研究目的：

评估针刺改善尼古丁“戒断症状”的“生物标志物调控”，通过检测血液中食欲素（OXA）、促肾上腺皮质激素释放因子（CRF）及唾液可替宁等生物标志物揭示针刺治疗烟草依赖的潜在生物学机制，为中医理论在成瘾医学中的应用提供科学依据。

Objectives of Study：

To evaluate the "biomarker regulation" of acupuncture in improving nicotine "withdrawal symptoms" by detecting biomarkers such as orexin (OXA) corticotropin-releasing factor (CRF) and cotinine in saliva in the blood the potential biological mechanism of acupuncture in the treatment of tobacco dependence was revealed providing a scientific basis for the application of traditional Chinese medicine theory in addiction medicine.

药物成份或治疗方案详述：

1.戒烟辅导基础治疗 针刺组均给予戒烟辅导基础治疗，并按照世界卫生组织临床戒烟辅导“5A”方案，即包括询问（ask）吸烟情况、建议（advise）戒烟、评估（assess）戒烟意愿、提供戒烟帮助（assist）和安排（arrange）随访，每次辅导时间不少于15分钟，每周1次，持续8周，8周治疗结束后戒烟辅导停止。 2.干预方法 针刺干预, 穴位选择：百会（GV 20）、列缺（LU 7）、合谷（LI 4）、足三里（ST 36）、 三阴交（SP 6）、太冲（LR 3），穴位定位参照国家标准《腧穴名称与定位》 （GB/T12345-2021）。除百会外，其余各穴均双侧取穴。本研究的针刺治疗方案是基于前期的文献整理与临床研究制定的，针对烟草戒断综合征的中医病因病机和主要临床症状表现而定。 针刺操作：患者仰卧位，医者用 75%的医用酒精棉球在穴位处消毒，选取华佗牌（苏州医疗用品厂）0.25mm×25mm 和 0.25mm×40mm 的一次性针灸针，百会向后平刺 15-20mm，列缺顺经脉循行平刺 15-20 mm，合谷、足三里、三阴交、太冲均直刺 20-30mm，得气以穴位局部出现酸麻胀重感为准；得气后，采用平补平泻法均匀提插捻转，捻转角度 90-120°，频率 60-90 次，每隔 10min 行针 1 次，同侧列缺与足三里连接 KWD-808 脉冲电疗仪（常州市武进长城医疗器械有限公司），断续波，频率 2/15 Hz，强度以患者可耐受为度，留针 30min，每周三次，隔日进行治疗，针刺戒烟治疗8周。

Description for medicine or protocol of treatment in detail：

1.Basic treatment for smoking cessation counseling in the acupuncture group was all given basic treatment for smoking cessation counseling and was carried out in accordance with the "5A" plan of clinical smoking cessation counseling of the World Health Organization which includes asking (ask) about smoking status advising (advise) on smoking cessation assessing (assess) willingness to quit smoking providing (assist) smoking cessation assistance and arranging (arrange) follow-up. Each counseling session should last no less than 15 minutes once a week for 8 weeks. The smoking cessation counseling will be stopped after the 8-week treatment is completed. 2. Intervention method: Acupuncture intervention. Acupoint selection: Baihui (GV 20) Lieche (LU 7) Hegu (LI 4) Zusanli (ST 36) Sanyinjiao (SP 6) Taichong (LR 3). Acupoint location refers to the national standard "Names and Locations of AcuPoints" (GB/T12345-2021). Except for Baihui all the other acupoints were selected from both sides. The acupuncture treatment plan of this study was formulated based on the previous literature review and clinical research and was determined according to the etiology pathogenesis and main clinical symptoms of tobacco withdrawal syndrome in traditional Chinese medicine. Acupuncture operation: The patient lies in the supine position. The doctor disinfects the acupoints with 75% medical alcohol cotton balls. Disposable acupuncture needles of Huatuo brand (Suzhou Medical Supplies Factory) 0.25mm×25mm and 0.25mm×40mm are selected. Baihui is inserted horizontally backward by 15-20mm and Lieche is inserted horizontally along the meridians by 15-20mm. Pierce Hegu Zusanli Sanyinjiao and Taichong points directly for 20-30mm. The qi is obtained when a local sensation of soreness numbness distension and heaviness occurs at the acupoints. After obtaining the qi use the flat filling and flat draining method to evenly lift insert and twist. The twisting Angle is 90-120° and the frequency is 60-90 times. Perform the needle once every 10 minutes. Connect the same side with the KWD-808 pulse electrotherapy device (Changzhou Wujin Great Wall Medical Equipment Co. LTD.) at Zusanli. Intermittent wave frequency 2/15 Hz. The intensity should be within the tolerance of the patient. The needles should be retained for 30 minutes three times a week and the treatment should be carried out every other day. The acupuncture smoking cessation treatment should last for 8 weeks.

纳入标准：

（1）烟草依赖患者：①纳入标准：1）符合ICD-11烟草依赖诊断标准；2）18岁≤年龄≤75岁的男性；3）烟龄≥1年，呼气一氧化碳检测≥6 ppm；4）进入研究前 30 天未进行过针灸治疗者；5）愿意参与本次研究，签署知情同意书。

Inclusion criteria

(1) Patients with tobacco dependence: ① Inclusion criteria: 1) Conforming to the ICD-11 diagnostic criteria for tobacco dependence; 2) Male aged between 18 and 75 years old; 3) Smoking age ≥1 year breath carbon monoxide test ≥6 ppm; 4) Those who have not received acupuncture treatment within 30 days before entering the study; 5) Be willing to participate in this research and sign the informed consent form.

②排除标准：具有临床意义的医学或神经系统疾病；患有严重的心、肺、脑、血液系统疾病；当前或最近（过去 2 个月）使用任何精神药物或抗组胺药以及戒烟治疗；当前使用强 CYP3A 活性酶或中度 CYP3A 抑制剂或强 CYP3A 诱导剂，当前使用地高辛；当前的其他物质使用障碍，如酒、可卡因等；当前患有口腔疾病、齿科疾病等。

② Exclusion criteria: Medical or neurological diseases of clinical significance; Suffer from serious diseases of the heart lungs brain and blood system; Currently or recently (in the past two months) using any psychotropic drugs or antihistamines as well as smoking cessation treatment; Currently strong CYP3A active enzymes or moderate CYP3A inhibitors or strong CYP3A inducers are being used and digoxin is currently being used. Current other substance use disorders such as alcohol cocaine etc. Currently suffering from oral diseases dental diseases etc.
