研究目的：

深入贯彻落实习近平总书记关于推进中医药现代化、产业化的重要精神和健康中国的战略部署，充分发挥我省“北华佗、南新安”名老中医资源和“大院大所”科技创新的优势，推动中医学跨学科研究攻关，利用现代科技抢救性、高保真、系统性的收集整理安徽省名老中医诊疗中风、不寐、痴呆、眩晕、泄泻的临床病例资料，挖掘和发现安徽省名老中医诊疗规律，为解决制约名老中医经验传承应用的理论和技术瓶颈，建立产学研医协同创新平台，助力安徽跻身中医药强省提供数据支撑。

Objectives of Study：

Thoroughly implement General Secretary Xi Jinping's important spirit of promoting the modernization and industrialization of traditional Chinese medicine and the strategic deployment of a healthy China fully leverage the advantages of our province's "North Huatuo South Xin'an" famous old Chinese medicine resources and "big courtyard and big institute" scientific and technological innovation promote interdisciplinary research and breakthroughs in traditional Chinese medicine use modern technology to rescue high fidelity and systematically collect and organize clinical case data on the diagnosis and treatment of stroke insomnia dementia dizziness and diarrhea by famous old Chinese medicine practitioners in Anhui Province explore and discover the diagnosis and treatment rules of famous old Chinese medicine practitioners in Anhui Province solve the theoretical and technical bottlenecks that restrict the inheritance and application of famous old Chinese medicine experiences establish a collaborative innovation platform between industry academia research and medical institutions and provide data support to help Anhui become a strong province in traditional Chinese medicine.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

一、中风纳入标准 纳入标准 （1）符合西医脑梗死恢复期诊断标准； （2）符合中医中风病中经络诊断标准； （3）符合中医气虚血瘀证辨证标准； （4）本次发病病程：从发病之日起14-180天； （5）既往脑梗死病史者，本次发作前无残障者（改良Rankin量表评分≤1分） （6）采用美国国立卫生研究院脑卒中量表（NIHSS）评分标准，神经功能缺损程度积分为≥7分，≤22分； （7）年龄≥40周岁，≤80周岁者； （8）年龄签署知情同意书。 注：如以上任何一个答案为“否”，此受试者不能入组。 二、眩晕纳入标准 纳入标准 （1）年龄 ≥ 18 周岁； （2）感觉自身或外界景物旋转，轻者闭目即止，重者如坐舟船，旋转不定，站立不稳，或伴有恶心、呕吐、汗出、耳鸣、耳聋，甚则仆倒； （3）症状持续时间数秒至数天不等，影响正常生活和工作者； （4）签署知情同意书。 注：如以上任何一个答案为“否”，此受试者不能入组。 三、痴呆纳入标准 纳入标准 （1）年龄 ≥ 18 周岁 （2）主要表现为认知功能明显下降,尤其是自身前后对比,记忆力下降； （3）2个以上认知功能障碍,如定向、注意、言语、视空间功能、执行功能、运动控制等,其严重程度已干扰日常生活,并经神经心理学测试证实； （4）符合CT、MRI上相应病灶。 参考《美国精神疾病统计和诊断手册第4版》(DSM-Ⅳ)痴呆诊断标准 注：如以上任何一个答案为“否”，此受试者不能入组。 四、不寐纳入标准 纳入标准 （1）年龄 ≥ 18 周岁； （2）入睡困难或夜间睡眠浅，易觉醒，多梦，或比一般人要早醒 1-2 小时且醒后难以复睡，次日 醒后感困倦、疲乏或日间精力不足难以投入正常的社会生活活动 ； （3）每周至少发生三次上述失眠表现且持续至少 1 个月或者以上。 （4）伴有显著的不安，苦恼，焦虑，抑郁或恐惧心理，并导致精神 活动效率下降，妨碍其社会功能。 （5）排除躯体疾病或其他因素导致的继发性失眠。 参考《中国精神障碍分类与诊断标准第 3 版》(CCMD-3)失眠症诊断标准 注：如以上任何一个答案为“否”，此受试者不能入组。 五、泄泻纳入标准 1、纳入标准 是 否 （1）年龄 18 -70周岁； （2）近1年有肠镜检查且未见异常； （3）对本研究知情并同意参加本研究； （4）反复发作性的腹痛，最近3个月内平均每周至少有1天出现症状，合并以下2条或多条：①与排便相关。②伴排便频率改变。③伴大便性状（外观）改变。并排除其他肠道器质性疾病。 诊断前症状出现至少6个月，近3个月满足以上标准。 （5）稀便（糊状便）或水样便排便比例≥25%，硬便或块状便排便比例＜25%。 参考罗马Ⅳ功能性胃肠病关于腹泻型肠易激综合征诊断标准； 注：如以上任何一个答案为“否”，此受试者不能入组。

