研究目的:

①评估多组分运动联合经颅磁刺激对肌少症伴轻度认知障碍患者的肌肉适能和认知功能的改善效果; ②探讨多组分运动联合经颅磁刺激改善肌少症伴轻度认知障碍的潜在神经机制及“脑肌轴”相关作用机制。

Objectives of Study:

①To evaluate the effects of multi-component exercise combined with transcranial magnetic stimulation on muscle fitness and cognitive function in patients with sarcopenia with mild cognitive impairment; ②To investigate the underlying neural mechanisms and the brain-muscle axis related mechanisms of multi-component exercise combined with transcranial magnetic stimulation to improve the muscle fitness and cognitive function of patients with sarcopenia with mild cognitive impairment.

Description for medicine or protocol of treatment in detail:

纳入标准:

①年龄≥60岁; ②符合亚洲肌少症工作组(AWGS) 2019对肌少症的诊断标准:用生物电阻抗分析仪(BIA)测量四肢骨骼肌质量指数(ASMI) (男性<7.0 kg/m2;女性<5.7 kg/m2)和握力(男子<28kg及以下;女子<18kg)和/或6米步行速度(<1m/s); ③符合2024年《阿尔茨海默病源性轻度认知障碍诊疗中国专家共识》中MCI的诊断标准:主观认知下降,客观认知测试异常(蒙特利尔认知评估量表(MoCA)得分<26,简易精神状态检查量表(MMSE)得分<27),日常功能保持完整;未达到痴呆诊断标准; ④无规律的运动习惯(规律运动定义为每周至少3天进行至少20分钟的剧烈运动,或每周至少5天进行至少30分钟的中等强度运动); ⑤经颅磁刺激适用性:无金属植入物(如脑起搏器)、心脏起搏器或其他影响经颅磁刺激的禁忌症;头皮适合放置经颅磁设备,无明显头皮过厚或脂肪过多; ⑥自愿参与本研究,并能按要求完成干预并签署知情同意书。

Inclusion criteria

① Age ≥ 60 years; ② Meet the diagnostic criteria for sarcopenia in the Asian Working Group on Sarcopenia (AWGS) 2019: measurement of limb skeletal muscle mass index (ASMI) (<7.0 kg/m2 for men; <5.7 kg/m2 for women) and grip strength (<28 kg and below for men; <18 kg for women) and/or 6-meter walking speed (<1 m/s) by bioelectrical impedance analyzer (BIA); and ③ Meet the diagnostic criteria for MCI in the 2024 Chinese Expert Consensus on the Diagnosis and Treatment of Mild Cognitive Impairment of Alzheimer's Disease Origin: subjective cognitive decline abnormalities in objective cognitive tests (Montreal Cognitive Assessment Scale (MoCA) scores <26 and Brief Mental State Examination (MMSE) scores <27) and intact daily functioning; and do not meet the diagnostic criteria for dementia; ④ Irregular exercise habits (regular exercise is defined as at least 20 minutes of vigorous exercise on at least 3 days per week or at least 30 minutes of moderate-intensity exercise on at least 5 days per week); ⑤ Suitability for transcranial magnetic stimulation: no metal implants (e.g. brain pacemakers) cardiac pacemakers or other contraindications affecting transcranial magnetic stimulation; scalp is suitable for placement of transcranial magnetic devices and there is no obvious excessive scalp thickness or excess fat; ⑥Voluntary participation in this study and ability to complete the intervention and sign the informed consent form as required.

排除标准:

①存在严重的认知障碍、精神疾病、听力障碍或视力障碍,导致无法配合研究; ②明确诊断的脑卒中、脑外伤史、癫痫、脑肿瘤或其他脑部疾病; ③当前患有严重抑郁症、精神分裂症或其他严重精神疾病; ④存在严重躯体疾病:未控制的高血压(收缩压>200 mmHg或舒张压>110 mmHg),心律失常或严重心血管疾病(如中风、急性心肌梗死、心绞痛、未控制的室速),中度或重度慢性阻塞性肺疾病(COPD); ⑤头皮有严重皮疹、感染或无法放置fNIRS或rTMS设备; ⑥过去3个月内定期服用可能干扰认知功能(如抗精神病药物、高剂量糖皮质激素)或运动功能的药物(如抗血小板药物、镇痛药物或抗凝血药); ⑦存在运动障碍:运动的绝对禁忌症(如深静脉血栓和/或凝血功能障碍),骨骼肌肉疾病(如骨折、脱臼),传染性皮肤病、未愈合的伤口或溃疡,残疾(如手或脚丧失)。

Exclusion criteria:

①Presence of severe cognitive impairment mental illness hearing impairment or visual impairment that prevents cooperation with the study; ②A definite diagnosis of stroke history of traumatic brain injury epilepsy brain tumor or other brain disease; ③Current major depression schizophrenia or other serious mental illness; ④ Presence of serious physical illness: uncontrolled hypertension (systolic blood pressure >200 mmHg or diastolic blood pressure >110 mmHg) cardiac arrhythmia or serious cardiovascular disease (e.g. stroke acute myocardial infarction angina pectoris uncontrolled ventricular tachycardia) moderate or severe chronic obstructive pulmonary disease (COPD); ⑤ Severe rash infection or inability to place fNIRS or rTMS devices on the scalp; ⑥Regular use of medications that may interfere with cognitive function (e.g. antipsychotics high-dose glucocorticoids) or motor function (e.g. antiplatelet agents analgesics or anticoagulants) within the past 3 months; ⑦ Presence of movement disorders: absolute contraindications to exercise (e.g. deep vein thrombosis and/or coagulation disorders) musculoskeletal disorders (e.g. fractures dislocations) infectious skin diseases unhealed wounds or ulcers disabilities (e.g. loss of hand or foot).

在本研究中,符合条件的60名参与者将被随机分配到三个等大小的组别:运动联合rTMS组(n=20)、运动联合假刺激组(n=20)和对照组(n=20)。这一随机化过程由一位不参与研究的统计学家负责,使用SPSS软件版本27(由SPSS公司,美国提供)来生成简单的随机化序列。为了确保研究的严谨性,随机化过程将根据性别和年龄进行分层,以保证两组在关键人口统计特征上的平衡。

Randomization Procedure (please state who generates the random number sequence and by what method):

In this study 60 eligible participants will be randomly assigned to three equal-sized groups: an exercise combined rTMS group (n=20) an exercise combined sham stimulation group (n=20) and a control group (n=20). This randomization process was carried out by a statistician who was not involved in the study using SPSS software version 27 (provided by SPSS Inc. USA) to generate simple randomization sequences. To ensure the rigor of the study the randomization process will be stratified by sex and age to ensure that the two groups are balanced on key demographic characteristics.
