研究目的：

通过中医标准化术语规范，构建前瞻性、结构化的前中风病历采集系统，从患者入院的时刻开始围绕中风中医诊疗特点，全面、完整、准确地采集中医体质、证候要素等中医特征数据，实现特定时间、地点的全样本信息采集，探讨构建互联网＋具有中医特色的中风病人健康管理及复发预警体系，为解决中风复发死亡率高的问题提供科学智能诊断、治疗及二级预防的依据。

Objectives of Study：

Through the standardized terminology specification of TCM, a prospective and structured pre-stroke medical record collection system is built to comprehensively, completely and accurately collect TCM characteristic data such as TCM physique and evidence elements from the moment of patient's admission around the TCM treatment characteristics of stroke, to realize the full sample information collection at specific time and place, to explore the construction of Internet + health management and recurrence warning system for stroke patients with TCM characteristics, and to provide scientific and intelligent diagnosis, treatment and secondary prevention basis to solve the problem of high recurrence mortality of stroke.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

①符合《1995 年全国第四届脑血管病学术会议》中脑卒中的诊断标准，并且经颅脑CT 或 MR 明确诊断者；②年龄在 18 岁以上，意识清醒者；③排除外伤、颅内肿瘤和感染、脑脱髓鞘病变、严重心肺肝肾功能不全及其他原因导致肢体、器官残缺或者畸形④患者知情，并能配合治疗、检查及随访。

Inclusion criteria

(1) Those who meet the diagnostic criteria for stroke in the 1995 Fourth National Symposium on Cerebrovascular Diseases and are clearly diagnosed by cranial CT or MR; (2) Those who are 18 years of age or older and conscious; (3) Those who exclude trauma, intracranial tumors and infections, cerebral demyelinating lesions, severe cardiopulmonary, hepatic and renal insufficiency and other causes of limb or organ mutilation or deformity; (4) Those who are informed and can cooperate with treatment, examination and follow-up.

排除标准：

①妊娠及哺乳期妇女；②合并严重心、肝、肾、肺功能损害③依从性差、不能配合者。

Exclusion criteria：

① Pregnant and lactating women; ② Combined with severe cardiac, hepatic, renal and pulmonary impairment; ③ Poor compliance and inability to cooperate.