Inclusion criteria

1、 Inclusion criteria for stroke Inclusion criteria (1) Meets the diagnostic criteria for stroke recovery in Western medicine; (2) Meets the diagnostic criteria for meridians in traditional Chinese medicine for stroke; (3) Meets the traditional Chinese medicine syndrome differentiation criteria for qi deficiency and blood stasis syndrome; (4) The course of this illness: 14-180 days from the date of onset; (5) Individuals with a history of cerebral infarction and no disabilities prior to this episode (modified Rankin scale score ≤ 1) (6) Using the National Institutes of Health Stroke Scale (NIHSS) scoring criteria, the score for the degree of neurological deficit is ≥ 7 points and ≤ 22 points; (7) Individuals aged ≥ 40 years old and ≤ 80 years old; (8) Age sign informed consent form. Note: If any of the above answers are "no", this subject cannot be enrolled. 2、 Inclusion criteria for dizziness Inclusion criteria (1) Age ≥ 18 years old; (2) Feeling oneself or external scenery spinning, mild cases may stop with closed eyes, severe cases may be like riding a boat, spinning unpredictably, standing unsteadily, or accompanied by nausea, vomiting, sweating, tinnitus, deafness, and even falling down; (3) The duration of symptoms varies from seconds to days, affecting normal life and work; (4) Sign the informed consent form. Note: If any of the above answers are "no", this subject cannot be enrolled. 3、 Inclusion criteria for dementia Inclusion criteria (1) Age ≥ 18 years old (2) The main manifestation is a significant decline in cognitive function, especially in self comparison before and after, and a decrease in memory; (3) Two or more cognitive impairments, such as orientation, attention, speech, visual spatial function, executive function, motor control, etc., whose severity has disrupted daily life and has been confirmed by neuropsychological testing; (4) Corresponding lesions on CT and MRI. Refer to the Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM-IV) dementia diagnostic criteria Note: If any of the above answers are "no", this subject cannot be enrolled. 4、 Insomnia is included in the criteria Inclusion criteria (1) Age ≥ 18 years old; (2) Difficulty falling asleep or having shallow nighttime sleep, easily waking up, having multiple dreams, or waking up 1-2 hours earlier than the average person and having difficulty falling back asleep after waking up. The next day, feeling tired, fatigued, or having insufficient energy during the day, making it difficult to engage in normal social activities; (3) At least three times a week, the above-mentioned insomnia symptoms should occur and last for at least one month or more. (4) Accompanied by significant anxiety, distress, anxiety, depression or fear, and leading to a decrease in mental activity efficiency, it hinders its social function. (5) Exclude secondary insomnia caused by physical illness or other factors. Refer to the diagnostic criteria for insomnia in the Chinese Classification and Diagnostic Criteria for Mental Disorders, 3rd edition (CCMD-3) Note: If any of the above answers are "no", this subject cannot be enrolled. 5、 Inclusion criteria for diarrhea 1. Is it included in the criteria (1) Age 18-70 years old; (2) I have undergone colonoscopy examination in the past year and no abnormalities have been found; (3) Informed and agreed to participate in this study; (4) Recurrent abdominal pain, with an average of at least 1 symptom per week in the past 3 months, accompanied by 2 or more of the following: ① related to bowel movements. ② Accompanied by changes in defecation frequency. ③ Accompanied by changes in stool characteristics (appearance). And exclude other organic intestinal diseases. The symptoms before diagnosis have been present for at least 6 months and have met the above criteria in the past 3 months. (5) The proportion of loose stool (paste like stool) or watery stool is ≥ 25%, and the proportion of hard stool or block like stool is ＜ 25%. Refer to the diagnostic criteria for diarrhea predominant irritable bowel syndrome in Rome IV functional gastrointestinal diseases; Note: If any of the above answers are "no", this subject cannot be enrolled.

