观察清肠消澼饮对UC的临床疗效及其与肠道微生物的关系

Objectives of Study：

To explore the clinical efficacy of Qingchang Xiaopi decoction in ulcerative colitis and its association with gut microbiota

药物成份或治疗方案详述：

项目申请者所在团队前期于东莞市中医院开展了多项清肠消澼饮干预UC的临床研究，并保存有生物标本可供生物检测，本项目拟申请使用前期临床研究中收集的大肠粘膜、血液、大便标本以及相关病历资料，以肠道微生物失调为基础阐述UC大肠湿热证的生物学基础和清肠消澼饮的效应机制。所有入组患者在参加相应研究前均已提供知情同意书，并已完成给药观察、样本采集工作，因此本研究不涉及前瞻性给药。前期研究中，所有入组患者于治疗期给予清肠消澼饮中药治疗，2次/日，餐后1h口服，疗程为8周。

Description for medicine or protocol of treatment in detail：

The research team of the project applicant previously conducted several clinical studies on the intervention of ulcerative colitis with the Qingchang Xiaopi Decoction at Dongguan Hospital of Traditional Chinese Medicine. Biological specimens have been preserved for biological testing. This project intends to apply for the use of colorectal mucosal blood fecal specimens and relevant medical records collected during the earlier clinical studies to elucidate the biological basis of the Damp heat syndrome of large intestine in UC from the perspective of gut microbiota dysbiosis and to explore the effect mechanisms of the Qingchang Xiaopi Decoction. All enrolled patients had provided informed consent before participating in the respective studies and the drug administration observation and sample collection have been completed. Therefore this study does not involve prospective drug administration. In the previous studies all enrolled patients were treated with Qingchang Xiaopi Decoction administered 2 times per day 1 hour after meals for a duration of 8 weeks.

纳入标准：

① 年龄18-70 岁，男女不限； ② 符合UC 诊断标准的患者； ③ 符合UC 处于轻、中度活动期的标准； ④ 符合大肠湿热证诊断标准； ⑤ 签署知情同意书者。

Inclusion criteria

1. Aged 18-70 years with no gender restriction; 2. Diagnosed with ulcerative colitis according to established criteria; 3. Presenting mild to moderate active phase of ulcerative colitis; 4. Meeting diagnostic standards for Large Intestine Dampness-Heat Syndrome; 5. Provision of signed informed consent.

排除标准：

① 慢性血吸虫病、细菌性痢疾、肠结核等感染性结肠炎及结肠型克罗恩病、 放射性肠炎、缺血性肠炎患者； ② 哺乳期、孕妇以及对药物不耐受的患者； ③ 有严重心、肺、肝、肾和造血系统等严重原发性疾病，精神病患者； ④ 有严重并发症如局部狭窄、中毒性巨结肠、肠梗阻、结肠癌、直肠癌、 肠穿孔及肛门疾病患者； ⑤ 法律规定的残疾患者（哑，聋，盲，肢体残疾，智力障碍）。

Exclusion criteria：

1. Patients with infectious colitis (e.g. chronic schistosomiasis bacillary dysentery intestinal tuberculosis) or other intestinal disorders including Crohn's disease radiation-induced enteritis and ischemic colitis; 2. Lactating or pregnant individuals or those with medication intolerance; 3. Severe primary diseases affecting major organ systems (cardiac pulmonary hepatic renal or hematopoietic) or psychiatric disorders; 4. Severe complications such as intestinal strictures toxic megacolon bowel obstruction colorectal cancer intestinal perforation or anorectal pathologies; 5. Legally defined disabilities (e.g. mutism deafness blindness physical disabilities intellectual impairments).
