研究目的：

观察中医推拿对慢性颈痛患者疼痛伴焦虑状态临床疗效及脑功能响应特征的影响，为推拿临床治疗慢性颈痛患者疼痛伴焦虑状态提供依据。

Objectives of Study：

To observe the effects of traditional Chinese medicine massage on the clinical efficacy and brain function response characteristics of pain with anxiety in patients with chronic neck pain and to provide a basis for the clinical treatment of pain with anxiety in patients with chronic neck pain by massage.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

1.慢性颈痛患者伴焦虑的纳入标准： （1）以颈部疼痛不适伴有焦虑状态、僵硬或颈椎活动受限为主诉，且焦虑自评量表（SAS）≥50分； （2）18岁≤年龄≤75岁，男女均可, 右利手； （3）颈部疼痛不适程度采用视觉模拟评分（VAS）每7天中有5天疼痛程度在3分及以上； （4）病程在3个月及以上； （5）依从性好，愿意配合研究的进行，自愿加入本研究，并签署患者知情同意书。 2.健康受试者的纳入标准： （1）18岁≤年龄≤75岁，男女均可, 右利手； （2）既往健康（世界卫生组织关于健康的定义：健康是一种在身体、精神上的完满状态，以及良好的适应力，而不仅仅是没有疾病和衰弱的状态），近1个月无身体及精神上不适； （3）无焦虑、抑郁等情绪障碍病史，且焦虑、抑郁量表评分在正常范围内； （4）签署知情同意书，自愿参加本项研究者。

Inclusion criteria

1. Inclusion Criteria for Patients with Chronic Neck Pain and Comorbid Anxiety: Chief complaint of neck pain and discomfort accompanied by anxiety stiffness or restricted cervical mobility with a Self-Rating Anxiety Scale (SAS) score ≥ 50;Age between 18 and 75 years (inclusive) male or female right-handed;Visual Analog Scale (VAS) score for neck pain ≥ 3 on at least 5 days within a 7-day period; Duration of symptoms ≥ 3 months; Good compliance willingness to cooperate with the study voluntary participation and provision of written informed consent. 2. Inclusion Criteria for Healthy Controls: Age between 18 and 75 years (inclusive) male or female right-handed;In good general health (as defined by the World Health Organization: health is a state of complete physical mental and social well-being and not merely the absence of disease or infirmity) with no physical or mental discomfort in the past month;No history of anxiety depression or other mood disorders and scores within the normal range on standardized anxiety and depression rating scales; Voluntary participation and provision of written informed consent.

排除标准：

(1) 18岁以下及75岁以上； (2) 近3个月内因慢性颈痛接受过针灸、推拿或药物治疗； (3) 颈椎椎体结核、肿瘤等椎管内占位性病变及脊柱侧弯畸形者； (4) 颈椎外病变（胸廓出口综合征、网球肘、腕管综合征、肘管综合征、肩周炎、肱二头肌腱鞘炎等）所致以上肢疼痛为主的 (5) 严重骨质疏松及合并心脑血管、消化、泌尿、血液、内分泌系统等严重疾病或临床控制不达标； (6) 有酒精依赖或止痛药等药物滥用习惯者； (7)体内有金属残留物者（包括义齿、起搏器、神经刺激器、医疗泵等） ； (8)对fMRI畏惧以及其它原因不能进行fMRI刺激者； (9) 妊娠期及哺乳期妇女，及未来半年内有生育要求的女性； (10) 正在参加其他临床研究者。

Exclusion criteria：

Age below 18 or above 75 years;Receipt of acupuncture Tuina (Chinese manual therapy) or pharmacological treatment for chronic neck pain within the past 3 months;Presence of space-occupying lesions in the spinal canal such as cervical vertebral tuberculosis tumors or spinal deformities (e.g. scoliosis);Cervical-extrinsic causes of upper limb pain such as thoracic outlet syndrome tennis elbow carpal tunnel syndrome cubital tunnel syndrome frozen shoulder or biceps tenosynovitis;Severe osteoporosis or coexisting poorly controlled systemic diseases including cardiovascular cerebrovascular gastrointestinal urogenital hematologic or endocrine disorders;History of alcohol dependence or substance abuse particularly analgesics;Presence of metal implants or retained metallic foreign bodies (e.g. dental prostheses pacemakers neurostimulators infusion pumps);Claustrophobia or other contraindications to undergoing functional magnetic resonance imaging (fMRI);Pregnant or breastfeeding women or women planning to become pregnant within the next six months;Concurrent participation in other clinical studies.

随机方法（请说明由何人用什么方法产生随机序列）：

106例慢性颈痛伴焦虑患者采取简单随机化方法，随机数由第三方使用 SPSS 软件生成，得出顺序号，制作随机卡片，加信封密封。1代表推拿手法组，2代表颈部功能锻炼组。合格志愿者进入试验时，按具体进入的先后次序以信封上的序号顺序拆开信封，依照随机卡片上的提示进行顺序分组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

A total of 106 patients with chronic neck pain and comorbid anxiety were randomly assigned to groups using a simple randomization method. Random numbers were generated by a third party using SPSS software to produce a sequence list. Each number was printed on a randomization card which was then sealed in an opaque envelope. The number 1 represented the Tuina (manual therapy) group and 2 represented the neck functional exercise group. When an eligible participant entered the study envelopes were opened in the order of enrollment according to the sequence number printed on the envelope. Group allocation was determined based on the number revealed inside the envelope.
