研究目的：

观察中医全科-专科联合管理模式对心脑血管疾病衰弱患者的管理效果。

Objectives of Study：

To observe the management effect of cardiovascular and cerebrovascular patients with frailty syndrome by TCM general-specialist team.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

(1) 年龄≥65 岁； (2) 心脑血管疾病病情稳定的患者； (3) FRAIL虚弱量表评分≥3分； (4) 理解量表内容、能进行正常交流并能配合完成评估测试； (5) 自愿参与本研究。

Inclusion criteria

1. Patients with age >= 65 years old; 2. Patients with stable cardiovascular and cerebrovascular diseases; 3. Patients with frail score >= 3; 4. Patients who understand the content of the scale, can communicate normally and cooperate to complete the assessment test; 5. Subjects who participated in this study voluntarily.

排除标准：

(1) 疾病终末期，预期寿命≤12个月； (2) 存在严重认知功能障碍或完全失能、卧床者； (3) 合并有急性的肝、肾、肿瘤和造血系统等疾病； (4) 正在参加其他与本研究相近的研究。

Exclusion criteria：

1. Patients with end-stage disease and life expectancy <= 12 months; 2. Patients with severe cognitive impairment or complete disability and bedridden; 3. Patients with acute liver, kidney, tumor and hematopoietic diseases; 4. Patients who are participating in other studies similar to this study.

采用区组随机方法，设定区组长度为4，按1:1比例分为试验组和对照组。利用SAS 9.4统计软件PROC PLAN过程语句，给定种子数，分别产生92例受试者所接受处理的随机数字表。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The patients are randomly divided into experimental group and control group according to the ratio of 1:1. The random number tables of 92 subjects are generated by using the procedure statement of proc plan in SAS 9.4 statistical software.
