研究目的：

目前我国成人II型糖尿病患病率约为10.9%，位居全球首位。糖尿病性视网膜病变（diabetes retinopathy，DR）为糖尿病最严重的眼部并发症。在我国成年人糖尿病患者中，DR患病率为24.7%-37.5%。DR患者康复更多依赖于出院后持续性的自我管理。而相关研究表明，我国DR患者普遍存在出院后遵医行为依从性差、自我管理能力低、缺乏疾病护理认知，以致血糖控制不佳，加速眼部微血管的病变，再入院率增加，影响患者生活质量。延续护理作为整体护理的一部分，旨在强调通过构建协作性与连续性的照护，提升患者院外自我管理能力和生活质量水平。延续护理干预目前受到很多医疗机构的重视。本研究结合文献研究、问卷调查、专家咨询、临床应用等方式，探索互联网+模式在DR患者延续护理的构建研究，旨在利用信息化服务平台，建立医院-家庭动态化、突出中医特色的延续护理服务，以期提高患者疾病认知及自我管理能力，改善低视力患者生活质量，提升整体护理水平。

Objectives of Study：

The prevalence of type 2 diabetes among Adults in China is about 10.9%, ranking first in the world. Diabetic retinopathy (DR) is the most serious ocular complication of diabetes. Among Chinese adults with diabetes, the prevalence of DR is 24.7%-37.5%. Rehabilitation of DR patients depends more on continuous self-management after discharge. Relevant studies have shown that DR patients in China generally have poor compliance with medical compliance behaviors after discharge, low self-management ability and lack of disease nursing awareness, resulting in poor blood glucose control, accelerated lesions of eye microvessels, increased readmission rate and affected patients' quality of life. Continuing care, as part of holistic care, aims to improve patients' ability to self-manage and quality of life by building collaborative and continuous care. At present, continuous nursing intervention is emphasized by many medical institutions. This study combining literature research, questionnaire investigation, expert consultation, clinical application and so on, to explore Internet + patterns in patients with DR continuation of the construction of nursing research, aims to use information service platform, set up the hospital - the continuation of family dynamic, highlight the characteristics of TCM nursing service, in order to improve the patient's disease cognition and self management ability, improve the quality of life in patients with low vision, Improve the overall nursing level.

药物成份或治疗方案详述：

纳入标准：

A.符合2型糖尿病Ⅲ期及以上分期的诊断标准；B.符合糖尿病视网膜病变诊断标准；C.签署知情同意书；D.年龄35-75岁。

Inclusion criteria

A. Meet the diagnostic criteria of stage ⅲ and above of type 2 diabetes; B. Meet the diagnostic criteria of diabetic retinopathy; C. Signed informed consent; D. Ages 35 to 75.

排除标准：

A.合并有其他眼部疾病：视神经萎缩、虹膜睫状体炎、葡萄膜炎、黄斑变性、青光眼；B.合并严重的心功能衰竭、其它严重的躯体疾病，生活不能自理的患者；C.有认知障碍及沟通障碍者；D.经门诊治疗未住院治疗者；E.不同意复查者。

Exclusion criteria：

A. Associated with other eye diseases: optic atrophy, iridocyclitis, uveitis, macular degeneration, glaucoma; B. Patients with severe heart failure or other serious physical diseases who are unable to take care of themselves; C. People with cognitive and communication disabilities; D. Outpatient treatment without hospitalization; E. Those who disagree with the review.

对照组出院时即按照常规护理干预进行出院指导及随访，告知患者遵医嘱定期医院复查，如出现视力下降、眼睛不舒适则随时回院复查。
