New Treatment Measure Clinical Study

研究目的:

通过无创的方式对患者进行评估,评估内容或仪器包括临床评估量表、X光、MRI、膝关节三维运动参数捕捉系统、膝关节肌骨超声、肌力测量仪、表面肌电测量仪、肌肉评估仪、足底压力数据采集仪,对受试者的膝关节状态及神经肌肉控制通路进行客观、动态、量化的评估,探讨基于仰卧凌空直蹬腿/手抱琵琶半蹲踢功法治疗在ACLD及ACLR术后的生物力学作用机制。

Objectives of Study:

The patients were evaluated in a non-invasive way. The evaluation contents or instruments included clinical evaluation scale X-ray MRI knee joint three-dimensional motion parameter capture system knee joint musculoskeletal ultrasound muscle strength measuring instrument surface electromyography measuring instrument muscle evaluation instrument and plantar pressure data acquisition instrument. Objective dynamic and quantitative evaluation of the knee joint state and neuromuscular control pathway of the subjects was carried out to explore the biomechanical mechanism of the treatment of ACLD and ACLR based on supine straight leg kicking / hand holding pipa half-squat kicking.

药物成份或治疗方案详述:

Description for medicine or protocol of treatment in detail:

纳入标准:

ACLD受试者: ①年龄18-40岁; ②术前确诊为单侧ACLD损伤,且不伴侧副韧带、半月板等结构性损伤; ③损伤大于1个月,且无伴膝关节中度或者中度肿胀、行走疼痛者; ④符合有以下肌力失衡表现之一者:患侧侧股四头肌或腘绳肌峰值肌力小于对侧85%,腘绳肌/股四头肌肌力比值小于70%,不符合以上表现、但体格检查发现某肌肉严重萎缩者(如股内侧肌),韧带松紧度测量重建侧与健侧双侧差异小于3mm、但近3个月有膝关节不稳或症状病史者;术后排除炎症等原因而膝痛患者; ⑤自愿加入本试验,签署知情同意书; ACLR受试者: ①年龄18-40岁; ②术前确诊为单侧ACLD损伤,且不伴侧副韧带、半月板等结构性损伤; ③单束解剖ACLR重建,术后12个月以上; ④符合有以下肌力失衡表现之一者:重建侧股四头肌或腘绳肌峰值肌力小于对侧85%,腘绳肌/股四头肌肌力比值小于70%,不符合以上表现、但体格检查发现某肌肉严重萎缩者(如股内侧肌),韧带松紧度测量重建侧与健侧双侧差异小于3mm、但近3个月有膝关节不稳或症状病史者;术后排除炎症等原因而膝痛患者; ⑤自愿加入本试验,并且签署知情同意书; ⑥依从性好,同意配合治疗和锻炼,并且能够坚持完成整个治疗过程者。 健康受试者: ①年龄18-40岁; ②健康良好、无神经骨骼肌系统疾病、无其他系统疾病者如呼吸系统、循环系统等; ③下肢无重大创伤史或者手术史; ④自愿加入本试验,签署知情同意书; ⑤依从性好,同意配合治疗和锻炼,并且能够坚持完成整个治疗过程者。

Inclusion criteria

ACLD participants: ① Aged 18 - 40 years old; ② Pre - operatively diagnosed with unilateral ACLD injury without collateral ligament meniscus or other structural injuries; ③ Injury duration over 1 month no moderate knee swelling walking pain; ④ Having one of these muscle strength imbalances: quadriceps or hamstring peak strength on the affected side below 85% of the contralateral side hamstring/quadriceps strength ratio below 70% or severe muscle atrophy (like in the vastus medialis) found on physical examination or ligament laxity difference between reconstructed and healthy sides less than 3mm but with knee instability or symptoms in the past 3 months or post - operative knee pain not due to inflammation; ⑤ Willing to participate and sign the consent form; ACLR participants: ① Aged 18 - 40 years old; ② Pre - operatively diagnosed with unilateral ACLD injury without collateral ligament meniscus or other structural injuries; ③ Single - bundle anatomic ACLR over 12 months post - surgery; ④ Having one of these muscle strength imbalances: quadriceps or hamstring peak strength on the reconstructed side below 85% of the contralateral side hamstring/quadriceps strength ratio below 70% or severe muscle atrophy (like in the vastus medialis) found on physical examination or ligament laxity difference between reconstructed and healthy sides less than 3mm but with knee instability or symptoms in the past 3 months or post - operative knee pain not due to inflammation; ⑤ Willing to participate and sign the consent form; ⑥ Good compliance agree to cooperate with treatment and exercise and can persist in completing the entire treatment process. Healthy participants: ① Aged 18 - 40 years old; ② In good health without neuromusculoskeletal diseases or other systemic diseases like respiratory or circulatory system diseases; ③ No major lower limb trauma or surgical history; ④ Willing to participate and sign the consent form; ⑤ Good compliance agree to cooperate with treatment and exercise and can persist in completing the entire treatment process.

排除标准:

ACL损伤受试者排除标准: ①存在神经骨骼肌系统疾病或其他系统疾病者,如小儿麻痹; ②临床诊断为膝骨性关节炎或者影像学诊断为膝关节严重退变(Kellgren-Lawrence分期大于I期者); ③BMI>30kg/m2 ④未能配合试验者; ⑤临床资料缺失者。 ACL重建受试者排除标准: ①存在神经骨骼肌系统疾病或其他系统疾病者,如小儿麻痹; ②临床诊断为膝骨性关节炎或者影像学诊断为膝关节严重退变(Kellgren-Lawrence分期大于I期者); ③未能配合试验者; ④BMI>30kg/m2 ⑤临床资料缺失者。 健康受试者排除标准: ①体格检查或肌力检查异常者(如Lachman试验阳性、McMurray试验阳性、肌力萎缩等); ②BMI>30kg/m2 ③临床资料缺失; ④未能配合试验者。

Exclusion criteria:

Exclusion Criteria for ACL Injury Subjects: 1. Presence of neuromusculoskeletal disorders or other systemic diseases such as polio; 2. Clinical diagnosis of knee osteoarthritis or radiological diagnosis of severe degenerative changes in the knee joint (Kellgren-Lawrence grade greater than I); 3. BMI > 30 kg/m²; 4. Inability to cooperate with the trial; 5. Missing clinical data. Exclusion Criteria for ACL Reconstruction Subjects: 1. Presence of neuromusculoskeletal disorders or other systemic diseases such as polio; 2. Clinical diagnosis of knee osteoarthritis or radiological diagnosis of severe degenerative changes in the knee joint (Kellgren-Lawrence grade greater than I); 3. Inability to cooperate with the trial; 4. BMI > 30 kg/m²; 5. Missing clinical data. Exclusion Criteria for Healthy Subjects: 1. Abnormal findings in physical examination or muscle strength testing (e.g. positive Lachman test positive McMurray test muscle atrophy etc.); 2. BMI > 30 kg/m²; 3. Missing clinical data; 4. Inability to cooperate with the experimenter.
