研究目的：

通过科学规范的随机对照试验，评估一周2次温针灸与一周5次温针灸治疗膝骨关节炎的临床疗效，观察高频次与低频次治疗在改善症状程度、疗效持续时间之间的差异，为探索膝骨关节炎更有效的治疗方案提供依据，为临床中不同患者选择住院高频治疗或门诊低频治疗提供指导意见。

Objectives of Study：

Through scientifically standardized randomized controlled trial, evaluate the clinical efficacy of moxibustion therapy twice a week versus five times a week in treating knee osteoarthritis as well as observing the differences in symptom improvement and duration of efficacy between high-frequency and low-frequency treatments. This will provide a basis for exploring more effective treatment options for knee osteoarthritis and offer guidance for selecting either high-frequency inpatient treatment or low-frequency outpatient treatment for different patients in clinical practice.

纳入标准：

① 符合西医诊断标准及中医辨证标准； ② 年龄50-80岁，男女不限； ③ 膝关节X线Kellgren-Lawrence分级I-III级； ④ 意识清楚，对答切题，能理解各项量表条文并完成评定； ⑤ 同意参加试验并签署书面知情同意书。

Inclusion criteria

1. Meets both Western medicine diagnostic criteria and Traditional Chinese Medicine diagnostic criteria. 2. Age between 50 and 80 years old, both genders. 3. Knee joint X-ray Kellgren-Lawrence grade I-III. 4. Clear consciousness, able to answer questions accurately, understand the items of various scales, and complete assessments. 5. Willing to participate in the trial and sign the informed consent form.

排除标准：

① 有膝关节手术史或明显急性外伤史或合并膝关节肿瘤、结核、骨髓炎； ② 近6个月关节镜检查或近3个月关节腔内注射激素或透明质酸； ③ 由医生评估诊断的身体条件及精神状况无法进行治疗，包括心、肝、肾、脑血管和造血系统严重急慢性器质性疾病、近期中风、活动性癌症、精神疾病、存在认知障碍或言语沟通障碍等； ④ 在过去1月内参加过其他临床试验。

Exclusion criteria：

1. History of knee joint surgery, significant acute trauma, or presence of knee joint tumors, tuberculosis, or osteomyelitis. 2. Knee arthroscopy within the past 6 months or intra-articular injection of steroids or hyaluronic acid within the past 3 months. 3. Medical conditions or mental states evaluated by a physician that would prevent treatment, including severe acute or chronic organic diseases of the heart, liver, kidneys, cerebrovascular system, or hematopoietic system, recent stroke, active cancer, mental disorders, cognitive impairment, or speech communication disorders. 4. Participation in another clinical trial within the past month.

The acupoint selection and operational methods for the low-frequency group are identical to those of the high-frequency group. However, the treatment frequency differs: During the treatment phase, treatment is administered once daily, with sessions held 2 days per week (treatment every 2-3 days). During the maintenance phase, treatment is administered once daily, once a week.

采用温针灸治疗。患者取仰卧位，患侧膝下垫毛巾卷，膝关节自然屈曲，皮肤消毒后，取患侧犊鼻、内膝眼、阳陵泉、阴陵泉、梁丘、血海、鹤顶穴，内膝眼穴、犊鼻穴向上斜刺25-40mm，其余选取的穴位直刺20-30mm，采用平补平泻手法，得气后留针。将艾柱套在犊鼻、内膝眼穴的针柄上，从艾柱底部点燃施灸，每个艾柱燃烧时间为10min，连续燃烧2个艾柱，烧尽后一起出针。治疗频率为：治疗期每天治疗1次，每周治疗5天。维持期每天治疗1次，每周治疗1次。

Warm acupuncture therapy are accepted. After disinfecting the skin, the following acupoints are selected on the affected side: Dǔ Bí (ST35), Xī Bī (SP10), Yáng Líng Quán (GB34), Yīn Líng Quán (SP9), Liáng Qiū (ST34), Xuè Hǎi (SP10), and Hè Dǐng (GV14).Treatment frequency: During the treatment phase, treatment is administered once daily for 5 consecutive days per week. 本试验采用区组随机化方法进行分组，由一位独立的统计学专家完成，以1：1比例将受试对象分为高频组和低频组。区组大小为4，分组个数为2，使用SPSS26.0软件生成随机数字表。

Randomization Procedure (please state who generates the random number sequence and by what method)：

This experiment utilized block randomization for grouping, conducted by an independent statistical expert. Participants were divided into high-frequency and low-frequency groups in a 1:1 ratio. Blocks consisted of 4 subjects each, with a total of 2 blocks. Random numbers were generated using SPSS 26.0.

盲法：

数据评估、录入和资料整理阶段采用盲法，评价者、统计者不知道受试者来自哪一组，由于干预措施的限制，对针灸师及患者不设盲。为保证屏蔽的实施，在试验实施前各研究人员将多次接受有关本研究规范实施的培训，均严格遵守各部门分离原则。

Blinding：

During the stages of data assessment, entry, and compilation, blinding is employed. Evaluators and statisticians are unaware of which group the subjects belong to. Due to the nature of the intervention, blinding is not feasible for acupuncturists and patients. To ensure effective blinding, all researchers undergo multiple trainings on the standardized implementation of the study protocol before its commencement. They strictly adhere to the principle of departmental separation.
