研究目的：

1.通过对膝关节炎临床诊病的观察，以中医理论为指导，不断摸索和实践总结中药的组方，探索治疗具有膝关节炎临床疗效的通络止痛外敷普方，为基层临床推广应用提供医学证据。 2.探索研究治疗骨性膝关节炎的有效新方法新技术，让我区广大膝关节炎患者更易接受和认可，在提高疗效的基础上，可改善KOA患者的健康状况，提高患者的生活质量和满意度，疗效确定后试行全区推广给广大患者带去福音。 3.研究自拟中药通络止痛外敷方结合电磁波谱理疗，集合了中医药疗、热疗、磁疗、中药离子渗透治疗疗法等多种功能, 融热度、湿度、药物浓度于一体,因病施治,起到舒筋、活络、止痛的作用, 可促进膝关节血液循环和局部炎性物质吸收,可加速局部的血液循环,减轻静脉疲滞,降低骨内压力,促进关节积液吸收,缓解肿胀及疼痛,从而起到改善膝关节结构和功能的作用。

Objectives of Study：

1.Through the observation of clinical diagnosis of knee arthritis, guided by the theory of traditional Chinese medicine, constantly explore and practice, summarize the composition of traditional Chinese medicine, and explore the Tong luo Zhi tong external application common prescription with clinical curative effect of knee arthritis, so as to provide medical evidence for the promotion and application of grass-roots clinical. 2.To explore new effective methods and technologies for the treatment of osteoarthritis of the knee, so that the majority of patients with osteoarthritis in our district can be more easily accepted and recognized. On the basis of improving the curative effect, it can improve the health status of patients with KOA, improve the quality of life and satisfaction of patients. After the curative effect is determined, it will be promoted in the whole district to bring gospel to the majority of patients. 3.Studying the self-made external application prescription of Tongluo Zhitong combined with electromagnetic spectrum physiotherapy, which integrates the functions of traditional Chinese medicine therapy, hyperthermia, magnetic therapy, ion penetration therapy of traditional Chinese medicine, and integrates heat, humidity, and drug concentration. It can relax tendons, activate collaterals, and relieve pain. It can promote blood circulation of knee joint and absorption of local inflammatory substances, and accelerate local blood circulation.It can relieve venous fatigue, reduce intraosseous pressure, promote absorption of joint effusion, relieve swelling and pain, so as to improve the structure and function of knee joint.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

1.年龄40-75岁，男女不限； 2.符合膝关节骨性关节炎诊断标准：①近1个月内反复膝关节疼痛；②膝关节晨僵时间≤30 min；③身体活动时膝关节有骨摩擦音（感）；④X线片（站立位或负重位）显示膝关节间隙变窄、软骨下骨硬化和（或）囊性变、关节边缘骨赘形成；⑤MRI示软骨损伤、骨赘形成、软骨下骨骨髓水肿和（或）囊性变、半月板退行性撕裂、软骨部分或全层缺失；（注：满足诊断标准1+2+3或1+4或1+5，可诊断KOA） 3.近期两周内未接受膝关节局部及口服药物、手术治疗的患者； 4.同意并签署知情同意书，能坚持按期治疗并配合医生的患者。

Inclusion criteria

1.Aged 40-75 years, male or female; 2.According to the diagnostic criteria of knee osteoarthritis. (1)Repeated knee pain in recent 1 month; (2)The time of morning stiffness of knee joint was less than or equal to 30 min; (3)There are bone frications in the knee joint during physical activity; (4)X-ray film (standing or weight-bearing position) showed narrowing of knee joint space, subchondral osteosclerosis and/or cystic degeneration, osteophyte formation at the edge of the joint; (5)MRI showed cartilage injury, osteophyte formation, subchondral bone marrow edema and (or) cystic degeneration, meniscal degenerative tear, partial or full-thickness cartilage loss; (Note: koa can be diagnosed if it meets the diagnostic criteria of 1+2+3or1+4or1+5) 3.Patients who have not received local or oral medication or surgery for knee joint in recent two weeks; 4.The patients who agree and sign the informed consent, can adhere to the treatment on schedule and cooperate with the doctor.

排除标准：

1.凡一切有发热症状者或施治部位有皮肤感染者； 2.严重心、肝、肾功能不全或全身衰竭者； 3.经确诊由骨结核、骨肿瘤、骨折所引起膝关节周围疼痛及活动受限者； 4.治疗中间发生膝关节部位的急性外伤者。

Exclusion criteria：

1.All patients with fever or skin infection at the treatment site; 2.Severe heart, liver and kidney insufficiency or systemic failure; 3.Patients with pain around knee joint and limited movement caused by bone tuberculosis, bone tumor and fracture; 4.Treat the patients with acute injury of knee joint.
