研究目的：

将超声可视化技术优势与传统针刀疗法相结合，实现股骨头坏死患者髋周软组织精准化松解。观察可视化针刀改善股骨头坏 死“带塌陷生存”患者髋关节功能的有效性与安全性。

Objectives of Study：

By combining the advantages of ultrasound visualization technology with traditional needle-knife therapy precise release of soft tissue around the hip in patients with femoral head necrosis was achieved. Observe visual needle knife to improve bad femoral head Efficacy and safety of hip function in patients with dead "survival with collapse".

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

①符合股骨头坏死诊断标准，且股骨头已塌陷（ARCOIII-IV期)；诊断标准：参照Mont等提出的国际诊断标准 1）特异性标准 1股骨头塌陷 2 X线软骨下线状透亮影 3前外侧死骨 4 ECT示环绕放射减低区的放射增高带 5 MRI T2加权相双线征 6骨组织活检骨陷窝空虚 2）非特异标准 1股骨头塌陷伴关节间隙狭窄 2股骨头内斑点状囊性变和骨硬化混杂存在 3骨扫描活性增加 4 MRI示骨髓水肿 5髋关节活动时疼痛但X线片正常 6饮酒或激素使用史 7骨组织活检骨髓水肿或纤维化 只要同时满足特异性指标和非特异性指标中的一条，诊断成立。 ②无明显疼痛或疼痛程度较轻（VAS评分≤3 分），合并有髋关节活动受限的患者； ③年龄 18-75 岁，男女不限； ④影像学提示坏死区域修复良好，股骨头前侧壁或外侧壁保留，头臼关系匹配，未出现股骨头半脱位； ⑤自愿参加本研究，同意签署伦理知情同意书。

Inclusion criteria

① Meet the above diagnostic criteria for femoral head necrosis and the femoral head has collapsed (ARCOIII-IV stage);Diagnostic criteria: refer to the international diagnostic criteria proposed by Mont et al 1) Specificity criteria 1. Collapse of the femoral head 2 X-rays of subcartilage linear translucent opacity 3 anterolateral dead bones 4 ECT shows a radioactive increase band surrounding the hyporadiation-low zone 5 MRI T2-weighted phase bilinear sign 6. Bone biopsy: Bone pit is empty 2) Non-specific criteria 1. Collapse of the femoral head with narrowing of the joint space 2. Speckled cystic changes and osteosclerosis in the femoral head are mixed 3. Increased bone scanning activity 4 MRI showed bone marrow edema 5. Pain when the hip joint is moved but the X-ray is normal 6. History of alcohol consumption or hormone use 7. Bone biopsy bone marrow edema or fibrosis As long as one of the specific and nonspecific indicators is satisfied the diagnosis is established. ② Patients with no obvious pain or less severe pain (VAS score ≤3 points) combined with limited hip mobility; ③Age 18-75 male or female ④Imaging showed that the necrotic area was well repaired the anterior or lateral wall of the femoral head was preserved the relationship between the femoral head and the acetabulum was matched and there was no subluxation of the femoral head. ⑤ Volunteer to participate in this study and agree to sign ethical informed consent.

排除标准：

①影像学提示坏死区域累及股骨头前外侧壁，股骨头进一步塌陷风险较高的患者； ②合并有严重心血管、脑血管、造血系统等原发性疾病者，尤其是凝血功能异常，不适合接受针刀治疗的患者； ③髋关节合并感染或者其他疾病患者，特别是行针刀部位有皮肤破损、溃烂的患者； ④孕妇及哺乳期妇女； ⑤正在参加其它临床研究或在入组前 30 天内接受其它相关治疗，可能影响本研究的疗效观察指标者；

Exclusion criteria：

① Imaging indicated that the necrotic area involved the anterior lateral wall of the femoral head and the risk of further collapse of the femoral head was high; ②Patients with severe cardiovascular cerebrovascular hematopoietic and other primary diseases especially those with abnormal coagulation function are not suitable for needle-knife treatment; ③ Patients with joint infection or other diseases especially patients with skin damage and ulceration at the acupotomy site; ④ Pregnant and lactating women; ⑤ Participants participating in other clinical studies or receiving other related treatments within 30 days prior to enrollment which may affect the efficacy observation indicators of this study;

随机方法（请说明由何人用什么方法产生随机序列）：

采用简单随机法进行随机分配，应用SPSS生成随机数字，获得试验分组并制作随机卡，将随机卡装于密封、不透光的信封，符合纳入排除标准的病例按照纳入试验的先后顺序，对应随机卡信封的序号，拆开信封，按照随机卡分组方案实施临床治疗。

Random allocation was carried out by simple random method random numbers were generated by SPSS experimental groups were obtained and random cards were made. The random cards were placed in sealed and opaque envelopes. Cases meeting the inclusion and exclusion criteria were assigned the serial number of the envelopes of random cards according to the sequence of inclusion in the trials and the envelopes were opened and clinical treatment was carried out according to the random card grouping scheme.
