以即时和长时程 taVNS 临床行为证据+多模态脑网络变化为出发点,通过探索 pDOC 患者在不同时间尺度和相应频段上额顶脑区动态功能连接变化,以及皮层水平微状态的时空动力学及脑网络机制研究。结合多维度脑网络指标在意识恢复过程中的变化及其与意识改善的关系,阐明 taVNS 在改善 pDOC 意识状态的效应机制

Objectives of Study:

Taking immediate and long-term taVNS clinical behavioral evidence + multimodal brain network changes as the starting point we explored the dynamic functional connectivity changes in frontal-parietal brain regions of pDOC patients at different temporal scales and corresponding frequency bands as well as the temporal dynamics and brain network mechanisms of cortical level microstates. intervals scales and corresponding frequency bands of dynamic functional connectivity changes in the superior fronto-parietal brain region as well as the study of spatiotemporal dynamics and brain network mechanisms of cortical-level microstates in pDOC patients. Combined with the changes in multidimensional brain network indexes in the process of consciousness recovery and its relationship with the improvement of consciousness we will elucidate the effect mechanism of taVNS in improving the consciousness of pDOC

药物成份或治疗方案详述:

Description for medicine or protocol of treatment in detail:

纳入标准:

①年龄 18 岁至 65 岁,性别不限;②评估适宜(神经影像、脑电图、各种诱发电位);③不使用影响意识的镇静药物;④在 CRS-R 评定时患者生命体征平稳;入组前均接受 1 周至少 5 次 CRS-R 量表评估,选取基线稳定的 pDOC 患者;⑤患者监护人或法定代理人签署书面知情同意书。

Inclusion criteria

(i) Age 18 to 65 years old regardless of gender; (ii) appropriate assessment (neuroimaging electroencephalogram various evoked potentials); (iii) no sedative drugs affecting consciousness; (iv) patients' vital signs were stable at the time of the CRS-R assessment; all patients were assessed by the CRS-R scale at least 5 times a week before enrollment and patients with pDOC who were stable at baseline were selected; and (vi) the patient's guardian or legal representative signed a written informed consent form.

排除标准:

①试验前三个月内参加过其他临床试验的患者;②脑电图监测前 24h 使用镇静药;③合并多系统多脏器功能受损病者;④因颅骨缺损或头皮异常无法完成脑电图监测的患者;⑤未获知情同意书;⑥病程不足 3 周。

Exclusion criteria:

(1) Patients who had participated in other clinical trials within three months before the trial; (2) used sedative drugs 24 hours before EEG monitoring; (3) patients with combined multi-system and multi-organ impairment; (4) patients who were unable to complete the EEG monitoring because of cranial defects or scalp anomalies; (5) patients who did not obtain informed consent; (6) patients who had been sick for less than 3 weeks.

Randomization Procedure (please state who generates the random number sequence and by what method):

The random sequence for participant allocation will be generated using the randperm function from MATLAB (version 2020b MathWorks Inc. Natick USA). The generation of the random sequence will be performed by an independent administrator who is not involved in any other aspects of the study to ensure impartiality and confidentiality. The randomization will allocate eligible participants in a 1:1 ratio to either the treatment group (taVNS group numbers 1-28) or the control group (sham control group numbers 28-56). To maintain the concealment of the allocation the results of the randomization will be stored in opaque sealed and sequentially numbered envelopes.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

The data sources for this study include electronic medical records questionnaires and brain function imaging examinations. The data collection tools include electrocardiogram (ECG) machines sphygmomanometers a customized electronic questionnaire system and near-infrared spectroscopy (NIRS) devices. Baseline data are collected when patients are enrolled and weekly brain function assessments and behavioral evaluations are conducted for a period of 4 weeks. All data are directly entered into the database through the customized electronic questionnaire system. After data entry two independent researchers perform double-checks to ensure the accuracy and completeness of the data. Regular data cleaning is conducted to handle missing values and outliers ensuring data quality. All data will undergo logical checks to exclude unreasonable or contradictory data.
