 50 mm on a 100 mm visual analogue scale (VAS) before enrolment; (3) Participants from 18 to 75 years of age; (4) No other acute or chronic painful conditions; (5) Right-handedness; (6) Participants who have voluntarily agreed to participate in the study and have signed written informed consent.

排除标准：

1.跟骨结节骨折/跟骨应力性骨折/跟骨骨刺/跟骨挫伤/足底筋膜断裂病史； 2.踝关节或足部手术病史； 3.跟腱附着点病变/踝管综合征/跟骨内侧神经卡压/神经损伤； 4.系统性或局部感染、足跟严重皲裂、足畸形（高弓足、扁平足、足外翻）； 5.系统性疾病（强直性脊柱炎、类风湿性关节炎、血清反应阴性关节炎、肿瘤、糖尿病）； 妊娠妇女； 6.合并严重心、肝、肾脏、造血系统、自身免疫系统疾病、全身营养状况严重不良患者； 7.认知障碍或精神、心理疾病； 8.过去6月局部使用类固醇注射或口服\过去2月曾使用止痛药物； 9.过去8周内曾接受针灸治疗的患者； 10.有MRI检查禁忌症者； 11.常规MRI检查有可见的结构异常。

Exclusion criteria：

1. History of calcaneal tubercle fracture, calcaneal stress fracture, calcaneal spur, calcaneal contusion or plantar fascia rupture; 2. Previous surgery to foot or ankle; 3. Lumbosacral radiculopathy or peripheral neuropathy around the ankle joint such as nerve entrapment tarsal tunnel syndrome or Achilles tendinopathy; 4. Joint, bone or skin infection in the affected foot; 5. Systemic disorders like rheumatoid arthritis, gout, Reiter syndrome, type 1 or 2 diabetes mellitus, osteoporosis, spondyloarthritis or osteomyelitis; 6. Clinically significant cardiovascular disorder, severe hepatic/renal insufficiency, coagulation disorder, or severe systemic malnutrition at baseline; 7. Cognitive impairment or mental and psychological diseases; 8. Local steroid injection or oral administration in the past 6 months, or analgesic drugs have been used in the past 2 months; 9. Patients who have received acupuncture treatment in the past 8 weeks; 10. Contraindications of MRI examination; 11. Routine MRI examination showed visible structural abnormalities.

患者不接受任何治疗在试验结束后，如病情未好转，将向患者提供4周12次免费的针刺治疗（针刺方案同针刺组）。

Participants will receive no treatment for their heel pain for a period of 16 weeks after randomisation, and subsequently have the option of 4 weeks (12 sessions) of acupuncture free of charge at the end of the follow- up period.

Acupuncture at sham Ashi (0.5 cun away from Ashi, one ‘cun’ is equivalent to the greatest width of the individual patients’ thumb, ~1.5 cm), sham BL57 (0.5 cun lateral to the true BL57 horizontally), sham KI3 (midway between the true KI3 and the heel tendon) and sham BL60 (midway between the true BL60 and the heel tendon) .
