研究目的：

本研究通过对比基于括约肌保护的渐进挂线术和传统切开挂线术的临床疗效，旨在为基于括约肌保护的渐进挂线术治疗经括约肌肛瘘的可行性和优效性提供临床证据。

Objectives of Study：

This study aims to provide clinical evidence for the feasibility and superiority of sphincter-protected progressive thread-drawing in the treatment of transsphincteric anal fistula by comparing the clinical efficacy of sphincter-protected progressive thread-drawing with traditional incision thread-drawing.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）符合上述经括约肌肛瘘诊断标准并接受手术治疗；符合《中医肛肠科常见病诊疗指南》中湿热下注型的诊断标准； （2）年龄在18~65岁之间的患者，男女不限； （3）直肠指诊、核磁共振等提示经括约肌肛瘘形成； （4）术前肛门外形及功能正常； （5）既往无肛瘘手术史。

Inclusion criteria

(1) It meets the diagnostic criteria of anal fistula through sphincter and receives surgical treatment; it meets the diagnostic criteria of damp-heat downward type in the Guide for Diagnosis and Treatment of Common Diseases in Anorectal Department of Traditional Chinese Medicine; (2) Patients aged between 18 and 65 years old male and female; (3) digital rectal examination and MRI suggested transsphincteric anal fistula; (4) Anus appearance and function were normal before operation; (5) No previous anal fistula surgery.

排除标准：

（1）入组病例年龄＞65岁或者＜18岁； （2）存在心、脑血管疾患，或者肝、肾及血液系统疾病； （3）因直肠骶前肿瘤、结核、克罗恩病等因素致肛瘘； （4）妊娠期或哺乳期； （5）伴有肛周湿疹、腹泻等其他疾病。 （6）精神障碍不能正常交流。

Exclusion criteria：

(1) Enrolled patients aged>65 years or <18 years; (2) There are heart and cerebrovascular diseases or liver kidney and blood system diseases; (3) anal fistula caused by rectal presacral tumor tuberculosis Crohn's disease and other factors; (4) Pregnancy or lactation; (5) accompanied by perianal eczema diarrhea and other diseases; (6) Mental disorders cannot communicate normally.
