研究目的：

观察“益髓醒神”针刺方案对急性卒中伴认知障碍的治疗作用，并评估其经济性、安全性和实用性，为中医针刺治疗卒中伴认知障碍提供高质量循证证据。

Objectives of Study：

To observe the efficacy of "supplementing the spirit and awakening the spirit" acupuncture program on cognitive impairment in acute stroke patients and evaluate its economy safety and practicability so as to provide high-quality evidence-based evidence for TCM acupuncture treatment of stroke accompanied by cognitive impairment.

药物成份或治疗方案详述：

（1）治疗组：“益髓醒神”针刺方案：以醒神开窍，补肾益髓为原则，取穴通里、悬钟、百会、四神聪、合谷、太冲等。每周5次，留针30分钟，持续8周。 （2）对照组：采用“益髓醒神”针刺方案对应的非经非穴进行针刺。每周5次，留针30分钟，持续8周。

Description for medicine or protocol of treatment in detail：

(1) Treatment group: "Yi Sui Xing Shen" Acupuncture Rehabilitation Treatment: on the principle of awakening the spirit and opening the orificium tonifying the kidney and supplementing the marrow select points Tongli Hanging Zhong Baihui Sishencong Hegu Taichong etc. The patients will be treated 5 times a week for 30 minutes each time for a total of 8 weeks. (2) Control group: The non-meridians and non-acupoints corresponding to the acupuncture scheme of benefiting the marrow and awakening the spirit will be used for acupuncture. The patients will be treated 5 times a week for 30 minutes each time for a total of 8 weeks.

纳入标准：

①3-14天内发生过可明确溯源的1次或1次以上急性脑血管病事件（缺血性），经CT或MRI诊断为脑梗死；②患者本人或家属主观报告在卒中事件发生之后出现的认知下降表现，且经神经内科或中医脑病科临床评估后，并经神经心理学量表确认未达到痴呆诊断标准，但需存在注意/执行功能、记忆、语言和视空间功能等至少1个认知域受损。日常生活能力可正常或轻度受损，但该日常生活能力受损与卒中事件导致的运动/感觉症状无关。③认知神经心理学量表：MOCA<26分，且MMSE文盲>17分、小学>20分、中学及以上>24分；④根据爱丁堡利手量表评分判定为右利手（EHI评分＞40）；⑤发病年龄18~80岁，性别及民族不限；⑥NIHSS评分＜20分；⑦受试者本人知情同意，并自愿签署知情同意书。

Inclusion criteria

(1) One or more acute cerebrovascular disease (ischemic) events occurred within 3 to 14 days which can be clearly traced and cerebral infarction was diagnosed by CT or MRI; (2) Patients or family members subjectively reported cognitive decline after the stroke event with normal or mildly impaired daily living ability but the impaired daily living ability was not associated with motor/sensory symptoms caused by the stroke event. (3) Cognitive neuropsychological scale: MOCA<26 points MMSE illiteracy >17 points primary school >20 points middle school and above >24 points; (4) judged to be right-handed (EHI score > 40); (5) The age of onset is 18-80 years old gender and nationality are not limited; (6)NIHSS score < 20 points; (7) The subject gives informed consent and signs the informed consent voluntarily.

排除标准：

①卒中事件发病前已有明确的任何已知认知障碍的诊断，包括酒精所致认知障碍等； ②可能影响神经心理评估的症状或表现，例如严重听觉、语言或情感障碍、未能控制的活动性精神疾病，卒中后的身体残障影响心理评估等； ③脑卒中合并有严重心肺功能异常、肝肾功能不全、生命体征不稳定等，合并严重肺部感染、泌尿系感染、褥疮等可能影响后期随访的患者； ④重度血管性痴呆：至少一个认知域存在显著的认知障碍，且脑血管事件与认知障碍发生之间存在明显的时间关系，且认知障碍的严重程度影响到日常生活独立性； ⑤患者在筛选期前1个月内接受过认知增强剂和其他抗痴呆药物治疗，包括但不限于胆碱酯酶抑制剂（多奈哌齐、卡巴拉汀、加兰他敏）和非竞争性N-甲基-D-天冬氨酸（NMDA）受体拮抗剂（美金刚）和其他药物（如甘露醇钠胶囊、奥拉西坦、阿尼西坦、吡拉西坦、来卡奈单抗、多纳尼单抗、阿度卡奈单抗等）； ⑥妊娠或哺乳期妇女。

Exclusion criteria：

(1) Any known cognitive impairment including alcohol-induced cognitive impairment has been diagnosed before the onset of the stroke event; (2) Symptoms or manifestations that may affect the neuropsychological evaluation such as severe hearing speech or emotional disorders uncontrolled active mental illness and post-stroke physical disabilities that affect the psychological evaluation; (3) Patients with severe comorbidities such as severe cardiopulmonary dysfunction liver and kidney dysfunction and unstable vital signs; (4) Patients had been treated with cognitive enhancers and other anti-dementia agents including but not limited to cholinesterase inhibitors and non-competitive n-methyl-D-aspartate receptor antagonists within 1 month prior to the screening period; (5) Pregnant or lactating women.
