研究目的：

（1）探讨基于龙砂开阖六气针法的电刺激胃肠起搏联合护理方案（联合方案）缓解脓毒症患者胃肠功能障碍的护理疗效及其作用机制。 （2）推广基于龙砂开阖六气针法电刺激胃肠起博治疗脓毒症胃肠功能障碍患者中医护理技术方案。

Objectives of Study：

（1）Exploration of the Nursing Efficacy and Mechanism of Action of the Combined Regimen Involving Longsha Kaihe Six-Qi Acupuncture Technique with Electrical Gastrointestinal Pacing in Alleviating Gastrointestinal Dysfunction in Sepsis Patients. （2）Promotion of Traditional Chinese Medicine Nursing Technical Protocol Based on Longsha Kaihe Six-Qi Acupuncture Technique and Electrical Gastrointestinal Pacing for Sepsis Patients with Gastrointestinal Dysfunction.

药物成份或治疗方案详述：

两组患者均根据《2021年国际脓毒症和脓毒性休克管理指南》及ICU护理循证常规进行循证护理。研究组在常规循证护理基础上，进行龙砂开阖六气针法电刺激胃肠起搏治疗，每日1次，7天为1个疗程。 常规循证护理方案： （1）脓毒症急性期护理 “1h集束化策略”是指在发现脓毒症后的1h内即迅速实施以下5项措施：①监测血乳酸浓度，若〉2 mmol/L，则应反复监测；②给予抗生素前抽血培养；③使用广谱抗生素；④如有低血压或血乳酸浓度〉4 mmol/L，则迅速静脉输注 30mL/kg 的晶体液；⑤在液体复苏期间或复苏后，如病人仍有低血压，应使用血管活性药物，将MAP维持在 65mmHg以上。 （2）脓毒症后续期间护理 ①抗生素治疗：在脓毒症被诊断并留取血标本以后，应尽早开始静脉使用抗生素。 ②液体复苏：早期开始液体复苏对于脓毒性休克至关重要，脓毒性休克患者的液体复苏应尽早开始，对脓毒症所致的低灌注，推荐在拟诊为脓毒性休克起3小时内输注至少30ml/kg的晶体溶液进行初始复苏。 ③血管活性药物的应用：推荐去甲肾上腺素作为首选血管加压药，对于快速性心律失常风险低或心动过缓的患者，可将多巴胺作为替代药物。 ④营养支持治疗：严重脓毒症/脓毒性休克复苏后血流动力学稳定者，应尽早开始营养支持（48小时内），首选肠内营养。 ⑤呼吸支持：在保证呼吸道通畅的前提下，通过呼吸支持可以纠正低氧或二氧化碳潴留以及代谢紊乱。 ⑥肝肾功能支持。要监测肝肾功能，注意保证肾的有效血流量，当出现肾功能不全时可以采用血液净化治疗。 ⑦预防深静脉血栓。对脓毒症病人可以应用普通肝素或低分子肝素预防深静脉血栓，如果有使用肝素的禁忌情况可以改用机械方法。 ⑧预防应激性溃疡。对于脓毒症及脓毒性休克患者，如果存在消化道出血危险因素，需进行应激性溃疡的预防。 龙砂开阖六气针法电刺激胃肠起搏标准操作流程： （1）携用物至患者处，核对患者身份。观察患者生命体征，确定生命体征平稳后开始下一步流程。 （2）床头抬高30度，协助患者取舒适体位。 （3）龙砂开阖六气针法电刺激要点 操作前准备：充分暴露腹部，清洁腹部皮肤（不可使用乙醚和纯酒精）并擦干皮肤。 站位：护士站在患者腹部左侧或右侧，面向患者。以肚脐为中心，取少阳（中脘穴）、少阴（中极穴）“双枢”以及及阳明、厥阴（左、右大横穴）“双阖”。 定穴：大横穴（肚脐旁4寸处）、中脘穴（脐中直上4寸处）、中极穴（脐中直下4寸处）。 胃肠起博操作：连接好导联线和电极，将导联线另一端插入起博器相应的刺激输出口，确认起博器接通电源。将电极粘贴定穴处，根据病人病情，选择合适的脉冲参数。脉冲频率：40Hz,40+2.5Hz,40+10Hz可选。脉冲间隔：2-6秒，可以2秒开始，根据需要调整。脉冲时间：多数选择20分钟左右，可根据病人的耐受程度，随时暂停和修改。 在护理治疗过程中须监测患者生命体征指标，以对辅助治疗效果加以判断，并适时调整相应脉冲刺激参数。 （4）操作完毕，收拾用物，将患者置于舒适体位，整理床单位。 （5）洗手，记录。胃肠起博器清洁消毒后备用。 （6）疗程：一般每日治疗1-2次，任一餐后进行，每次治疗20～30分钟，7-15天为一个疗程。 注意事项：专责监护护士全程参与整个胃肠起博过程；随时观察患者生命体征，有病情恶化倾向的应立即终止，并予以规范诊治措施。

Description for medicine or protocol of treatment in detail：

纳入标准：

(1) 符合脓毒症的诊断标准；脓毒症和脓毒性休克共识定义（Sepsis-3)中提出的脓毒症诊断标准； (2) 符合胃肠功能障碍Ⅱ-Ⅲ级诊断标准； (3) 年龄大于等于18 岁； (4) 患者或家属同意患者接受龙砂开阖六气针法电刺激胃肠起搏治疗。

Inclusion criteria

(1) Meeting the diagnostic criteria for sepsis as defined in the Sepsis-3 consensus (The Third International Consensus Definitions for Sepsis and Septic Shock); (2) Meeting the diagnostic criteria for gastrointestinal dysfunction (Grade II-III); (3) Age ≥18 years; (4) Written informed consent obtained from the patient or their legal guardian for receiving Longsha Kaihe Six-Qi Acupuncture combined with electrostimulation gastrointestinal pacing therapy.

排除标准：

(1)不符合脓毒症诊断标准和 GDⅡ-Ⅲ级诊断标准； (2)精神疾病、孕妇或哺乳期妇女； (3)精神障碍、孕妇或哺乳期妇女； (4) ICU住院时间小于 3 天； (5) 年龄小于18 岁； (6) 食管、胃或肠道内科和外科病史或胃肠道原发性损伤； (7) 患者及其家属不愿接受龙砂开阖六气针法电刺激胃肠起搏治疗。

Exclusion criteria：

(1) Not meeting the diagnostic criteria for sepsis or GD grade II-III classification; (2) Mental disorders pregnant or lactating women; (3) Mental disorders pregnant or lactating women; (4) ICU hospitalization duration less than 3 days; (5) Age under 18 years; (6) History of esophageal gastric or intestinal medical/surgical diseases or primary gastrointestinal injuries; (7) Patients or their families refusing Longsha Kaihe Liuzhi acupuncture method with electrical stimulation for gastrointestinal pacing therapy.

研究者陆佳基于Excel程序按病例入组顺序，采用随机数字表法，二组按1:1的比例，根据入组顺序分别纳入循证护理对照组（对照组）和龙砂开阖六气针法综合护理研究组（研究组）。

Researcher Lu Jia utilized Excel software to randomly allocate patients into two groups in a 1:1 ratio according to their enrollment sequence using the random number table method. Based on the order of enrollment patients were respectively assigned to either the evidence-based nursing control group (control group) or the Longsha Kaihe Liufi Acupuncture combined with comprehensive nursing intervention group (study group).
