研究目的：

在初级卫生保健背景下，研究比较中医“辨体施膳”与常规健康管理，对1级高血压患者的临床和中医相关指标的效果。

Objectives of Study：

To compare the effectiveness of traditional Chinese medicine (TCM) constitution-based dietary intervention versus usual care on clinical and TCM-related outcomes in grade 1 hypertensive patients in the primary care setting.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

1）年龄35岁及以上； 2）临床诊断为单纯性1级高血压； 3）经评估，心血管和肾脏事件的发生风险较低； 4）接受国家基本公共卫生服务下的家庭医生团队定期随访； 5）未接受有效的高血压药物治疗； 6）未服用可能对血压水平或营养代谢产生附加影响的营养补品。

Inclusion criteria

1) Aged >= 35 years; 2) clinically diagnosed with uncomplicated, grade 1 hypertension; 3) assessed with low risk of cardiovascular and renal events; 4) on regular follow-up by 'family doctor team' in the context of the National basic public health service delivery; 5) with no active medications on hypertension; and 6) currently not having nutritional supplements that may exert additional effects on BP levels or nutrient metabolism.

排除标准：

1）患有认知功能障碍、痴呆、严重精神障碍或其他无法恰当表达意愿者； 2）患有继发性高血压或顽固性高血压者； 3）患有高血压相关靶器官损害，或高血压严重并发症，例如脑血管疾病、冠状动脉疾病、心力衰竭、外周动脉疾病、心房颤动等； 4）有家庭成员（包括亲属）参与本研究并处于不同分组； 5）曾参与需改变饮食的中医干预研究。

Exclusion criteria：

1) with symptoms of cognitive dysfunction, dementia, severe mental disorders, or other conditions with inability to appropriately convey personal thoughts; 2) secondary hypertension or resistant hypertension; 3) with a presence of hypertension-mediated organ damage (HMOD), or with severe complications of hypertension, i.e., cerebrovascular disease, coronary artery disease, heart failure, peripheral artery disease, atrial fibrillation, etc.; 4) have a family member (including relatives) who participated in the trial with a different group assignment; and 5) previously enrolled in traditional Chinese medicine (TCM) interventions requiring dietary modifications.

研究对象将遵循研究协调员采用电脑生成的随机数字，按1:1比例随机分至对照组或干预组。随机数字将采用不可预测的随机序列区组设计分组，该随机序列区组将不会被告知研究对象及实施者。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Participants will be randomly assigned to either control or intervention group with a 1:1 allocation as per computer-generated random numbers informed by the research coordinator. Permuted blocks of random sizes will be used. The block will not be disclosed to trial participants and trial implementers.

盲法：

研究结局指标将由不知晓研究对象所属组别的临床人员进行评估和数据收集。鉴于本干预的特点，虽然研究对象与营养师均不可避免地知晓分组，但将被强烈告知在照护及随访评估时，不得向临床人员透露参与的试验分组。

Blinding：

Study outcomes will be assessed and collected by clinical personnel blind to treatment allocation. Due to the nature of the intervention neither trial participants nor the dietitian can be blinded to allocation, but are strongly inculcated not to disclose the allocation status of the participant to usual care providers at the point of care and follow-up assessments.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

原始数据将以纸质形式进行收集。全部原始病例报告表将采用独立双录入的方式，由研究人员录入电脑。数据将利用电子数据管理系统进行管理和常规备份且采用密码保护，只有被授权的研究团队成员才可获取数据。密码将定期更新。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data will be initially collected on paper forms. All the original case report forms (CRFs) will be entered electronically by research personnel through independent double data entry. An electronic data curation system will be used for data management with routine backup. Data will be stored with encryption at the server which is password protected, and only the authorised research team members will be granted with access to the data. The passwords will be changed on a regular basis.
