研究目的：

（1）以提高患者生存质量、身心全面康复为目的，采用中医整体观和ICF为理论基础进行横断面调查，探索卒中后居家康复患者生存质量及其影响因素间关系，筛选卒中后居家中医康复模块中重点条目。 （2）基于互联网技术进行多元结合平台搭建，多学科团队分工管理，针对重点条目进行精准中医特色康复，构建依托信息化平台的中医康复模式。

Objectives of Study：

(1) For the purpose of improving the quality of life and comprehensive physical and mental recovery of patients, a cross-sectional survey was conducted based on the holistic view of TCM and ICF theory to explore the relationship between the quality of life of patients undergoing home rehabilitation after stroke and its influencing factors, and key items in the module of home rehabilitation of TCM after stroke were screened. (2) Build a multi-platform based on Internet technology, manage the division of labor of a multi-disciplinary team, carry out precise TCM rehabilitation based on key items, and build a TCM rehabilitation mode relying on the information platform.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

（1）初次经 CT 或 MRI 检查确诊为脑梗死和脑出血的恢复期病人； （2）脑血管疾病后 Glasgow 昏迷量表(GCS)评分>8分，患者无明显的意识障碍； （3）愿意参加该研究并签署知情同意书者； （4）出院后返家，且家庭住址在主城区范围内； （5）年龄在18～80岁的患者，自己或陪护会使用智能手机。

Inclusion criteria

(1) Convalescent patients diagnosed with cerebral infarction and cerebral hemorrhage by CT or MRI examination for the first time; (2) Glasgow Coma Scale (GCS) score after cerebrovascular disease >8, the patient had no obvious disturbance of consciousness; (3) Those who are willing to participate in the study and sign the informed consent; (4) Return home after discharge, and the home address is within the main urban area; (5) Patients aged between 18 and 80 use smart phones by themselves or their escorts.

排除标准：

（1）出院时昏迷； （2）伴有其他危重疾病，如恶性肿瘤、获得性免疫缺陷综合征、心功能衰竭、肾功能衰竭、呼吸功能衰竭、肝功能衰竭、严重外伤、大脑皮层损害者； （3）既往有认知障碍或精神疾患病史者； （4）住址迁移性较大，联系方式不固定者； （5）不符合上述诊断标准。

Exclusion criteria：

(1) Coma upon discharge; (2) patients with other critical diseases, such as malignant tumor, acquired immune deficiency syndrome, heart failure, renal failure, respiratory failure, liver failure, severe trauma, or cerebral cortex damage; (3) those who have a history of cognitive impairment or mental disorders; (4) Domicile transfer is large, and contact information is not fixed; (5) Does not meet the above diagnostic criteria.