排除标准：

一、中风排除标准 排除标准 （1）短暂性脑缺血发作(TIA)； （2）既往曾患脑血管病，并留有运动功能障碍者； （3）影响药物评价的并发症，包括中风后的抑郁、痴呆等； （4）经检查证实由脑肿瘤、脑外伤、血液病等引起的中风患者； （5）因风湿性心脏病、冠心病及其他心脏病合并房颤，引起脑栓塞者； （6）有出血倾向者或12周内发生过严重出血者； （7）合并有其他影响肢体活动功能的疾病者，治疗前合并有骨关节炎、类风湿关节炎、痛风性关节炎等引起的肢体活动功能障碍可能影响神经或功能检查者； （8）本次疾病前因为各种疾病和体质造成不能独立完成日常活动等严重影响疗效评价者； （9）对本药物成分过敏及过敏体质者； （10）精神病患者； （11）其它原因不能按时服药者； （12）妊娠和哺乳期妇女； （13）正参加其它临床试验的患者； （14）研究人员认为其他原因不适合临床试验者。 注：如以上任何一个答案为“是”，此受试者不能入组。 二、眩晕排除标准 排除标准 （1）依从性差或不配合的患者； （2）妊娠或哺乳期妇女； （3）有严重的肝、心、肾、脑或造血系统及严重代谢性疾病或合并精神病或或 其他精神类疾病患者； （4）正在参加其他药物临床试验者。 注：如以上任何一个答案为“是”，此受试者不能入组。 三、痴呆排除标准 排除标准 （1）依从性差或不配合的患者； （2）妊娠或哺乳期妇女； （3）由意识障碍、谵妄等导致的记忆力减退和其他认知能力减退； （4）有严重的肝、心、肾、脑或造血系统及内分泌系统等严重原发性疾病； （5）正在参加其他药物临床试验者。 注：如以上任何一个答案为“是”，此受试者不能入组。 四、不寐排除标准 排除标准 是 否 （1）依从性差或不配合的患者； （2）妊娠或哺乳期妇女； （3）由躯体疾病导致的继发性失眠； （4）有严重的肝、心、肾、脑或造血系统及严重代谢性疾病或合并精神病或或 其他精神类疾病患者； （5）正在参加其他药物临床试验者。 注：如以上任何一个答案为“是”，此受试者不能入组。 五、泄泻排除标准 排除标准 是 否 （1）依从性差或不配合的患者； （2）妊娠或哺乳期妇女； （3）有严重的肝、心、肾、脑或造血系统及严重代谢性疾病或合并精神病或其他精神类疾病患者； （4）正在参加其他药物临床试验者； （5）资料不全影响信息采集者。 注：如以上任何一个答案为“是”，此受试者不能入组。

Exclusion criteria：

1 Exclusion criteria for stroke Exclusion criteria (1) Transient ischemic attack (TIA); (2) Individuals who have previously suffered from cerebrovascular disease and have motor dysfunction; (3) Complications that affect drug evaluation including depression and dementia after stroke; (4) Confirmed stroke patients caused by brain tumors traumatic brain injury hematological disorders etc. through examination; (5) Patients with cerebral embolism caused by rheumatic heart disease coronary heart disease and other heart diseases combined with atrial fibrillation; (6) Individuals with a tendency towards bleeding or those who have experienced severe bleeding within the past 12 weeks; (7) Patients with other diseases that affect limb movement function and those with limb movement dysfunction caused by osteoarthritis rheumatoid arthritis gouty arthritis etc. before treatment may affect neurological or functional examinations; (8) Prior to this illness the evaluator was severely affected by various illnesses and physical conditions that prevented them from independently completing daily activities; (9) Individuals who are allergic to the ingredients of this medication or have an allergic constitution; (10) Psychiatric patients; (11) Those who cannot take medication on time due to other reasons; (12) Pregnant and lactating women; (13) Patients currently participating in other clinical trials; (14) Researchers believe that other reasons are not suitable for clinical trial participants. Note: If any of the above answers are "yes" this subject cannot be enrolled. 2 Dizziness exclusion criteria Exclusion criteria (1) Patients with poor compliance or lack of cooperation; (2) Pregnant or lactating women; (3) Patients with severe liver heart kidney brain or hematopoietic system diseases as well as severe metabolic disorders or comorbidities of mental illness or other psychiatric disorders; (4) Participants in clinical trials of other drugs. Note: If any of the above answers are "yes" this subject cannot be enrolled. 3 Dementia exclusion criteria Exclusion criteria (1) Patients with poor compliance or lack of cooperation; (2) Pregnant or lactating women; (3) Memory loss and other cognitive impairment caused by consciousness disorders delirium etc; (4) Serious primary diseases such as liver heart kidney brain hematopoietic system and endocrine system; (5) Participants in clinical trials of other drugs. Note: If any of the above answers are "yes" this subject cannot be enrolled. 4 Insomnia exclusion criteria Does the exclusion criterion apply (1) Patients with poor compliance or lack of cooperation; (2) Pregnant or lactating women; (3) Secondary insomnia caused by physical illnesses; (4) Patients with severe liver heart kidney brain or hematopoietic system diseases as well as severe metabolic disorders or comorbidities of mental illness or other psychiatric disorders; (5) Participants in clinical trials of other drugs. Note: If any of the above answers are "yes" this subject cannot be enrolled. 5 Exclusion criteria for diarrhea Does the exclusion criterion apply (1) Patients with poor compliance or lack of cooperation; (2) Pregnant or lactating women; (3) Patients with severe liver heart kidney brain or hematopoietic system diseases as well as severe metabolic disorders or comorbidities of mental illness or other psychiatric disorders; (4) Participants in clinical trials of other drugs; (5) Incomplete data affects information collectors. Note: If any of the above answers are "yes" this subject cannot be enrolled.
