研究目的：

探讨音乐联合针灸干预对全面性发育迟缓（GDD）患儿的神经发育、情绪行为及社会生活能力的改善效果，验证其协同治疗的有效性与安全性，为GDD康复提供中西医结合的新策略。

Objectives of Study：

To investigate the effects of music combined with acupuncture intervention on neurodevelopment emotional-behavioral outcomes and social adaptive skills in children with Global Developmental Delay (GDD) validate the efficacy and safety of this synergistic therapy and propose a new integrative strategy combining traditional Chinese and Western medicine for GDD rehabilitation.

药物成份或治疗方案详述：

（1）疗程安排 每天治疗1次，每周连续治疗5天，休息2天；连续治疗3周休息1周，为1疗程；连续治疗3个疗程。分别在治疗结束后3个月进行随访。治疗场地位于门诊治疗室或住院部。 （2）针灸干预 1）主穴选取靳三针 ①四神针：百会穴前后左右各旁开1.5寸，共4穴，采用方向为由百会向外四周针刺，针刺角度小于15°，针刺深度0.5~0.8寸； ②智三针：神庭及双侧本神，共3穴，方向由颜面向头顶方向针刺，针刺角度小于15°，针刺深度0.5~0.8寸； ③颞三针：双侧耳尖上入发际2寸及同一水平前后各1寸，共6穴，方向由上向下平刺，针刺角度小于15°，针刺深度0.5~0.8寸； ④脑三针：脑户及双侧脑空，共3穴，方向由上向下平刺，针刺角度小于15°，针刺深度0.5~0.8寸； ⑤手三针：双侧合谷、曲池、外关，共6穴，直刺，针刺深度0.5~0.8寸； ⑥足三针：双侧足三里、三阴交、太冲，共6穴，直刺，针刺深度0.5~0.8寸。 2）针具型号：选用0.25mm×0.25mm不锈钢毫针。 3）留针时间：“徐入”手法进针，留针20分钟后，使用“徐出”手法出针，将体针全部取出，头针继续留针40分钟，同时进行音乐疗法。 （3）音乐干预： 1）选乐：选用五音乐曲，在中华医学会音像出版社出版的《中国传统五行音乐正调式》中选取宫音、羽音交替播放，宫音选乐《春花江月夜》、《平湖秋月》、《塞上曲》、《月儿高》，羽音选乐《二泉映月》、《梁祝》、《汉宫秋月》、《梅花三弄》。 2）播放地点：在音乐治疗室中使用功放媒体设备（播放器：手机、mp3、录音机、平板选一），若无音乐治疗室，则使用耳机或在单间对每个患儿进行单独播放，不可互相干扰；于头针留针的同时进行音乐播放。 3）播放音量：音量＜40dB 4）播放时间：每日1次，每次40分钟。 4.常规康复治疗 基础治疗：引导式教育、感觉统合训练、作业疗法、情景互动、运动疗法PT、OT等。

