研究目的：

通过随机对照试验的方法，研究并评估一种采用多切口拖管与挂线相结合的微创手术的安全性与有效性；同时分析该手术方式在缩短伤口愈合时间、减轻患者症状评分、提高肛门功能评估结果以及改善肛肠动力学指标等方面的潜在益处。

Objectives of Study：

Using the methodology of randomized controlled trials this study investigates and evaluates the safety and efficacy of a minimally invasive surgical procedure that combines multi-incision tube traction with ligature techniques. Simultaneously it analyzes the potential benefits of this surgical approach in reducing wound healing time alleviating patient symptom scores improving the outcomes of anal function assessments and enhancing anorectal motility indices.

Description for medicine or protocol of treatment in detail：

纳入标准：

①符合国家中医药管理局《中医病证诊断疗效标准》及中华中医药学会肛肠分会、中华医学会结直肠肛门外科学组、中国中西医结合学会大肠肛门病专业委员会联合发布的2006年版《肛瘘临床诊治指南》中关于高位马蹄型肛瘘（含全马蹄型与半马蹄型）的临床诊断标准；②采用经直肠超声（TRUS）或肛周增强磁共振（EMRI）检查得到确诊；③年龄在18～65岁之间的住院病人，男女不限；④术前肛门无畸形；⑤已签署知情同意书者。

Inclusion criteria

①Diagnosis of high horseshoe-type anal fistula (complete horseshoe-type/partial horseshoe-type) according to the Criteria for Diagnosis and Therapeutic Efficacy of TCM Diseases and Syndromes (National Administration of Traditional Chinese Medicine) and the 2006 Clinical Guidelines for Anal Fistula Management (jointly issued by ​​the​​ Chinese Society of Coloproctology Chinese Association of Traditional Chinese Medicine​​​​ Colorectal and Anal Surgery Group​​​​ Chinese Medical Association​​​​ and Colorectal and Anorectal Diseases Committee​​​​ Chinese Association of Integrative Medicine);②Confirmation via transrectal ultrasound (TRUS) or contrast-enhanced perianal MRI (EMRI);③Hospitalized patients aged 18–65 years with no sex-based exclusion criteria;④Anatomically intact anal canal;⑤Written informed consent obtained

排除标准：

①妊娠和哺乳期妇女；②经电子结肠镜检查发现的恶性肿瘤、特异性感染（如结核、克罗恩病、溃疡性结肠炎等）；③ 伴有重度混合痔、肛裂、肛周湿疹等其它肛周疾病；④合并循环系统、造血系统、消化系统和重要脏器严重疾病者或精神病者；⑤严重过敏、凝血机能障碍者；⑥近四周内参加过其它临床试验病人。

Exclusion criteria：

①Pregnant or lactating women;②Malignancies or specific infections (e.g. tuberculosis Crohn's disease ulcerative colitis) confirmed by colonoscopy;③Concurrent severe anorectal disorders including mixed hemorrhoids anal fissures or perianal eczema;④Severe systemic comorbidities (e.g. circulatory hematopoietic or digestive system dysfunction major organ failure) or psychiatric disorders;⑤Severe allergies or coagulation dysfunction;⑥Participation in other clinical trials within the preceding 4 weeks.

采用随机数字法进行随机分组。在每一位患者中产生0-1之间的均匀随机数，大于0.5的患者入组“多切口拖管联合挂线术”组，即“试验组”；而小于0.5的患者进入“切开挂线术”组，即“对照组”。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Randomization was performed using a random number method. For each patient a uniformly distributed random number between 0 and 1 was generated. Patients with a random number ​>0.5 were assigned to the ​Multiple Incisions Tract-Draining Tubes and Suspension Suture Technique group (experimental group) while those with a random number ≤0.5 were assigned to the ​Incision and threading group (control group).
