研究目的：

随着人口老龄化进程加速及健康管理需求提升，本研究依托"药王"孙思邈中医药文化发源地（陕西铜川）的学术优势，通过前瞻性队列研究，动态监测老年疾病发生发展规律，建立疾病演变预测模型；基于多维度健康数据分析，为优化诊疗方案、预防策略提供科学依据。

Objectives of Study：

With the acceleration of population aging and the increasing demand for health management this study draws on the academic strengths of the birthplace of Sun Simiao's traditional Chinese medicine culture (Tongchuan Shaanxi). Through a prospective cohort study it dynamically monitors the development patterns of geriatric diseases and establishes disease progression prediction models. Based on multi - dimensional health data analysis it provides a scientific basis for optimizing treatment plans and prevention strategies.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

①基线调查年龄为50 岁≤年龄≤75岁；男女不限；②在调查地区居住满6个月及以上的常住人口；③自愿参加并签署知情同意书。

Inclusion criteria

①Age Requirement: Baseline age 50–75 years (inclusive) with no gender restrictions.②Residency Status: Permanent resident in the study area for ≥6 months prior to enrollment.③Informed Consent: Voluntary participation with signed informed consent form.

排除标准：

①严重慢性疾病或慢性疾病急性加重期，如：慢性阻塞性肺疾病急性加重期、慢性心力衰竭NYHA IV级、肾功能不全（eGFR＜15ml/1.73m2·min或需要肾脏替代治疗）、自身免疫性疾病急性活动期、正在接受治疗的恶性肿瘤患者；②急性重症疾病，如：急性心力衰竭、急性肝功能衰竭、急性肾功能衰竭、急性呼吸窘迫综合征、急性冠脉综合征、急性脑卒中、重症肺炎、脓毒血症等；③严重传染性疾病，如开放性肺结核、艾滋病等；④妊娠或哺乳期妇女；⑤严重失能、严重精神疾病、读写认知功能障碍等不能配合者。

Exclusion criteria：

①Severe chronic diseases or acute exacerbation of chronic diseases such as: acute exacerbation of chronic obstructive pulmonary disease chronic heart failure (NYHA IV) renal insufficiency (eGFR <15 ml/min/1.73m2 or requiring renal replacement therapy) active autoimmune diseases or patients undergoing treatment for malignant tumors;②Acute critical illnesses such as: acute heart failure acute liver failure acute renal failure acute respiratory distress syndrome acute coronary syndrome acute stroke severe pneumonia sepsis etc.;③Severe infectious diseases such as open pulmonary tuberculosis AIDS etc.;④Pregnant or breastfeeding women; ⑤Severely disabled individuals those with severe mental illness or those with read/write cognitive impairments who cannot cooperate.
