研究目的:

基于优化的中医证候诊断量表遴选快速进展DKD患者,采用适宜中医药研究的母方案设计,对“肾虚络瘀”核心证候体系(肾虚络瘀证、肾虚络瘀兼内热证、肾虚络瘀兼湿浊证)下的“补肾填精通络消癥”方案延缓快速进展DKD的疗效进行循证研究,优化防治方案。

Objectives of Study:

Based on the optimized TCM syndrome diagnosis scale to select the patients with rapidly progressive DKD and adopt the maternal scheme design suitable for TCM research objective: to study the effect ofTonifying the kidney to master collaterals and dissipating syndrome on delaying the rapid development of DKD under the core syndrome system ofKidney deficiency and collaterals stasis(kidney deficiency and collaterals stasis with internal heat syndrome kidney deficiency and collaterals stasis with dampness turbidity syndrome)

Description for medicine or protocol of treatment in detail:

纳入标准:

1: 年龄18-80岁之间,性别不限; 2: 符合糖尿病肾脏病 (DKD) 诊断标准;糖尿病 (DM) 诊断标准参考《中国2型糖尿病防治指南(2020年版)》。DKD 诊断标准参考《中国糖尿病肾脏病防治指南(2021年版)》制定:糖尿病病史同时与尿蛋白、肾功能变化存在因果关系,并排除其他原发、继发性肾小球疾病,与系统性疾病,符合以下情况之一的可以诊断为 DKD。①排除干扰因素的情况下,在3-6个月内的3次检测中至少2次尿微量白蛋白/尿肌酐 (UACR)≥30mg/g或UACR≥30mg/24h(≥20ug/min)。②估算肾小球滤过率(eGFR)<60ml/(min·1.73m²) 持续3个月以上。③肾活检符合DKD 的病理诊断; 3: 符合糖尿病肾脏病快速进展:UACR≥300mg/g 或24-UTP >0.5g/24h, 且 eGFR≥15mL/(min·1.73m²) 4: 符合优化的中医证候诊断量表判定为肾虚络瘀证或肾虚络瘀证兼内热证或肾虚络瘀证兼湿浊证患者; 5: 充分了解研究目的,自愿签署知情同意书。

Inclusion criteria

1: Age between 18 and 80 years old regardless of gender ; 2: Conform to the diagnostic criteria of diabetic kidney disease ( DKD ) ; the diagnostic criteria of diabetes mellitus ( DM ) refer to the ' Chinese Guidelines for the Prevention and Treatment of Type 2 Diabetes ( 2020 Edition ) '. The diagnostic criteria of DKD refer to the ' Guidelines for the Prevention and Treatment of Diabetic Kidney Disease in China ( 2021 Edition ) ' : the history of diabetes is also causally related to urinary protein and renal function changes and excludes other primary and secondary glomerular diseases and systemic diseases. DKD can be diagnosed if one of the following conditions is met. 1 Excluding the interference factors at least 2 times of urine microalbumin / urine creatinine ( UACR ) ≥ 30mg / g or UACR ≥ 30mg / 24h ( ≥ 20ug / min ) in 3-6 months. Estimated glomerular filtration rate ( eGFR ) < 60ml / ( min · 1.73m2 ) lasted for more than 3 months. 3 Renal biopsy was consistent with the pathological diagnosis of

排除标准:

1: 明确诊断的原发性肾小球疾病、隐匿性肾炎、自身免疫性疾病及结缔组织病、血液病、肿瘤、药物继发肾损害、双侧肾动脉狭窄患者; 2: 24小时尿蛋白定量>3 . 5g/24h; 3: 6个月内发生糖尿病急性并发症如糖尿病酮症酸中毒、高渗高血糖昏迷的患者; 4: 合并有严重的呼吸、消化、血液系统等原发疾病的患者,射血分数降低的心力衰竭者(LVEF≤40%)。目前发生严重感染、创伤、重大手术、急性冠脉综合征、急性心力衰竭、未控制的恶性心律失常等其他急症的应激状态下的患者; 5: 妊娠、哺乳、有生育计划者; 6: 活动性肝炎及肝功能受损者(谷丙转氨酶、谷草转氨酶大于正常上限2倍); 7: 1型糖尿病及特殊类型糖尿病; 8: 血压控制不佳:收缩压≥160mmHg和/或舒张压≥100mmHg,或低血压:收缩压<90mnHg和/或舒张压<60mmHg; 9: 糖化血红蛋白(HbA1c)≥9%,空腹血糖>8mmol/L; 10: 血清钾浓度超过研究单位实验室正常值上限; 11: 近3个月内降血糖、降血脂的基础治疗不稳定的患者近1个月内降血压的基础治疗不稳定(更换药物类型、如β受体阻断剂更换为钙离子拮抗剂等)的患者; 12: 近2周内开始服用:a. 治疗 DKD的其他药物(非奈利酮,GLP-1类药物等),b.治疗糖尿病视网膜病变的羟苯磺酸钙,c.具有相同成分或功能主治的中成药、中药汤剂、配方颗粒等中药制剂,d. 雷公藤相关制剂、黄葵胶囊、渴络欣胶囊等; 13: 正在使用抗凝药,剂量稳定未达到1个月; 14: 临床研究者认为参与临床研究的受试者不能配合观察时。

Exclusion criteria:

1: Clearly diagnosed patients with primary glomerular disease occulted nephritis autoimmune disease and connective tissue disease blood disease tumor drug-induced renal damage and bilateral renal artery stenosis; 2: 24-hour urinary protein quantity > 3.5g /24h; 3: Patients with acute complications of diabetes such as diabetic ketoacidosis and hyperosmolar hyperglycemia coma within 6 months; 4: Patients with severe respiratory digestive blood system and other primary diseases heart failure patients with reduced ejection fraction (LVEF≤40%). Patients currently under stress with severe infection trauma major surgery acute coronary syndrome acute heart failure uncontrolled malignant arrhythmia and other emergencies; 5: Pregnancy lactation family planning; 6: Active hepatitis and liver function impairment (alanine aminotransferase aspartate aminotransferase greater than 2 times the upper limit of normal); 7: Type 1 diabetes mellitus and specific type diabetes mellitus; 8: Poor bloo