Description for medicine or protocol of treatment in detail：

1 Treatment schedule Treatment is administered once daily for 5 consecutive days per week followed by 2 days of rest. Each course consists of 3 consecutive weeks of treatment followed by 1 week of rest. A total of 3 consecutive courses are completed. Follow-up visits were conducted 3 months after the end of treatment respectively. The treatment site was located in the outpatient treatment room or inpatient department. 2 Acupuncture intervention 2.1 The main acupuncture points were selected as Jin San acupuncture points. 2.1.1 Sishenzhen (Four Spirit Needles): Four points located 1.5 cun anterior posterior left and right to Baihui (GV20). Needles are inserted from Baihui outward at an angle <15° depth 0.5–0.8 cun. 2.1.2 Zhisanzhen (Intelligence Three Needles): Shenting (GV24) and bilateral Benshen (GB13). Needles are inserted from the face toward the vertex at an angle <15° depth 0.5–0.8 cun. 2.1.3 Niesanzhen (Temporal Three Needles): Six points: 2 cun above the auricular apex on the bilateral hairline plus 1 cun anterior and posterior to this point on each side. 2.1.4 Naosanzhen (Brain Three Needles): Naohu (GV17) and bilateral Naokong (GB19). Horizontally inserted from superior to inferior at an angle <15° depth 0.5–0.8 cun. 2.1.5 Shousanzhen (Hand Three Needles): Bilateral Hegu (LI4) Quchi (LI11) and Waiguan (TE5). Perpendicular insertion depth 0.5–0.8 cun. 2.1.6 Zusanzhen (Foot Three Needles): Bilateral Zusanli (ST36) Sanyinjiao (SP6) and Taichong (LR3). Perpendicular insertion depth 0.5–0.8 cun 2.2 Needle specifications: Stainless steel filiform needles 0.25 mm in diameter and 25 mm in length. 2.3 Needle retention and manipulation 2.3.1 Insertion: Slow insertion technique ("Xu-in") is applied. 2.3.2 Retention: Body needles are retained for 20 minutes then removed using the slow withdrawal technique ("Xu-out"). 2.3.3 Head needles: Retained for an additional 40 minutes during music therapy. 3 Music intervention 3.1 Music selection 3.1.2 Type: Pentatonic music from Chinese Five-Element Music in Standard Modes published by the Chinese Medical Association Audio-Video Press. 3.1.3 Tones alternated 3.1.3.1 Gong-mode: Chunjiang Huayueye (Spring River Flower Moon Night) Pinghu Qiuyue (Autumn Moon over the Calm Lake) Saishang Qu (Song of the Frontier)Yueer Gao (The Moon Rises High) 3.1.3.2 Yu-mode: Erquan Yingyue (Moon Reflected in the Second Spring) Liang Zhu (Butterfly Lovers) Han Gong Qiuyue (Autumn Moon over the Han Palace) Meihua Sannong (Three Variations on the Plum Blossom). 3.2 Playback settings 3.2.1 Location: Conducted in a dedicated music therapy room using amplified media equipment (e.g. smartphone MP3 player tablet or digital recorder). 3.2.2 Alternate settings (if no dedicated room): Use headphones for each child. Play music individually in separate rooms to avoid interference. 3.2.3 Timing: Music is played during the retention of head needles. 3.3 Volume: <40 dB (measured at the childs ear level). 3.4 Session duration: 40 minutes per session once daily. 4 Basic treatments 4.1 Conductive education 4.2 Sensory integration training 4.3 Occupational therapy (OT) 4.4 Situational interaction therapy 4.5 Exercise therapy 4.6 Physical therapy (PT) 4.7 other standard rehabilitation interventions.

纳入标准：

（1）符合 2013 年美国《精神障碍诊断与统计手册》（DSM-5）中 GDD的诊断标准； （2）年龄2～5岁，性别不限； （3）其家属知情并签署知情同意书。

Inclusion criteria

1.Meet the diagnostic criteria for Global Developmental Delay (GDD) as defined in the Diagnostic and Statistical Manual of Mental Disorders Fifth Edition (DSM-5; American Psychiatric Association 2013). 2. Aged 2–5 years regardless of gender. 3. Legal guardians are informed of the study details and sign an informed consent form.

排除标准：

（1）其他任何可导致脑功能紊乱的严重躯体或精神疾患； （2）明确头颅外伤史；入组前1个月曾服用可导致认知功能损害药物者； （3）共患癫痫、严重视听觉障碍、心血管疾病、呼吸系统疾病等不适合进行康复训练者； （4）不能听懂简单指令，配合评定及训练的患儿。

Exclusion criteria：

1. Any severe physical or mental disorder that may cause cerebral dysfunction. 2. Documented history of head trauma or use of medications associated with cognitive impairment within 1 month prior to enrollment. 3. Comorbid conditions contraindicating rehabilitation (e.g. epilepsy severe visual/hearing impairment cardiovascular or respiratory diseases). 4. Inability to comprehend simple instructions or cooperate with assessments/training.

由研究团队专人进行随机分组，采用随机数字表法，计算机生成随机数字。 操作步骤： (1)编号：60例患者按照就诊顺序进行编号； (2)获取随机数字：从随机数字表中任意一个数开始，沿同一方向顺序获取每例患者一个随机数字； (3)求余数：随机数除以2求余数 (4)分组：按余数分组，1为对照组，0为治疗组。

Randomization was performed by designated researchers using the random number table method with computer-generated random numbers. Operational steps: 1. Numbering: 60 patients are sequentially numbered according to their enrollment order. 2. Random number assignment A starting point is randomly selected within the random number table. Sequential numbers are assigned to each patient in a fixed direction. 3. Calculation of remainder: Each random number is divided by 2. 4. Group allocation Remainder 1: Assigned to the control group. Remainder 0: Assigned to the treatment group.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据录入员及时、准确地将病例报告表中的数据录入CRF，所有受试者完成试验，病例报告表全部录入系统，并经数据管理员的审核无误后，由数据管理员对数据进行锁定。数据全部锁定后，由数据管理员将其导入到指定数据库，交统计人员进行统计分析。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The data entry personnel timely and accurately input the data in the case report form into the CRF. All subjects completed the experiment and all the case report forms were entered into the system. After being reviewed by the data administrator and found to be correct the data was locked by the data administrator. After all the data is locked the data administrator will import it into the designated database and submit it to the statistical personnel for statistical analysis.
