研究目的：

构建5种心脑血管疾病复发加重的中医风险预警模型，制定中医干预技术标准，应用于东部中心城市、中部县域、西部省会城市3000余例5种心脑血管疾病患者，管理1年，评判干预效果。

Objectives of Study：

Established the risk of early-warning model and the criteria ofr intervention of the 5 kinds of cardiovascular and cerebrovascular diseases which were based on the system of the TCM. And were used to the 3000 patients of estern central city, central county and western provincial city of China,managing for 1 year and evaluted the effect of intervention.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

1.符合5种慢病的诊断标准：心绞痛参照2007年《不稳定性心绞痛和非ST段抬高心肌梗死诊断与治疗指南》、心肌梗死参照2018年《急性心肌梗死中西医结合诊疗指南》、心力衰竭参照2018年《中国心力衰竭诊断和治疗指南》、脑梗死参照《中国脑梗死中西医结合诊治指南》、脑出血参照《中国脑出血治疗指南》； 2.年龄≥18 岁； 3.知情同意自愿参加本研究者。

Inclusion criteria

1. Meet the diagnostic criteria of the five kinds of chronic diseases.The diagnosis of angina refer to the Guidelines for the diagnosis and treatment of unstable Angina and non-ST-elevation myocardial infarction;the diagnosis of Myocardial infarction refer to the Guidelines for integrated traditional and Western medicine in the diagnosis and treatment of acute myocardial infarction;the diagnosis of heart failure refer to the Guidelines for the diagnosis and treatment of heart failure in China;the diagnosis of Cerebral infarction refer to Chinese guide to diagnosis and treatment of cerebral infarction with integrated traditional Chinese and Western medicine;the diagnosis of Cerebral hemorrhage refer to Guidelines for the treatment of cerebral hemorrhage in China; 2. Aged over 18 years; 3. Signing the informed consent.

排除标准：

1.有严重精神、心理疾病者； 2.认知功能障碍，沟通交流障碍或不配合者； 3.正在参与其他管理项目者。

Exclusion criteria：

1. Patients with severe mental illness; 2. Patients with severe cognitive impairment,serious communication deficiencies,or unable to match the researchers to complete the study; 3. Participated in other drug clinical trials or other study of chronic disease management now.
