研究目的：

(1) 主要目的:通过任务态功能磁共振成像,结合不同食物图片刺激下的大脑功能区域活动水平的变化及其与行为数据间的联系,研究真/假针刺前后的即时效应和长期治疗一个疗程前后的长期效应对大脑功能的影响。 (2) 次要目的:观察电针治疗肥胖的临床疗效。

Objectives of Study：

(1) The main objective is to investigate the immediate and long-term effects of real and sham acupuncture on brain function through task-state functional magnetic resonance imaging (fMRI) by analyzing the changes in the activity levels of brain functional regions under the stimulation of different food images and their correlation with behavioral data as well as the long-term effects after one course of treatment. (2) The secondary objective is to observe the clinical efficacy of electroacupuncture in treating obesity.

纳入标准：

入选标准 肥胖受试者组 ①18岁≤年龄≤65岁，性别不限； ②BMI≥27kg/m2； ③女性腰围≥85cm（2尺6（33）码及以上裤子）； ④男性腰围≥90cm（2尺7（34）码及以上裤子）； ⑤在参加本研究之前未进行与肥胖相关治疗的患者； ⑥自愿加入治疗并签署知情同意书。 健康受试者组 ①18岁≤年龄≤65岁，性别不限； ②18.5kg/m2≤ BMI≤ 23.9 kg/m2； ④右利手； ⑤无高血压、糖尿病、心脏病、精神疾病发作等疾病史； ⑥健康组性别、年龄及受教育程度与肥胖受试者组相对匹配。

Inclusion criteria

Obesity Subject Group ①Age 18≤age≤65 years gender not limited; ②BMI≥27kg/m2; ③Female waist circumference≥85cm(2 yards 6 (33) inches or more); ④Male waist circumference≥90cm(2 yards 7 (34) inches or more); ⑤Patients who have not received treatment related to obesity before participating in this study; ⑥Voluntarily join the treatment and sign an informed consent form. Healthy Subject Group ①Age 18≤age≤65 years gender not limited; ②18.5kg/m2≤BMI≤23.9 kg/m2; ④Right-handed; ⑤No history of hypertension diabetes heart disease or mental illness; ⑥The gender age and educational level of the healthy group are matched with those of the obese subject group.

排除标准：

①年龄＜18岁或年龄＞65岁者； ②BMI＜27kg/m2或BMI＜18.5kg/m2； ③继发性肥胖者； ④妊娠期或哺乳期者； ⑤装有心脏起搏器，严重惧针、晕针或金属过敏者； ⑥有凝血功能障碍或服用抗凝药，或患有肿瘤、血液等相关性疾病，或有严重心、肝、肾损伤等，或者一般状况差，无法配合的患者。

Exclusion criteria：

①Individuals under the age of 18 or over the age of 65; ②Individuals with a BMI of less than 27 kg/m2 or less than BMI<18.5kg/m2; ③Individuals with secondary obesity; ④Pregnant or lactating women; ⑤Individuals with pacemakers severe phobia of needles hypodermic needle phobia or metal allergy; ⑥Individuals with impaired coagulation function or taking anticoagulants or those with related diseases such as tumors blood disorders or severe heart liver or kidney damage or those with poor general condition and unable to cooperate.

尽管基于效力计算样本量更具规范性，但相关理论的发展应用十分局限，且缺乏可使用的计算模型及工具。针灸影像学研究不同于一般的临床试验，不能照搬传统的样本量计算方法。在当前情况下，根据成像方案自身特点直接设定满足统计效力的样本量可能是较为合理的方式。研究发现，近年针灸影像学研究中单组样本量为12例以上已基本形成共识，样本量的估算范围主要为20～30例，每组20例成为多数研究的首要选择，这与《针灸影像学》建议的在伦理和试验条件允许的情况下，单组样本量最好达到20例相一致。因此，本研究将根据《针灸影像学》的建议和当前的试验条件，选取每组20例作为研究样本量，共需要招募60例肥胖受试者和20例健康受试者。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Despite the fact that sample size based on efficacy calculations is more normative the development and application of related theories are very limited and there are no available calculation models or tools. Unlike general clinical trials the imaging studies of acupuncture cannot be applied the traditional sample size calculation methods. In the current situation setting the sample size directly according to the characteristics of the imaging protocol may be a more reasonable approach. The research findings show that a sample size of 12 or more per group has become a general consensus in recent acupuncture imaging studies and the estimated sample size range is mainly 20-30 per group. 20 per group has become the first choice for most studies which is consistent with the recommendation in "Acupuncture Imaging" that the single group sample size should be at least 20 when ethical and experimental conditions permit. Therefore this study will select 20 per group as the research sample size based on the recommendation in "Acupuncture Imaging" and the current experimental conditions and a total of 60 obese subjects and 20 healthy subjects will be recruited.

盲法：

由于针灸治疗存在临床特殊性，无法实施绝对标准的盲法评价，故最终由不知分组情况的第三者进行疗效评价；实行疗效评价者、操作者及统计人员三分离。对受试者、疗效评价者及统计分析人员设盲。

Blinding：

Due to the clinical specificity of acupuncture treatment it is impossible to implement an absolute standard blind method evaluation so the final effect evaluation is carried out by a third party who is unaware of the group allocation; the effect evaluation operation and statistical personnel are separated. The subjects effect evaluation personnel and statistical analysis personnel are blinded.
