研究目的：

1. 收集并分析研究对象健康信息及体质状态，常见慢性病、亚临床疾病人群的中医证候特征、疾病风险因素的流行病学分布及长期变化趋势。 2.发现各种影响常见慢性病发病、转重及病死的独立危险因素，中医证候特征与生化指标间的关联，建立预测模型实现早期预警。 3.评价中医药主动健康指导效果，为慢性病的中医药防治提供循证证据。

Objectives of Study：

1. Collect and analyze the health information and physical status of the subjects, the TCM syndrome characteristics of common chronic diseases and subclinical diseases, the epidemiological distribution and long-term change trend of disease risk factors. 2. To find out the independent risk factors affecting the incidence, severity and death of common chronic diseases, the correlation between TCM syndrome characteristics and biochemical indicators, and establish a prediction model to realize early warning. 3. To evaluate the effect of TCM active health intervention and provide evidence-based evidence for the prevention and treatment of chronic diseases with TCM.

纳入标准：

①　天津市常驻人口； ②　签署知情同意书，自愿参加本项目（<18岁由家长签署）； ③　年龄不限（覆盖全年龄段）。

Inclusion criteria

①Tianjin permanent population; ② signed informed consent form (signed by parents of children under 18 years old); ③ No age limit (covering all age groups).

排除标准：

①　不能完整完成数据采集工作； ②　不能配合完成随访工作。

Exclusion criteria：

① Unable to complete the data collection work; ② not to cooperate to complete follow-up work.
