研究目的：

1选取青少年为研究对象，通过前瞻性队列研究设计，观察失眠因素暴露下青少年抑郁症的发病情况。 2观察耳穴压豆对失眠青少年睡眠的改善及抑郁症的预防效果。

Objectives of Study：

1 Adolescents were selected as study subjects to observe the onset of depression in adolescents exposed to insomnia factors through a prospective cohort study design. 2 Observe the effect of auricular pressure bean on the improvement of sleep and the prevention of depression in adolescents with insomnia.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）符合ICSD-3诊断标准； （2）年龄为13-18岁； （3）PSQI评分>7分； （4）未接受精神类药物治疗； （5）无沟通和认知障碍； （6）签署知情同意书。

Inclusion criteria

(1) Meets ICSD-3 diagnostic criteria; (2) Aged 13-18 years; (3) PSQI score >7; (4) Not receiving psychotropic medication; (5) No communication or cognitive impairment; (6) Signed informed consent.

排除标准：

（1）癫痫，中风等器质性疾病引起的睡眠障碍； （2）精神分裂症或其他严重精神疾病引起的睡眠障碍； （3）其他睡眠障碍的诊断，如阻塞性睡眠呼吸暂停，快速眼动(REM)睡眠障碍或不宁腿综合征； （4）过去一个月曾接受过耳穴压豆治疗失眠； （5）酒精和/或其他药物滥用或依赖。

Exclusion criteria：

(1) Sleep disorders caused by epilepsy stroke and other organic diseases; (2) Sleep disorders caused by schizophrenia or other serious mental illnesses; (3) Diagnosis of other sleep disorders such as obstructive sleep apnoea rapid eye movement (REM) sleep disorder or restless legs syndrome; (4) Have received auricular pressure beans for insomnia in the past month; (5) Alcohol and/or other drug abuse or dependence.

睡眠宣教：积极与患者沟通了解其失眠的原因，给予针对性指导和讲解，以减轻其负性情绪，予以睡眠宣教手册，告知患者睡眠的重要性、指导患者创造良好的睡眠环境，改变不良的睡眠习惯，坚持适当的体育锻炼等等。并对其进行后续随访，定时通过ASHS、PSQI、ASWS、CDI、BDI了解其睡眠及心理健康。若患者出现抑郁状态，及时予以专业的治疗。

在对照组的基础上给予耳穴压豆，所有操作均由5年以上临床经验的针灸医师完成。 取穴：参照上海科技出版社出版的《图解耳穴疗法》中耳穴的定位、功能和应用。神门穴（三角窝内，对耳轮上、下脚分叉处稍上方）、皮质下（对耳屏内侧面）、内分泌（屏间切迹内，耳甲腔的前下部）、肝穴（耳甲艇后下方）、心穴（耳甲腔中央）、肾穴（对耳轮上、下脚分叉处下方的耳甲艇部）。 操作方法：参照《图解耳穴疗法》中耳穴的定位评估患者后，在疼痛最敏感处贴耳穴压豆贴。每次贴一侧耳，3日后更换另一侧耳，交替进行，嘱患者自行按压3~5次/日，1~2min/次，治疗4周。 疗程：3天1次，每周2次，治疗4周，共8次。

使用SPSS 26.0软件生成随机数字，并分为耳穴压豆组和对照组，将随机数字、组别分别写在卡片上放入密封、不透光的信封，信封编号与随机卡片相对应，当就诊患者符合纳入标准时，根据就诊顺序打开同编号的信封，纳入对应的组别进行治疗。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Random numbers were generated using SPSS 26.0 software and divided into auricular pressure bean group and control group. Random numbers and groups were written on cards and put into sealed opaque envelopes with envelope numbers corresponding to the randomized cards and when the patients attending the clinic met the inclusion criteria envelopes with the same number were opened according to the order of the clinic visit and included in the corresponding groups for treatment.

盲法：

为确保临床试验结果的真实性、准确性，本研究采用单盲，仅对数据采集及分析者设盲，无法对治疗医师及受试青少年设盲。

Blinding：

To ensure the authenticity and accuracy of the results of the clinical trial the study was single-blinded and only the data collector and analyzer were blinded and it was not possible to blind the treating physician and the subject adolescents.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集，采用纸质版“青少年生活事件量表、青少年睡眠卫生量表、匹兹堡睡眠质量指数、青少年睡眠觉醒量表 、儿童抑郁量表、贝克抑郁量表”和电子病历系统（内网）相结合的模式。研究中所有研究对象资料及原始研究数据均保存于南京中医药大学附属医院江苏省中医院针灸康复科科研项目数据库。

For data collection a combination of paper-based "Adolescent Self-Rating Life Events Checklist Adolescent Sleep Hygiene Scale Pittsburgh Sleep Quality Index Pittsburgh Sleep Quality Index Adolescent Sleep-Wake Scale Childrens Depression Inventory Beck Depression Inventory" and electronic medical record system (intranet) was used. All the information of the research subjects and the original research data in the study were saved in the database of the scientific research program of the Department of Acupuncture and Rehabilitation Jiangsu Provincial Hospital of Traditional Chinese Medicine Affiliated Hospital of Nanjing University of Traditional Chinese Medicine.
